Emerging vistas in theranostic medicine

Recent years have witnessed a paradigm shift in the focus of healthcare towards development of customized therapies which cater to the unmet needs in a myriad of disease areas such as cancer, infections, cardiovascular diseases, neurodegenerative disorders and inflammatory disorders. The term 'theranostic' refers to such multifunctional systems which combine the features of diagnosis and treatment in a single platform for superior control of the disease. Theranostic systems enable detection of disease, treatment and real time monitoring of the diseased tissue. Theranostic nanocarriers endowed with multiple features of imaging, targeting, and providing on-demand delivery of therapeutic agents have been designed for enhancement of therapeutic outcomes. Fabrication of theranostics involves utilization of materials having distinct properties for imaging, targeting, and programming drug release spatially and temporally. Although the field of theranostics has been widely researched and explored so far for treatment of different types of cancer, there have been considerable efforts in the past few years to extend its scope to other areas such as infections, neurodegenerative disorders and cardiovascular diseases. This review showcases the potential applications of theranostics in disease areas other than cancer. It also highlights the cardinal issues which need to be addressed for successful clinical translation of these theranostic tools.

Synthesis and In Vitro Evaluation of Inherent Properties of L-Glutamic Acid Based Dendritic Lipopeptide Oligomers

Summary: This study reports the synthesis, characterization, and in vitro evaluation of dendritic lipopeptide oligomers with different surface properties. The synthesized compounds demonstrated the potential to form self-assembled polymeric nanoparticles and surfactant-like properties. A compound with a hydroxyl terminus showed hints of anticancer activity. These molecules have potential applications as drug delivery materials and warrant further investigation.

Summary: Rifampicin-loaded bovine serum albumin nanoparticles were prepared using a desolvation method with EDC as the cross-linking agent, showing a biphasic in vitro release pattern. The formulation demonstrated suitability for pulmonary delivery and the potential for reducing the dose required for tuberculosis treatment.

Summary: The nasal route of drug administration has been used for therapeutic purposes due to its easy access and non-invasive nature. It is preferred over other invasive approaches for treating CNS ailments. However, there are challenges in delivering drugs via the nasal route, such as mucociliary clearance and dose limitations. This review article discusses approaches to overcome these difficulties and enhance drug concentration in the brain.

Emerging avenues for the management of cerebral malaria

Summary: Cerebral malaria (CM) is a deadly complication of Plasmodium falciparum infection. The complex pathogenesis of CM and the challenges associated with its treatment require a multi-faceted approach. Resistance to current therapies, difficulties in delivering drugs to the brain, and the need to prevent oxidative stress and neuroinflammation are major obstacles in the treatment of CM. Research is focused on developing compounds that can target neuroinflammation, brain-targeted nanocarriers for drug delivery, and using a combination of antimalarial and anti-inflammatory or immunomodulatory molecules for CM treatment. Evaluation protocols for new therapies should include monitoring of neurological parameters alongside parasite load and survival.

Banana Peel Waste: An Emerging Cellulosic Material to Extract Nanocrystalline Cellulose

Summary: This study successfully extracted and prepared nanocrystalline cellulose (NCC) from banana peel powder (BPP). Under optimal conditions, the yield of NCC was 29.9%, with a particle size of 209 nm and a zeta potential of -43 mV. Characterization techniques including infrared spectroscopy, scanning electron microscopy, transmission electron microscopy, and X-ray diffraction analysis confirmed the purification and crystal structure of NCC. Thermogravimetric analysis showed good stability of NCC.

Application of quality by design approach to examine the effect of nanocrystalline cellulose extracted from agricultural waste as a disintegrant

Summary: Nanocrystalline cellulose, with its unique physical and biological properties, has gained attention from researchers. The utilization of agricultural waste, such as sugarcane bagasse and banana peels, for the production of nanocrystalline cellulose can help protect the environment. Researchers optimized the concentration of these nanocrystalline cellulose materials as disintegrants in the preparation of Diclofenac potassium IR tablets. The tablets showed promising results and can be explored as a new disintegrant for solid oral dosage forms.

Bimatoprost-loaded lipidic nanoformulation development using quality by design: liposomes versus solid lipid nanoparticles in intraocular pressure reduction

Summary: This study developed liposomes and solid lipid nanoparticles for the delivery of Bimatoprost, a drug used to lower intraocular pressure in glaucoma treatment. The optimized liposomes showed better encapsulation efficiency and drug kinetics compared to solid lipid nanoparticles, and when loaded in a thermo-sensitive hydrogel, they effectively decreased intraocular pressure for 18 hours.

3D human foreskin model for testing topical formulations of sildenafil citrate

Summary: This study aimed to investigate the loading of sildenafil citrate in three commercial transdermal vehicles using 3D full-thickness skin equivalent and compare the results with permeability experiments using porcine skin. The results showed that the results obtained using the 3D skin equivalent were comparable to those obtained using porcine skin, suggesting that the 3D skin model can be a valid alternative for ex-vivo skin absorption experiments.

Large volume subcutaneous delivery using multi-orifice jet injection
