Summary: Diabetic nephropathy (DN) is a common microvascular complication of diabetes that leads to end-stage renal disease. This study aimed to identify key transcription factors (TFs) associated with DN progression by performing a meta-analysis on gene expression profiles from multiple datasets. The analysis revealed distinct clusters of dysregulated susceptibility genes in different renal compartments, with a subset of these genes encoding critical TFs involved in DN development and podocyte function. The role of these TFs was validated in a hyperglycemic zebrafish model, showing that altered expression of these genes leads to morphological abnormalities in various kidney structures. This study provides insights into the molecular drivers, essential genes, and pathways involved in DN initiation and development.

Summary: This retrospective cohort study aimed to explore the risk factors associated with reaching the final stage of chronic kidney disease (CKD-G5) while receiving conservative kidney management (CKM). The study found that low eGFR and albuminuria were the primary risk factors. Male gender and uncontrolled blood pressure had a greater impact on the need for kidney replacement therapy (KRT) compared to CKD-G5 with CKM. Increasing age and rurality reduced the probability of KRT but increased the probability of CKD-G5 with CKM. Higher incomes decreased the likelihood of developing CKD-G5 with and without KRT.

The use of SGLT2is is increasing among patients with diabetes, but remains low even in those with cardiovascular disease (CVD). Patient characteristics associated with lower odds of filling an SGLT2i prescription included female sex, older age, and lower income.

Summary: We evaluated the implementation and clinical outcomes of a perioperative glycemic management pathway in gynecologic oncology. The postimplementation period showed an increase in preoperative screening with glycated hemoglobin and blood glucose measurement after surgery for diabetic patients. However, there was no improvement in the proportion of patients with hyperglycemia, and major quality gaps in perioperative glycemic management still existed despite the implementation of a multidisciplinary care pathway. Optimal strategies for improvement in perioperative glycemic management are still unknown.

Summary: In a randomized trial conducted in Alberta, Canada, a tailored self-management education and support (SMES) program designed using advertising principles was found to reduce the rate of clinical outcomes among low-income older adults. The intervention included health promotion messaging and relay of clinical information to patients' healthcare providers. The study showed that the rate of the primary outcome, including death, myocardial infarction, stroke, coronary revascularization, and hospitalizations for cardiovascular-related conditions, was lower in the SMES group compared to the control group.
