Long-term fatigue and quality of life among epithelial ovarian cancer survivors: a GINECO case/control VIVROVAIRE I study

Background: Few data are available on long-term fatigue (LTF) and quality of life (QoL) among epithelial ovarian cancer survivors (EOCS). In this case-control study, we compared LTF, symptoms and several QoL domains in EOCS relapse-free >= 3 years after first-line treatment and age-matched healthy women. Patients and methods: EOCS were recruited from 25 cooperative GINECO centers in France. Controls were randomly selected from the electoral rolls. All participants completed validated self-reported questionnaires: fatigue (FACIT-F), QoL (FACT-G/O), neurotoxicity (FACT-Ntx), anxiety/depression (HADS), sleep disturbance (ISI), and physical activity (IPAQ). Severe LTF (SLTF) was defined as a FACIT-F score <37/52. Univariate and multivariate logistic regressions were conducted to analyze SLTF and its influencing factors in EOCS. Results: A total of 318 EOCS and 318 controls were included. EOCS were 63-year-old on average, with FIGO stage I/II (50%), III/IV (48%); 99% had received platinum and taxane chemotherapy, with an average 6-year follow-up. There were no differences between the two groups in socio-demographic characteristics and global QoL. EOCS had poorer FACIT-F scores (40 versus 45, P < 0.0001), lower functional well-being scores (18 versus 20, P = 0.0002), poorer FACT-O scores (31 versus 34 P < 0.0001), and poorer FACT-Ntx scores (35 versus 39, P < 0.0001). They also reported more SLTF (26% versus 13%, P = 0.0004), poorer sleep quality (63% versus 47%, P = 0.0003), and more depression (22% versus 13%, P = 0.01). Fewer than 20% of EOCS and controls exercised regularly. In multivariate analyses, EOCS with high levels of depression, neurotoxicity, and sleep disturbance had an increased risk of developing SLTF (P < 0.01). Conclusion: Compared with controls, EOCS presented similar QoL but persistent LTF, EOC-related symptoms, neurotoxicity, depression, and sleep disturbance. Depression, neuropathy, and sleep disturbance are the main conditions associated with severe LTF.

Use of genome-wide testing in oncology: French expert opinion based on the Delphi methodology

Summary: Genome-wide testing plays a crucial role in advancing precision medicine in oncology; however, current recommendations lack specificity regarding its indications and utility for different types of cancer. To address this, a national consensus approach using a modified Delphi methodology was employed in France to provide expert opinions on the use of genome-wide testing in clinical practice. Consensus was reached through multiple rounds of rating, resulting in 24 statements agreed upon by the expert panel. While genome-wide testing was recommended for non-small cell lung cancer (NSCLC) and cancer of unknown primary (CUP), its routine use was not recommended for breast cancer and melanoma. The standardization of broad molecular screening in oncology may be facilitated by improved access to innovation in France.

Impact of lymphopenia on efficacy of nivolumab in head and neck cancer patients

Summary: This study evaluated the impact of lymphopenia on the efficacy of nivolumab immunotherapy in patients with head and neck cancer. The results showed that lymphopenia upon initiation of nivolumab did not affect patients' survival, but persistent lymphopenia during immunotherapy might be a prognostic marker of patient survival.

Association of chemotherapy and comorbidities with overall survival in elderly patients with early breast cancer: a French population-based propensity score-matched analysis

Summary: This study aimed to examine the effect of additional chemotherapy on overall survival in patients aged 70 years or older with early breast cancer, as well as the impact of comorbidities on chemotherapy benefit. The results showed that additional chemotherapy was associated with improved overall survival, while chronic obstructive pulmonary disease and polypharmacy were associated with worse overall survival. These findings support the importance of chemotherapy considerations in elderly patients with early breast cancer.

Impact of adjuvant trastuzumab treatment on fatigue, emotional status and quality of personal and work life of patients with localised breast cancer: results of the 'HER-ception' study

Summary: The study aimed to analyze the impact of adjuvant trastuzumab on fatigue, emotional status, and quality of personal and work life in patients with localized breast cancer. The results showed that trastuzumab did not affect fatigue and return to work, but patients treated with trastuzumab may experience a stronger feeling of helplessness in terms of emotional well-being. Therefore, emotional support should be provided for patients receiving trastuzumab.

Bioaugmentation effect of Absidia cylindrospora on a PAHs and trace metals co-contaminated soil within a 3-month microcosm-experiment

Summary: Soil contamination is a significant issue due to urbanization and industrialization. This paper introduces the bioaugmentation effect of fungal strain Absidia cylindrospora on co-contaminated industrial soil for the first time. The study investigates the evolution of metals and PAHs content in the soil over time, as well as the adsorption and/or accumulation of metals within the mycelium of Absidia cylindrospora. The results show that the fungal strain can simultaneously accumulate Cd, Co, Cu, Ni, and Zn, even in the presence of high levels of PAHs.

Splenectomy in epithelial ovarian cancer surgery

Summary: ObjectiveSplenectomy is performed in a significant percentage of cytoreductive surgeries for ovarian cancer. This study aimed to evaluate the impact of splenectomy on overall and disease-free survival in patients with epithelial ovarian cancer. MethodsA retrospective study was conducted on patients who underwent cytoreduction for ovarian cancer between January 2000 and December 2016. Patients were stratified into splenectomy and non-splenectomy groups, and postoperative complications and survival outcomes were analyzed. ResultsA total of 464 patients were included in the study. The splenectomy group had higher disease stages, peritoneal carcinomatosis scores, and radical surgery rates. However, there was no significant difference in complete cytoreduction rates between the two groups. Splenectomy was associated with extensive surgical procedures and increased risk of abdominopelvic lymphocele and blood transfusion. The average length of hospital stay and delay in adjuvant chemotherapy were longer in the splenectomy group. Overall and disease-free survival rates were not significantly different between the groups. ConclusionSplenectomy may be a safe procedure with no impact on overall or disease-free survival in patients with ovarian cancer. However, it is associated with longer hospital stay and delay in chemotherapy.

Sex-related quality of life after total laryngectomy for cancer

Summary: This study analyzed the impact of total laryngectomy on the quality of sexual life for cancer patients and the methods used to assess it. The results showed that the quality of sexual life was severely affected, including issues like erectile dysfunction, decreased ejaculatory function, and low libido. Lack of postoperative support was also reported. Therefore, these factors should be taken into account before performing total laryngectomy, and there is a need for a common information tool to improve the management of sexuality based on patient demand.
