Summary: This multicenter international study aimed to describe the outcomes of fenestrated-branched endovascular aortic repairs (FB-EVAR) in patients treated for chronic post-dissection thoracoabdominal aortic aneurysms (PD-TAAAs). The study found that FB-EVAR was associated with high technical success and a low rate of mortality and disabling complications at 30 days. However, patient survival at 5 years was low, likely due to significant comorbidities. The freedom from secondary interventions at 5 years was 44%, highlighting the need for continued patient surveillance.

Summary: The early experience with endografts with inner branches for complex abdominal aortic aneurysms and thoracoabdominal aortic aneurysms (TAAAs) shows high technical success with acceptable short-term branch-related reintervention rates. Further long-term follow-up is needed to determine the durability of this technology.

Summary: The study found that patients with TAAA in the United States have significantly lower mortality and morbidity when operated on at high-volume centers compared to low-volume centers, with the difference being consistent across elective and urgent/emergent surgeries. Increasing TAAA volume was associated with increased use of distal aortic perfusion.

Summary: Despite the development of fenestrated and branched endovascular aortic repair, the surgical management of thoraco-abdominal aortic aneurysms remains challenging in Switzerland. A study analyzed hospital incidence and mortality rates of TAAAs patients using nationwide administrative discharge data. The use of f/bEVAR in TAAA treatment increased significantly over the observed period, and it was associated with lower mortality rates for ruptured aneurysms.

Summary: This study investigated the long-term outcomes of patients treated with fenestrated and branched endovascular aneurysm repair (F-BEVAR) or open surgical repair (OSR) for complex abdominal aortic aneurysms (c-AAAs). No significant differences were found in overall and aneurysm-related mortality between the two treatment groups, but chronic renal decline was higher after OSR and the reintervention rate was higher in the F-BEVAR group.

Summary: This study investigated endovascular repair techniques for ruptured suprarenal and thoracoabdominal aortic aneurysms. In situ fenestration (ISF) was found to be the most commonly used technique, associated with improved survival in hypotensive patients. However, postoperative complications and reinterventions were still common, requiring further validation and assessment.

Summary: This study investigated aortic deformation during EVAR and complex EVAR and its association with preoperative anatomy and postoperative outcomes. The results showed consistent aortic deformation in the visceral segment during EVAR, but unpredictable deformation at the iliac bifurcation. However, aortic-iliac deformation was not related to adverse perioperative outcomes, branch stability, or reinterventions during short-term follow-up.

Summary: This study quantifies the displacement of target vessels during fenestrated and branched endovascular aneurysm repair caused by stiff guidewires and stent graft delivery systems. The introduction of such devices leads to significant, non-uniform displacement of the ostium of visceral and renal target vessels, affecting the accuracy of preoperative CTA roadmaps.

Summary: The study found that children with asthma have lower levels of DUSP1, but after exposure to farm dust stimulation, their DUSP1 expression is upregulated, and inflammatory MAPK is downregulated at the gene and protein level. The farm environment regulates inflammatory MAPK in various immune cell types, contributing to anti-inflammatory characteristics.

Summary: Pleural effusions occur in around 17.9% of patients with TBAAD, typically in older patients with more comorbidities. The presence of PEff is associated with higher in-hospital mortality, increased rates of complications, and decreased long-term survival. However, PEff is not independently predictive of worse outcomes based on propensity-matched analysis, but may serve as an early surrogate marker for identifying higher-risk patients.

Summary: This study aimed to evaluate the potential of proteoglycans and collagens as serological biomarkers in abdominal aortic aneurysm (AAA) and to investigate the underlying mechano-biological interactions and signaling pathways. The results showed that collagen and proteoglycans levels were significantly increased in AAA patients compared with controls, and serum levels of collagen and endocan were also elevated in AAA patients. Tissue stiffness and certain serum markers were correlated with mechanical properties in AAA patients. The signaling pathways in AAA were associated with extracellular matrix synthesis and vascular smooth muscle cell proliferation.

Summary: The arterial baroreflex plays a crucial role in regulating blood pressure through the sensing of pressure changes by baroreceptors and transmitting signals to the central nervous system. However, the molecular nature of baroreceptors and the mechanisms of mechanosensory transmission in the aortic arch are still not fully understood. Furthermore, the impact of aortic pathologies on baroreceptor function, especially in aortic dissection, remains unknown.

Summary: A study in Switzerland found that there is an association between inter-hospital transfer and hospital mortality in patients with ruptured abdominal aortic aneurysms (rAAA). Analysis of hospital discharge data from 2009 to 2018 showed that patients who were directly treated at the hospital of first presentation had a higher mortality rate compared to those who were transferred between hospitals.

Summary: This study investigates the association between the proximal landing zone and its morphology with long-term outcomes in patients with TBAD or IMH. The results showed that landing in zone 2 was associated with higher rates of reinterventions, spinal cord ischemia, and pSINE compared to landing in zone 3. Landing in dissected segments did not impact the outcomes.

Summary: This study suggests a potential association between oral anticoagulation therapy and increased risk of late aortic events in patients with conservatively managed acute type B aortic dissection. It emphasizes that type B aortic dissection should not be the primary indication for oral anticoagulation, and patients on anticoagulation for other indications need close monitoring.
