Preparedness and attitudes towards medical emergencies in the dental office among Polish dentists

Background Medical emergencies in dental offices are considered a problem in most countries owing to dentists' concerns about emergency preparedness, practical skills, lifesaving equipment and staff availability. The prevalence of medical emergencies in dental offices and dentists' preparedness have been analysed in several countries but have never been studied in Poland. Aim To assess the prevalence of medical emergencies in dental offices in Poland, as well as dentists' preparedness and attitudes towards medical emergencies. Methods An 18-item questionnaire was completed by 419 dentists. It asked for information on their cardiopulmonary resuscitation training, availability of emergency medical equipment in the dental office, prevalence of medical emergencies and self-assessed competence in various dental office emergencies. Data were analysed using the Statistica 13.3 software package. Variability was measured with standard deviation. Pearson's linear correlation coefficient was used to determine correlation strength. Results The most common medical emergencies in Polish dental offices were vasovagal syncope (46.30% of study participants experienced at least one case in the preceding 12 months), orthostatic hypotension (18.85%), hyperventilation crisis (18.61%), mild allergic reactions (16.23%), hypoglycaemia (15.99%) and seizures (11.81%). The prevalence of medical emergency situations requiring an emergency medical service call or medical assistance within the preceding 12 months was 0 for 80.66% of dentists, one for 12.65%, two for 4.53%, three for 1.20%, four for 0.48%, 5-10 for 0.48% and more than 10 for 0%. As many as 41.29% of the participants did not feel competent in managing sudden cardiac arrest, 74.47% in managing hypertensive crisis, 55.61% in managing asthma, 55.13% in managing anaphylactic shock and 52.99% in managing seizures. Conclusion The prevalence of medical emergencies in dental offices in Poland is comparable with that in other countries. A large number of dentists do not feel competent enough to manage medical emergencies. Better undergraduate and postgraduate training in medical emergencies is recommended, as well as broader availability of emergency medical equipment in the dental office.

A comprehensive review and meta-analysis of suPAR as a predictor of acute kidney injury

Biomarkers for Pregnancy Latency Prediction after Preterm Premature Rupture of Membranes-A Systematic Review

Summary: Preterm premature rupture of membranes (PPROM) leading to preterm birth has significant implications for neonatal and maternal health. This systematic review examined existing data on biomarkers that can predict the duration of pregnancy following PPROM. Several biomarkers, including IL-6, IL-8, CRP, IL1RA, and others, showed a positive predictive value for timing of delivery. Proinflammatory biomarkers, particularly IL-6 or CRP, were found to be the most correlated with delivery timing, regardless of intrauterine infection.

Ventilation with the esophageal-tracheal Combitube during general anaesthesia: assessing complications in 540 patients

The study found that the ETC can be used for short procedures requiring general anaesthesia, but there is a high rate of minor complications. The study suggests adhering to recommended cuff volumes, having experience with the ETC, and limiting its use to surgeries lasting less than 2 hours to reduce the rate of complications.

The Vitamin D Serum Levels in Pregnant Women Affected by COVID-19: A Systematic Review and Meta-Analysis

Summary: Vitamin D deficiency is common in pregnant women with COVID-19 and is strongly correlated with the severity of the illness. Vitamin D serum levels are connected with the risk and severity of COVID-19 in pregnant women. Adequate vitamin D supplementation during pregnancy is suggested due to its correlation with COVID-19 symptoms and occurrence.

Systematic Review and Meta-Analysis of Efficiency and Safety of Double-Lumen Tube and Bronchial Blocker for One-Lung Ventilation

Summary: One-lung ventilation is used in various types of surgeries, and the comparison between double-lumen tubes (DLT) and bronchial blockers (BB) is still inconclusive. This study aimed to assess the outcomes of lung collapse and other parameters in DLT and BB groups. The results showed that DLT had a lower malposition rate and shorter placement time and lung collapse time compared to BB. However, DLT was associated with a higher risk of hypoxemia, hoarseness, sore throat, and bronchus/carina injuries. Larger multicenter trials are needed for definitive conclusions.

Sodium-Glucose Cotransporter 2 Inhibitors to Decrease the Uric Acid Concentration-A Novel Mechanism of Action

Summary: Sodium-glucose cotransporter 2 inhibitors (SGLT2is) are glucose-lowering agents that have a positive impact on cardiovascular risk. They influence lipid profile, blood pressure, atherosclerosis risk, hemoglobin level, and insulin resistance, and also reduce cardiovascular events, all-cause mortality, and hospitalization rates. The reduction of serum uric acid (SUA) concentration with SGLT2is may contribute to these effects, but the mechanisms are still unknown.

The Impact of COVID-19 on Carotid-Femoral Pulse Wave Velocity: A Systematic Review and Meta-Analysis

Summary: This study investigated the effect of COVID-19 on carotid-femoral pulse wave velocity (cfPWV) as a measurement to evaluate its impact on arterial stiffness and might help predict COVID-19-related cardiovascular (CV) complications. The results showed a strong association between COVID-19 infection and increased cfPWV, illustrating the cardiovascular implications of COVID-19. However, further research is needed to better understand the connection between COVID-19 infection, arterial compliance, and subsequent CV events.

ETView SL versus Macintosh Direct Laryngoscope for Endotracheal Intubation Amid Simulated COVID-19 Cardiac Arrest: A Randomized Crossover Study

Summary: This study assessed the impact of personal protective equipment on intubation success rates, time to intubation, and glottic visualization. The results showed that ETView outperformed MAC in terms of first-pass success rate, initiation time, and glottic visualization, especially when PPE was used. PPE had little impact on ETView's performance but negatively affected the Macintosh laryngoscope.
