Summary: This study defined and validated asthma subtypes using large longitudinal primary care electronic health records (EHRs). The results showed that asthma subtypes were primarily defined by the level of steroid use, level of healthcare utilization, and the presence of comorbidities. This has important clinical implications for defining asthma subtypes, facilitating patient stratification, and developing more personalized monitoring and treatment strategies.

Summary: The COVID-19 pandemic has highlighted the importance of managing chronic respiratory conditions and differentiating between asthma attacks and COVID-19 infection. Well-managed asthma is unlikely to be a risk factor for severe COVID-related outcomes and may even have a protective effect. This article provides practical advice on the recognition, classification, treatment, and continuity of care for asthma attacks, particularly during a pandemic. Asthma attacks carry a significant morbidity and mortality burden and require assessment and management in primary care.

Summary: The study aimed to develop and validate a primary care frailty index (PC-FI), based on health records, to detect and quantify frailty in older primary care patients. The PC-FI was developed using data from a database in Italy and validated in a Swedish cohort. The PC-FI showed an association with mortality and hospitalization, as well as good discriminative ability. The study highlights the importance of identifying frailty in primary care settings.

Summary: This study explores pharmacists' views and recommendations on pragmatic collaboration models between General Practice Pharmacists (GPPs) and general practitioners for asthma management services. Twenty-five interviews revealed challenges in current asthma management practices and practicality concerns regarding implementing GPP models. Major barriers include pharmacists' time constraints, workload, and patients' reluctance to seek assistance, while facilitators include inter-professional communication channels and funding considerations.

Summary: This study examined the real-world engagement of patients with chronic pain in urban and rural settings in Ontario, Canada using the Manage My Pain app alongside standard care. Results showed that patients who used the app had lower anxiety and decreased pain catastrophizing compared to those who did not engage with the app. Further research is needed to understand factors that impact continued engagement and clinical outcomes in patients with chronic pain.

Summary: This study analyzed the effects of amiodarone and lidocaine on the return of spontaneous circulation (ROSC) in relation to the time of treatment for patients with out-of-hospital cardiac arrest. The probability of ROSC decreased as time to drug administration increased, and the effect of amiodarone to restore ROSC declined with longer times to drug administration.

Summary: The FIT After Surgery study aims to investigate and characterize the incidence, trajectories, risk factors, and impact of new significant disability after non-cardiac surgery in older adults. This will be assessed using the WHO Disability Assessment Scale and participants' level of care needs at multiple time points up to 1 year post-surgery. The study is expected to provide valuable insights into postoperative recovery outcomes that are more patient-centered and comprehensive.

Summary: The objective of this scoping review is to map the evidence on clinical tools to assess functional capacity prior to elective non-cardiac surgery. Functional capacity is a strong prognostic indicator before surgery, which can be used to identify patients at elevated risk of postoperative complications, yet, there is no consensus on which clinical tools should be used to assess functional capacity in patients prior to non-cardiac surgery.
