Effectiveness and safety of topical tacrolimus monotherapy for repigmentation in vitiligo: a comprehensive literature review

Thus far, several small studies and case reports on the use of topical immunomodulators in vitiligo have been published. We undertook a comprehensive literature review, searching for studies evaluating clinical response to tacrolimus topical therapy for vitiligo. A search was performed on PubMed/Medline using the term vitiligo, combined with topical and ointment. Our inclusion criteria were: use of tacrolimus ointment as monotherapy to treat vitiligo. We found 29 studies from 2002 to 2014. Overall, 709 patients were treated in 29 studies. Pooling the lesions, 50% repigmentation of vitiligo patches was never achieved before 2 months of treatment, with a peak after 6 months of therapy. The best results were obtained on lesions of the cephalic region, especially the face, with tacrolimus 0.1% ointment two times daily. The percentage of non-responsive patients ranged from 0% to 14%. Treatment was generally well-tolerated; only localized adverse effects were reported. Our objective was to verify the effectiveness and safety of tacrolimus ointment monotherapy. It has good efficacy and tolerability. At present, only small trials and case series are available in the literature. Further, standardized investigations on a larger number of patients are needed.

Return to Work after Breast Reduction: A Comparative Study

Summary: The aim of this study was to assess the return to work for women with breast hypertrophy after reduction mammaplasty. The occurrence of postoperative complications increased the patients' return to work time, while comorbidities and preoperative parameters did not affect the length of sick leave. Based on individual variations, a recovery period of approximately 3 weeks is suggested.

Extensive skin necrosis following total hip arthroplasty performed through the direct anterior approach

Summary: Hypertensive disorders of pregnancy affect a significant number of pregnant women globally and has serious implications for fetal and maternal health. This study found evidence of poor pregnancy outcomes in patients with blood pressure below the accepted cut-off of 140/90 mmHg, indicating the need for interventions to reduce the risk of hypertensive disorders in pregnancies with blood pressures ranging from 120-139/80-89 mmHg.

Spiradenocarcinoma: SEER Study of Epidemiology, Survival, and Treatment Options

Summary: This study analyzed the demographic and pathological characteristics, treatment patterns, and survival outcomes of patients with spiradenocarcinoma. The results showed that spiradenocarcinoma affects males and females equally, with low rates of regional and distant invasion. Surgical excision is the main treatment modality, and the overall and disease-specific survival rates are high for patients with spiradenocarcinoma.

Maternal Outcomes Following Active vs. Expectant Management of Previable Preterm Pre-Labor Rupture of Membranes: A Meta-Analysis

Summary: The diagnosis of previable preterm pre-labor rupture of membranes (PROM) is associated with poor outcomes for both the mother and the fetus. A meta-analysis was conducted to compare the effects of active management and expectant management on maternal risks. The results showed no significant differences in maternal risks between active management and expectant management for previable preterm PROM at <24 weeks.

Association between first trimester platelet to lymphocyte ratio and stillbirth

Summary: This study explored the association between neutrophil to lymphocyte ratio (NLR) and platelet to lymphocyte ratio (PLR) measured during pregnancy, and the occurrence of stillbirth (SB). The study found that a PLR higher than 156.4 in the first trimester can reliably exclude the possibility of SB later during pregnancy. Lower platelet and higher lymphocyte levels may be related to an early inflammatory process. Larger studies are needed to confirm the results.
