Diagnostics and treatment of acromegaly - updated recommendations of the Polish Society of Endocrinology

Acromegaly is a rare disease caused by excessive production of growth hormone (GH), typically by a pituitary tumour. The diagnosis is usually delayed, and patients frequently develop various complications that cause premature mortality. In patients with hypertension, heart failure, diabetes, and arthropathies that are not age-specific, attention should be paid to signs of acromegaly. Insulin-like growth factor 1 (IGF-1) assay should be used as a screening test whenever acromegaly is suspected. Further diagnostic investigations and treatment should be carried out at specialist centres. First-line treatment involves selective excision of pituitary adenoma using transsphenoidal access. Patients with chances of cure with surgical removal of the pituitary tumour should be referred to centres that have experience in this type of procedure, following pharmacological preparation. Other patients, as well as patients after failed neurosurgical treatment, should first receive chronic treatment with first-generation somatostatin analogues. For second-line treatment, pasireotide, pegvisomant, cabergoline, or combinations thereof should be considered. In every case, acromegaly sequelae require life-long monitoring and active treatment. Current recommendations, being an updated version of the recommendations published in Endokrynologia Polska in 2014, which take into account the Polish situation, should prove useful in the management of patients with acromegaly.

Summary: It is found that vitamin D deficiency is prevalent among the Polish population based on epidemiological studies. To address this concern, a Polish multidisciplinary group reviewed the evidence and formulated recommendations for prevention and treatment of vitamin D deficiency. The final Polish recommendations included a range of serum vitamin D levels indicating deficiency, guidelines for prophylaxis and treatment, and the importance of improving the overall population's vitamin D status and treating at-risk patient groups.

Specific Alteration of Branched-Chain Amino Acid Profile in Polycystic Ovary Syndrome

Summary: Polycystic ovary syndrome (PCOS) is a common endocrinopathy in women of reproductive age. A study investigated the levels of branch chain amino acids (BCAA) in women with PCOS and found significantly higher levels of BCAA compared to healthy controls. These elevated BCAA levels were associated with metabolic alterations and other disturbances in PCOS patients. The findings suggest that BCAA profiles could be used as a predictive factor for PCOS-related complications.

Newly Diagnosed Monostotic Paget's Disease of Bone during Living Kidney Donor Candidate Evaluation

Summary: The popularity of living-donor organ donation has increased recently due to the growing need for organs and a shortage of deceased-donor organs. This procedure requires a comprehensive health assessment of candidates, who must be in good physical and mental health. We present a case of a potential living-kidney donor who was withdrawn from donation due to Paget's disease of bone, and various imaging and assessment techniques were used to thoroughly evaluate the affected vertebra.

Frequency of COVID-19 Infection as a Function of Vitamin D Levels

Association between Biomarkers (VEGF-R2, VEGF-R3, VCAM-1) and Treatment Duration in Patients with Neuroendocrine Tumors Receiving Therapy with First-Generation Somatostatin Analogues

Summary: This study aimed to assess the changes in angiogenic factors (AF) serum levels in neuroendocrine tumour (NET) patients treated with prolonged-acting somatostatin analogues (SSAs). The concentrations of vascular endothelial growth factor receptors 2, 3 (VEGF-R2, VEGF-R3), and vascular cell adhesion molecule-1 (VCAM-1) showed significant differences depending on the duration of SSA treatment. However, monitoring these biomarkers did not help in assessing the effectiveness of SSA treatment in NET patients.

Is H19 RNA a Useful Marker of Acromegaly and Its Complications? A Preliminary Study

Summary: This study investigated the expression of H19 RNA in acromegaly patients and found that it is not relevant for the diagnosis of acromegaly. Acromegaly patients have a higher risk of hypertension, goitre, and cholelithiasis, with cholelithiasis being associated with higher H19 RNA expression.

The Influence of SARS-CoV-2 Infection on the Thyroid Gland

Summary: It is important to understand the impact of COVID-19 on the thyroid gland and the relationship between thyroid status and SARS-CoV-2 severity. The virus uses angiotensin-converting enzyme-2 as a receptor and can cause organ failures through a cytokine storm. Proinflammatory molecules are elevated in COVID-19 patients, but there is no evidence of a direct impact of thyroid dysfunctions on disease severity.

The authors discuss 10 troublesome questions in everyday clinical practice based on their extensive experience. Multidisciplinary decision-making is still necessary for the treatment of NF-Pan-NEN.

Assessment of the Role of Leptin and Adiponectinas Biomarkers in Pancreatic Neuroendocrine Neoplasms

Summary: This study investigated the potential relationship between leptin, adiponectin, and the leptin-adiponectin ratio with the grade, stage, and metastases of PanNENs. It was found that adiponectin concentration was significantly higher in the control group compared to the study group, and both leptin and adiponectin levels were higher in females than males. An increased leptin-adiponectin ratio was observed in well-differentiated PanNENs and PanNENs with low Ki-67. Additionally, lower leptin levels and a decreased leptin-adiponectin ratio were found in PanNENs with distal disease. These findings suggest that leptin, adiponectin, and their ratio may serve as potential biomarkers for the diagnosis, prognosis, and prediction of PanNENs.

FTO Gene Polymorphisms and Their Roles in Acromegaly

Summary: The study reveals that the polymorphisms of the FTO gene are associated with lipid metabolism and an increased risk of cardiovascular diseases in patients with acromegaly. Furthermore, these polymorphisms also have an impact on IGF-1 synthesis. Larger scale investigations are needed for more precise evidence.

Assessment of Osteoporosis and Vitamin D3 Deficiency in Patients with Idiopathic Benign Paroxysmal Positional Vertigo (BPPV)

Summary: The aim of this study was to assess the prevalence of osteoporosis and 25(OH) vitamin D3 deficiency in patients with idiopathic BPPV. The results showed no significant relationships between age, BMI, or vitamin D3 levels and bone densitometry results in patients with idiopathic BPPV.
