Summary: This study examined the filter settings of 175 kidney transplant centers in 2007 and 2019, and found that centers became more open to receiving offers over time. Centers with more open filter settings had higher transplant volume and offer acceptance ratios, but there was geographic heterogeneity in the distribution of these centers.

Summary: Deceased donor kidney allocation allows exceptions in special circumstances, but these exceptions are often concentrated in a few outlier centers. The kidneys involved in the exceptions are usually transplanted to larger centers with higher acceptance rates.

Summary: The study found that community members prioritize equity in kidney allocation, while healthcare professionals focus on maximizing utility. There are discordant views between community members and healthcare professionals, which should be taken into consideration in the design, evaluation, and implementation of deceased donor kidney allocation protocols.

Summary: The kidney donor risk index (KDRI) and kidney donor profile index (KDPI) provide continuous measures of donor quality. Through studying the impact of high KDPI labeling, it was found that the high KDPI label is associated with lower kidney utilization and increased discard rates.

Summary: Kidney procurement and discard rates from deceased donors with AKI have increased from 2010 to 2020, especially for stage 3 AKI donors. Despite the increase in procurement of stage 3 AKI kidneys, high discard rates suggest room for improvement in utilization.

Summary: The study findings suggest that using older grafts from donors aged 70 years and above can lead to acceptable outcomes in a highly selected cohort at experienced transplant centers. Advanced age should not be considered an absolute contraindication for utilizing DCD grafts from donors aged 70 years and older.

Summary: Expanding donor acceptability criteria to include nonstandard donors could be a potential solution to the organ shortage issue. Despite higher costs per organ, kidney transplantation from nonstandard donors remained cost-effective based on contemporary US data, with economies of scale observed in high OPO volume leading to lower costs.

Summary: This study investigated the impact of local market competition and/or organ availability on kidney procurement and utilization. The results showed that lower competition was associated with higher export, while market monopoly was weakly associated with lower discard. Higher organ availability was associated with both export and discard.

Summary: Lung transplantation from donors after circulatory death is now seen as a viable alternative to traditional brain death donation. The emergence of TA-NRP as a technique for cardiac allograft procurement from circulatory death cases allows for in situ organ assessment, potentially impacting lung allograft quality. However, successful cases of transplantation using this method have been reported.

Summary: The study found that there was no significant change in the percentage of eligible donors accepted during the accelerated placement project, but the conversion rate from acceptance to transplant slightly increased. Waiting to accelerate placement after kidneys have been declined by multiple transplant programs may be too late to effectively increase utilization. Future research should continue to investigate how to improve transplant rates of nationally shared and marginal kidneys.

Summary: The article points out the issues of fairness in kidney transplantation, focusing on the healthcare system features that disadvantage Black patients and inadequate medical standards. It proposes several initiatives to improve equity, including standardized wait-listing eligibility criteria, increased education for clinicians, innovations in electronic medical record capabilities, and advocating for rapid listing of patients. If these initiatives fail, consideration should be given to eliminating preemptive wait-listing and transplantation with deceased donor kidneys.

Summary: The volume of abdominal organ offers received by the Baylor Simmons Transplant Institute has increased over time, resulting in a higher workload for the donor call team. The study collected data on organ offers and analyzed the increase in workload for the transplant center. The results showed an increase in organ offers but stable transplant numbers, indicating an inefficiency in organ allocation and increased workload for the center.

Summary: Inclusion of blood group- and human leukocyte antigen-compatible donor and recipient pairs (CPs) in kidney paired donation (KPD) programs can increase living donor transplantation. However, using the Living Donor Kidney Profile Index (LKDPI) to promote CP participation is not effective in discriminating death-censored graft survival (DCGS) between living donors. The addition of LKDPI to recipient-based models does not significantly improve discrimination, and in prognosis-matched pairs, LKDPI has no better discriminatory ability for DCGS than chance alone.

Summary: Studies have shown that kidney transplant recipients have lower immune responses to mRNA coronavirus vaccines, especially in those recently transplanted. Simple humoral and cellular monitoring is recommended to determine the need for repeated doses. Overall, a significant portion of healthy controls, dialysis patients, and kidney transplant recipients developed some form of immune response to SARS-CoV-2.

Summary: Frailty is common among advanced CKD patients who are kidney transplant candidates, and its prevalence may vary according to different scales. The study found that the agreement between the Physical Frailty Phenotype (PFP) and FRAIL scale was not good, suggesting that the FRAIL scale may misclassify frail/pre-frail patients as robust.

Summary: Frailty is associated with poorer outcomes among patients waiting for kidney transplantation. This study evaluated the predictive value of the Physical Frailty Phenotype (PFP) and the FRAIL scale in KT candidates. The results showed that patients who did not receive a transplant were more frequently frail according to PFP. Robust patients had fewer hospital admissions and cardiovascular/major infectious events. PFP scoring had an impact on patient survival and chance of transplantation, while FRAIL scale did not show any association.

Summary: Antibody-mediated rejection is a major cause of long-term graft loss in kidney transplant patients, and patients receiving anti-thymocyte globulin (ATG) treatment showed an increase in T follicular helper (Tfh) cells at 6 months post-transplant. In mouse experiments, ATG-treated animals had an expansion of Tfh cells, increased germinal center B cells, and higher levels of antigen-specific antibodies. ATG depletion favors Tfh expansion, enhancing antibody-mediated response.

Summary: Antibody-mediated rejection (ABMR) plays a significant role in allograft loss after kidney transplantation. This study investigates the activation of the mTOR pathway and its modulation by mTOR inhibitors in ABMR with donor-specific HLA-antibodies (DSA). Summary: This study aimed to evaluate the performance of the phase angle (PhA) as a screening tool for malnutrition in patients with advanced chronic kidney disease (CKD) waiting for kidney transplantation (KT). The study found that a PhA threshold of <= 4.85 degrees had a sensitivity of 72.7%, specificity of 65.9%, and positive and negative likelihood ratios of 2.13 and 0.41, respectively. However, when compared to the Global Leadership Initiative for Malnutrition (GLIM) criteria, the PhA <= 4.85 degrees only showed fair validity for detecting malnutrition in this population.
