Impact of multi-vessel vasospastic angina on cardiovascular outcome

Background and aims: Since clinical characteristics and prognosis of patients with multi-vessel vasospastic angina (VSA) are not clear, we investigated the nature and prognosis of multi-vessel VSA in Koreans. Methods: Among 2960 patients enrolled in the VA-KOREA (Vasospastic Angina in Korea) registry, 104 definite multi-vessel VSA patients, 163 single vessel VSA patients and 737 non-VSA patients were identified using the intracoronary ergonovine provocation test. Results: Multi-vessel VSA and single vessel VSA groups showed similar baseline characteristics and medical treatment on discharge, but different from the non-VSA group. The primary composite endpoint (cardiac death, acute coronary syndrome, and symptomatic new onset arrhythmia) over a 36-month follow-up period was significantly higher in the multi-vessel VSA group than in the single vessel VSA and non-VSA groups (8.7% vs. 1.8% and 1.1%, each log-rank p < 0.05, respectively). The rate of death and acute coronary syndrome of the multi-vessel VSA group was higher than in the single vessel VSA and non-VSA groups (5.8% vs. 1.2% and 0.9%, each log-rank p < 0.05, respectively). In addition, multi-vessel VSA was an independent predictor of the primary composite endpoint at 36 months (HR 8.5, 95% CI [2.6-27.2], p < 0.0001). Conclusions: Patients with multi-vessel VSA had worse clinical outcomes than single vessel VSA and non-VSA groups, suggesting that the existence of multi-vessel VSA itself is highly prognostic.

Summary: This study examined the impact of sex and initial clinical presentation on the long-term outcomes in patients undergoing percutaneous coronary intervention (PCI). Based on pooled individual patient data from 5 Korean-multicenter drug-eluting stent (DES) registries, it was found that there was no sex difference in cardiac death in SAP and UAP/NSTEMI patients, while in STEMI patients, women had higher rates of cardiac death and all-cause mortality. However, after adjusting for confounders, female sex was not an independent predictor of mortality.

Summary: Increase in vessel inflammation represented by PVAT density is independently associated with the progression of the lipid component of coronary atherosclerotic plaques. The progression of calcified PV was not associated with the increase in PVAT density.

Prognostic impact of nitrate therapy in patients with myocardial bridge and coexisting coronary artery spasm

Summary: This study investigated the impact of nitrate therapy on the prognosis of patients with myocardial bridge and coexisting coronary artery spasm. The results showed that long-term nitrate administration did not reduce major adverse cardiovascular events but was associated with a higher incidence of recurrent angina requiring follow-up angiography.

Summary: The HOST-EXAM Extended study investigated the long-term outcomes of antiplatelet monotherapy in patients who underwent percutaneous coronary intervention. Summary: This study compared the long-term clinical outcomes of patients treated with ultrathin-strut biodegradable polymer sirolimus-eluting stents (BP-SES) and thick-strut biodegradable polymer biolimus-eluting stents (BP-BES). The results showed that the ultrathin-strut BP-SES had significantly lower target lesion failure rates and better outcomes compared to the thick-strut BP-BES.

Summary: This study used the Bayesian quantile regression model to analyze the relationship between multiple cardiovascular risk factors and different stages of coronary artery disease. The results showed that the lumen diameter stenosis of different heart vessels ranged from 41% to 50% and the maximum stenosis change ranged from 5.6% to 7.3%. The study found that angina was associated with a high quantile (90%) of stenosis in the left anterior descending artery, diabetes was associated with higher quantiles (75% and 90%) of stenosis in the left circumflex artery, and dyslipidemia was associated with the highest quantile (90%) of stenosis in the right coronary artery. This study demonstrates the clinical utility of the Bayesian quantile regression model in evaluating the comprehensive relationship between risk factors and baseline-grade coronary artery disease and its progression.

Summary: This study aimed to evaluate the efficacy and safety of prasugrel dose de-escalation therapy in patients with diabetes mellitus-acute coronary syndrome who underwent percutaneous coronary intervention. The results showed that dose de-escalation significantly reduced adverse clinical events, primarily driven by a decrease in bleeding without an increase in ischemic events.

Summary: This study compared the clinical outcomes of immediate and staged complete revascularization in patients with ST-segment elevation myocardial infarction and multivessel disease. The study found that the incidence of major adverse cardiovascular events was slightly higher in the immediate revascularization group compared to the staged revascularization group at 1-year follow-up. However, there were no significant differences in other clinical outcomes between the two groups. The study suggests that further large randomized clinical trials are needed to determine the optimal timing of complete revascularization for these patients.

Summary: This study aimed to determine the optimal antiplatelet therapy (APT) for patients undergoing non-cardiac surgery within 1 year after percutaneous coronary intervention (PCI). The study found that continuing APT in these patients may reduce the incidence of 30-day net adverse clinical events (NACE) and major adverse cardiovascular events (MACE), with no significant difference in the incidence of major bleeding events.

Effects of delayed hospitalization on the 3-year clinical outcomes of patients with or without diabetes who had non-ST-segment-elevation myocardial infarction and underwent new-generation drug-eluting stent implantation

Summary: The clinical outcomes after NSTEMI were compared between patients with (SDT >= 24 h) and without (SDT < 24 h) delayed hospitalization, in both diabetic and non-diabetic patients. In the diabetic group, there was no significant difference in primary and secondary clinical outcomes between the SDT < 24 h and SDT >= 24 h groups. However, in the non-diabetic group, the rates of all-cause and cardiac death were significantly higher in the SDT >= 24 h group than in the SDT < 24 h group.

Summary: This study aimed to investigate the impact of ACEI and ARB on 3-year clinical outcomes in elderly AMI patients without a history of hypertension who underwent successful PCI with DES. A total of 13,104 patients were included and divided into ACEI group and ARB group. The use of ACEI was associated with lower incidences of stroke and re-hospitalization due to HF compared to the use of ARB.

Summary: Arterial stiffness index, exercise hemodynamic parameters, and echocardiographic left ventricular functional parameters are related to exercise-induced ST depression. Older age, higher pulmonary arterial systolic pressure, left atrial volume index, baPWV, ankle brachial index at rest, hypertensive response, higher heart rate, and rate-pressure product at peak exercise are significantly associated with ST depression. Higher peak heart rate, PASP, and baPWV are independently related to ST depression.

Summary: In this study, the effects of sex differences in delayed hospitalization on major clinical outcomes in patients with non-ST-segment elevation myocardial infarction after new-generation drug-eluting stent implantation were compared. The results showed that female patients had higher mortality rates and cardiac death rates in the 0-24 hour delayed hospitalization group compared to male patients, and also had higher mortality rates over a 3-year follow-up period.

Association of Early Hysterectomy With Risk of Cardiovascular Disease in Korean Women

Summary: This Korean nationwide cohort study found that early hysterectomy before age 50 is associated with an increased risk of cardiovascular disease (CVD), particularly stroke. Even after excluding women who underwent oophorectomy, the hysterectomy group still had higher risks of CVD compared to the nonhysterectomy group. However, due to the relatively low incidence of CVD, a change in clinical practice may not be necessary.

Current Status of Cardiovascular Disease According to the Duration of Hypertension in Korean Adults

Summary: This study investigates the prevalence and management of hypertension in Korean adults and evaluates its correlation with the risk of cardiovascular disease (CVD) and stroke. The results show that the longer the duration of hypertension, the higher the risks of CVD and stroke. However, achieving optimal blood pressure control reduces the risk of all CVD and stroke by nearly half.
