Serum magnesium level and hematoma expansion in patients with intracerebral hemorrhage

Spontaneous intracerebral hemorrhage (ICH) is a devastating subtype of stroke that results in significant rates of mortality and morbidities. The initial hematoma volume, hematoma expansion (HE), blood pressure (BP), and coagulopathy are considered strong predictors of clinical outcomes and mortality. Low serum magnesium (Mg++) levels have been shown to be associated with larger initial hematoma and greater HE. Coagulopathy, platelet dysfunction, high BP, and increased inflammatory response might form the mechanistic link between low serum Mg++ levels, larger hematoma size and greater HE. However, randomized clinical trials administering intravenous Mg++ have shown no benefit over placebo in ICH patients. The confounding effect of hypocalcemia and a delay in Mg++ trafficking across the blood-brain barrier might explain the futile results for intravenous Mg++ therapy. In the current review, we will discuss the evidence regarding the possible role of low serum Mg' level on HE in acute ICH.

Summary: This study aimed to verify the morphological and histological criteria of fatal cases of PAH and evaluate the lymphocytic populations associated with lesions. The results showed that inflammatory infiltrate is essential for a correct diagnosis of PAH and may play a key role in the pathogenesis. This finding is crucial for ruling out secondary pulmonary hypertension and adding new insights to the immunologically related pathogenesis of PAH.

A study found that the neutrophil-to-lymphocyte ratio (NLR) at admission in AIS patients undergoing endovascular treatment (EVT) is associated with early neurological deterioration (END). Higher NLR values predicted a higher risk of END.

Treatment with Cenobamate in Adult Patients with Lennox-Gastaut Syndrome: A Case Series

Monomeric C-reactive protein: A novel biomarker predicting neurodegenerative disease and vascular dysfunction

Summary: Circulating C-reactive protein (pCRP) concentrations significantly increase during acute and chronic infection and disease, playing a role in complement fixation. It is now known that pCRP dissociates into the monomeric form (mCRP) when exposed to activated immune cells or damaged tissue, becoming biologically active. Studies have shown that mCRP is distributed within the brain tissue of individuals with neuroinflammatory disease and has been linked to neurovascular dysfunction, blood brain barrier compromise, and increased risk of dementia. Future therapeutic options aim to inhibit pCRP-LPC mediated dissociation and prevent associated damage.

Perampanel in Brain Tumor-Related Epilepsy: A Systematic Review

Summary: Brain tumor-related epilepsy (BTRE) is a common comorbidity in patients with brain neoplasms. Glutamate receptors antagonist Perampanel (PER) may have therapeutic potential in reducing tumor growth and controlling epileptiform activity. Both pre-clinical and clinical studies suggest that PER can be an effective and well-tolerated option for patients with BTRE, but more research is needed for further understanding of its effects on tumor progression.
