Insulin-like growth factor-binding protein 7 (IGFBP 7) as a new biomarker in coronary heart disease

Purpose: The role of insulin-like growth factor-binding protein-7 (IGFBP-7) in atherosclerosis is still not well-known. The objective of this study was to find out the following: 1) whether IGFBP-7 may act as a biomarker of coronary artery disease (CAD) occurrence and extent; 2) whether IGFBP-7 is potentially related to the classical and new markers of cardiovascular risk (carotid intima-media thickness - cIMT); 3) whether IGFBP-7 may be a marker of mortality in the group of patients with myocardial infarction (MI). Materials/Methods: The study group consisted of 212 patients with MI and 75 patients with stable CAD, the control group included 100 healthy volunteers. IGFBP-7 serum concentration was measured. Results: IGFBP-7 value was considerably higher in the study group (MI and CAD patients - 35.1 ng/ml (P = 0.000001) and 32.7ng/ml (P = 0.0001), respectively), than in the controls - 25.2ng/ml. No statistically significant differences between IGFBP-7 concentrations in the MI and CAD group were found. No relationship between IGFBP-7 and the coronary lesions advancement in the study group was observed. No changes in IGFBP-7 concentration in the MI patients during hospitalization were observed. In the group of MI patients who died during follow-up, a considerably higher cIMT values were found whereas no statistically significant difference was observed in relation to IGFBP-7 (34.6 vs. 35.2 ng/ml). Conclusions: IGFBP-7 is a good biomarker of CAD occurrence but not of its advancement. We demonstrated the existence of the relation between higher IGFBP-7 concentration and the selected classical risk factors of cardiovascular events as well as cIMT values. IGFBP-7 cannot serve as a marker of acute ischemia. Also, IGFBP-7 was not confirmed as a predictor of mortality in the MI patients.

Prognostic value of late gadolinium enhancement mass index in patients with pulmonary arterial hypertension

Summary: Dysfunction of the right ventricle is crucial for survival in patients with pulmonary arterial hypertension, with late gadolinium enhancement at RV insertion points being associated with RV dysfunction. Quantification of LGE at RVIPs can provide additional prognostic information, as shown in the study where higher LGEMI correlated with worse prognosis in PAH patients. The indexed mass of LGE at RV septal insertion points may serve as a non-invasive marker of PAH prognosis.

Smoking cessation in patients with established coronary artery disease: data from the POLASPIRE survey

Factors related to persistent smoking in these patients include older age, exposure to secondhand smoke, low socioeconomic status, lack of consultation with a cardiologist, and not participating in cardiac rehabilitation.

Summary: The study found that participants in moderate and high cardiovascular risk classes had similar unfavorable cardiometabolic profiles and lifetime cardiovascular risk, suggesting the need for more aggressive treatment management in the moderate cardiovascular risk population. Therefore, new prospective population studies are necessary to establish different cardiovascular risk profiles in a changing society.

Undiagnosed Diabetes and Prediabetes in Patients with Chronic Coronary Syndromes-An Alarming Public Health Issue

Summary: This study aimed to evaluate the prevalence of undiagnosed diabetes and prediabetes in patients with chronic coronary syndromes (CCS) and identify factors associated with dysglycemia. Results showed a 5.1% newly diagnosed diabetes rate and 72.3% prediabetes rate among study participants. Body mass index and high-density lipoprotein cholesterol were identified as important factors in assessing the risk of dysglycemia development after hospital discharge.

Summary: In patients with coronary artery disease, serum levels of Chemerin are correlated with inflammatory markers, insulin resistance, and an unfavorable lipid profile. The correlation with fat mass depends on glucose metabolism status. The mechanisms regulating Chemerin secretion may differ depending on the presence of diabetes/prediabetes.

Effectiveness and safety of a simple home-based rehabilitation program in pulmonary arterial hypertension: an interventional pilot study

New factors that may affect the development of CAD, such as the gut microbiome, are extensively investigated. CAD is associated with altered gut microbiome composition and function, including changes in Firmicutes/Bacteroidetes ratio and alpha-beta biodiversity.

Expectations of family nurses among residents of a midsize eastern European city: A population-based cohort study in Poland

Summary: To improve the quality of care, it is important to understand patients' needs and expectations. This study found that socioeconomic status, marital status, and age were associated with patients' expectations of family nurses. Younger patients were more likely to expect health counseling from family nurses, while those who expected home visits had lower self-evaluated health. These findings suggest that family nurses should tailor their services to individual patients' situations and needs.
