Summary: Esophageal hypomotility, particularly ineffective esophageal motility according to the Chicago criteria, is a common motility disorder diagnosed on high resolution manometry. Most patients present with GERD symptoms or dysphagia. Prokinetic agents are commonly prescribed but their beneficial effects are limited to certain drugs.

Summary: The study found that integrated relaxation pressure (IRP) can distinguish between the reflux-predominant and dysphagia-predominant phenotypes in patients with absent contractility (AC). This may have important clinical implications, as procedures such as fundoplication may benefit patients with reflux and a low IRP, while procedures like peroral endoscopic myotomy (POEM) may benefit patients with dysphagia and a relatively high IRP.

Summary: Esophageal peristalsis is a complex process involving both inhibition and excitation. Abnormalities in this process can lead to dysphagia and other swallowing difficulties. Studying the distensibility and contraction characteristics of the esophagus during peristalsis can provide valuable insights for the development of new treatments for patients with swallowing disorders.

Summary: This study compared the demographic, clinical, endoscopic, and manometric findings of achalasia patients between Israeli Arabs and Israeli Jews. It found that Israeli Arab achalasia patients had different clinical symptoms and higher integrated relaxation pressure compared to Israeli Jewish patients.

Summary: The study found that using solid food swallows (SFS) during esophageal motility assessment can improve diagnostic accuracy and is important for diagnosing clinically relevant disorders. The application of solid food swallows in HRM has a significant impact on changing diagnoses and can be widely used in daily clinical practice.

Summary: This study demonstrates that an integrated approach with parent-preferred therapy based on mechanistic understanding of VFSS and HRM metrics improves oral feeding outcomes, despite evidence of penetration or aspiration. The application of new knowledge of the physiology of swallowing and airway protection may contribute to these findings.

Summary: Dysphagia lusoria is a rare condition caused by impingement of the esophagus by an aberrant right subclavian artery, leading to progressive difficulty in swallowing. Diagnosis is typically made through various imaging studies and high-resolution impedance manometry, which can help strengthen the diagnosis of true dysphagia lusoria.

Summary: The Chicago Classification 4.0 introduces new concepts for the diagnosis of achalasia, potentially reducing unnecessary treatment. Conditions such as esophagogastric junction outflow obstruction, distal esophageal spasm, and hypercontractile esophagus are now considered clinically significant only in certain conditions, leading to a potential decrease in treatment, especially peroral endoscopic myotomy, solely based on manometric diagnosis.

Summary: Patients with absent contractility (AC) have more esophageal symptoms and require more intense gastrointestinal follow-up than controls. Only a minority of patients with AC have underlying rheumatologic disease. Those without rheumatologic disease at baseline did not subsequently develop one, suggesting that a rheumatologic evaluation is likely unnecessary. The clinical course of AC in patients with rheumatologic disease and those without appears to be similar.

Summary: This study aimed to assess the prevalence of esophageal motility disorders (EMD) diagnosed by high-resolution esophageal manometry (HREM) after cervical or thoracic radiation therapy. The results showed that dysphagia was the most frequent symptom, 60% of patients had abnormal esophageal motility, with 15% presenting with major motility disorder.

Summary: The study evaluates the use of Minimally Invasive Marshmallow Swallow Study (MSS) as a screening technique and its correlation with High Resolution Manometry (HRM) results and post-operative dysphagia. The results suggest that MSS is effective in ruling out major esophageal motility disorders and reducing the need for invasive HRM.

Summary: Esophageal motor abnormalities are common in patients with eosinophilic esophagitis (EoE) and may contribute to symptoms. Eosinophils influence esophageal motility, and some motility abnormalities can be improved after medical treatment.

Summary: A European consensus was developed on the clinical application of esophageal high-resolution manometry (HRM), providing guidance on when to consider HRM, how to perform it, and how to interpret the results. Areas of uncertainty and lack of consensus were identified, particularly in terms of treatment implications, highlighting the need for future research in this area.

Summary: The study found that the esophageal dysphagia questionnaire (BEDQ) is a valid clinical assessment tool in French, accurately reflecting the frequency and intensity of dysphagia in patients, and showing good correlation with other scoring tools. For patients undergoing examination, the BEDQ score is significantly higher in the dysphagia group compared to the GERD or other symptom groups.

Summary: This study assessed symptoms potentially related to adverse events experienced by cancer outpatients treated with OAAs using patient-reported outcomes (PROs). The results showed that almost three-quarters of the patients reported at least one high-level symptom, with fatigue, various psychological disorders, and general pain being the most commonly experienced symptoms. This study highlights the importance of close coordination between community and hospital health professionals and the integration of patient self-reporting systems in oncology clinical practice.
