Summary: This study highlights impaired mobility as the best single predictor for postoperative delirium and 2-year mortality in high-risk elderly patients undergoing TAVI. Prospective multicenter studies are needed to further explore and optimize the use of predictors for delirium in TAVI care pathways.

Summary: This study reports the experience of explanting a transcatheter heart valve from 2013 through 2021. Surgical bailout was the most common indication for valve explantation, followed by infective endocarditis and paravalvular leak. Aortic root reconstruction was frequently required, and the in-hospital mortality rate was high.

Summary: This study aimed to determine whether TAVI was noninferior to surgery in patients at moderately increased operative risk. The results showed that among patients aged 70 years or older with severe, symptomatic aortic stenosis and moderately increased operative risk, TAVI was noninferior to surgery with respect to all-cause mortality at 1 year.

Summary: Studies have shown improved outcomes among married patients who underwent cardiovascular surgery. This study aimed to examine the impact of marital status and patient sex on outcomes after transcatheter aortic valve implantation (TAVi). The findings revealed that married females had lower survival rates, married males had higher survival rates, and widowed status was associated with higher readmission rates.

Summary: The study found that the discharge location after TAVI is associated with clinical outcomes. Patients discharged home were younger, more likely to be male, and had lower risk compared to those discharged to a rehabilitation facility or another institution. Patients discharged home or to rehabilitation had better clinical outcomes in terms of lower risk for death and bleeding within the first year compared to those transferred to another institution.

Summary: This study aimed to investigate the association of the multidimensional prognostic index (MPI) based on comprehensive geriatric assessment (CGA) with survival in elderly patients undergoing TAVI. The results showed that patients in the MPI-1 group had a higher overall survival rate compared to those in the MPI-2-3 group, suggesting that the MPI tool could be useful for assessing frailty and predicting which patients may benefit more from TAVI procedures.

Summary: The number of transcatheter aortic valve implantations (TAVI) has increased significantly in recent decades. This study provides an analysis of the causes of TAVI failure, including degenerative, functional, and infective factors, as well as the outcomes of surgical aortic valve replacement (SAVR) after TAVI. The causes of SAVR after TAVI failure are multifactorial, and the prognosis varies depending on the underlying factors.

Summary: Frailty is strongly associated with time in hospital and mortality among patients undergoing transcatheter aortic valve replacement. Higher frailty levels are linked to longer hospital stays and increased risk of death within one year. Strategies aimed at preventing hospitalization among frail patients could be beneficial in reducing the burden of care.

Summary: Cerebral embolization is a known complication of transcatheter aortic valve implantation (TAVI), but the effect of the procedure on ocular perfusion is unclear. This study found that TAVI resulted in reduced choroidal perfusion, areas of retinal ischemia, and embolization in a considerable proportion of patients. These complications should be monitored and included in consent procedures for TAVI.

Summary: Transcatheter aortic valve implantation (TAVI) has become the standard of care for elderly high-risk patients with symptomatic severe aortic stenosis. However, diagnosing bioprosthetic valve dysfunction after TAVI is challenging and there are limited evidence-based criteria to guide therapy. This review focuses on the role of imaging modalities in monitoring the integrity of transcatheter heart valves.

Summary: Heart failure is a leading cause of death and disability in older adults, but the implementation of guideline-directed medical therapies to reduce the burden of the disease has been suboptimal. This review discusses interventions that increase the uptake of these therapies, barriers and facilitators of implementation, conceptual frameworks in implementation science, and trial design suggestions. An evidence-to-care conceptual model is proposed to generate evidence and promote long-term implementation. By applying principles of implementation science, policymakers, researchers, and clinicians can help alleviate the burden of heart failure worldwide.

Summary: This study aimed to evaluate the predictive value of the GRACE risk score in acute pulmonary embolism (PE) patients for 30-day mortality and the need for thrombolytic treatment. The study found that the GRACE risk score, along with the PESI score, were independent risk factors associated with 30-day mortality and the need for thrombolytic treatment. The GRACE risk score showed high sensitivity and specificity in predicting mortality, and its combination with the PESI score helped define high-risk PE patients and predict poor prognosis.

Summary: Transthoracic echocardiography (TTE) is commonly used to evaluate isolated tricuspid regurgitation (TR), but there is limited research comparing its quantification with magnetic resonance imaging (MRI). In this study, a novel TTE algorithm was developed to identify severe TR based on TTE parameters. The algorithm had a higher accuracy than the current guidelines' criteria in detecting severe TR by MRI.

Summary: This article presents a case of a patient with known situs inversus who underwent cardiac catheterization and was found to have chronic total occlusion in the right coronary artery. Situs inversus is a rare congenital abnormality characterized by the inverted position of chest and abdominal organs. Cardiac catheterization is uncommon in patients with this particular abnormality, highlighting the importance of customizing techniques to engage coronary arteries and optimize guide support for percutaneous coronary intervention if needed.

Summary: This study evaluated the angiogenic capacity of adipose tissue in patients undergoing cardiac surgeries and found that patients with coronary artery disease (CAD) had reduced angiogenic capacity in their pericardial adipose tissue. Gene expression analysis revealed upregulated expression of the anti-angiogenic gene TSP-1 in CAD patients, with no significant differences in other angiogenic factors. Additionally, inhibiting TSP-1 expression significantly improved angiogenic deficiency in CAD patients.

Summary: The current guidelines recommend prophylactic implantable cardioverter-defibrillator (ICD) for patients with symptomatic heart failure (HF) and low left ventricular ejection fraction. However, not all patients will benefit from ICD treatment. This study evaluated the feasibility of using the MADIT-II-based Risk Stratification Score (MRSS) to predict the survival benefit of prophylactic ICDs in patients with HF. The results showed that different risk subgroups had varying levels of ICD survival benefit.
