Health Care Utilization and Expenditures Associated With Remote Monitoring in Patients With Implantable Cardiac Devices

Several randomized trials and decision analysis models have found that remote monitoring may reduce health care utilization and expenditures in patients with cardiac implantable electronic devices (CIEDs), compared with in-office monitoring. However, little is known about the generalizability of these findings to unselected populations in clinical practice. To compare health care utilization and expenditures associated with remote monitoring and in office monitoring in patients with CIEDs, we used Truven Health MarketScan Commercial Claims and Medicare Supplemental Databases. We selected patients newly implanted with an implantable cardioverter defibrillators (ICD), cardiac resynchronization therapy defibrillator (CRT-D), or permanent pacemaker (PPM), in 2009, who had continuous health plan enrollment 2 years after implantation. Generalized linear models and propensity score matching were used to adjust for confounders and estimate differences in health care utilization and expenditures in patients with remote or in-office monitoring. We identified 1,127; 427; and 1,295 pairs of patients with a similar propensity for receiving an ICD, CRT-D, or PPM, respectively. Remotely monitored patients with ICDs experienced fewer emergency department visits resulting in discharge (p = 0.050). Remote monitoring was associated with lower health care expenditures in office visits among patients with PPMs (p = 0.025) and CRT-Ds (p = 0.006) and lower total inpatient and outpatient expenditures in patients with ICDs (p <0.0001). In conclusion, remote monitoring of patients with CIEDs may be associated with reductions in health care utilization and expenditures compared with exclusive in-office care. (C) 2016 Elsevier Inc. Summary: The evidence of direct oral anticoagulants (DOACs) usage for venous thromboembolism (VTE) in patients at extremes of body weight or mass index is limited. In such situations, warfarin may be more frequently used. We investigated warfarin time in the therapeutic international normalized ratio range (TTR) and DOAC adherence based on the calculated proportion of days covered (PDC) by pill coverage from a DOAC prescription in patients with VTE across all body sizes. Evaluation: 5/10.

Associations of Serum and Dialysate Potassium Concentrations With Incident Atrial Fibrillation in a Cohort Study of Older US Persons Initiating Hemodialysis for Kidney Failure

Summary: This study aimed to investigate the association between serum potassium concentration and dialysate potassium concentration with atrial fibrillation (AF) incidence. The results showed that lower serum potassium concentration was associated with an increased risk of AF, while higher serum potassium concentration was not. Dialysis with a dialysate potassium concentration of 3 mEq/l compared to 2 mEq/l was independently associated with a lower incidence of AF. A rating of 7 out of 10.

Dronedarone versus sotalol in patients with atrial fibrillation: A systematic literature review and network meta-analysis

Summary: This study compared the safety and efficacy of two commonly used Vaughan-Williams class III antiarrhythmic drugs, dronedarone and sotalol, for the maintenance of sinus rhythm in adults with nonpermanent atrial fibrillation. The results showed that dronedarone was associated with a lower risk of all-cause death compared to sotalol, but there was no significant difference in AF recurrence.

This report summarizes a workshop focused on identifying key research opportunities related to stroke prevention in AF, addressing areas such as improving risk stratification tools, overcoming challenges with oral anticoagulants, and determining the optimal roles of percutaneous left atrial appendage occlusion and surgical closure/excision. The report aims to promote innovative research that leads to personalized and effective stroke prevention strategies in individuals with AF.

Association of CHA2DS2-VASc and HAS-BLED to frailty and frail outcomes: From the TREAT-AF study

Summary: High CHA2DS2-VASc and HAS-BLED scores are strongly associated with frailty. However, in frail patients, oral anticoagulant use is associated with a reduction in 1-year mortality, but not a significant decrease in the risk of stroke or major bleeding. Therefore, more prospective studies are needed to support clinical decision-making.

Summary: This retrospective study evaluated the safety and effectiveness of dronedarone and sotalol for treating patients with atrial fibrillation. The results showed that compared with sotalol, dronedarone was associated with a lower risk of ventricular proarrhythmic events and symptomatic bradycardia, but there was no difference in the risk of cardiovascular hospitalization and mortality. These findings provide the basis for future prospective trials.

Can GRACE Risk Score Predict Mortality and the Need for Thrombolytic Treatment in Acute Pulmonary Embolism?

Summary: This study aimed to evaluate the predictive value of the GRACE risk score in acute pulmonary embolism (PE) patients for 30-day mortality and the need for thrombolytic treatment. The study found that the GRACE risk score, along with the PESI score, were independent risk factors associated with 30-day mortality and the need for thrombolytic treatment. The GRACE risk score showed high sensitivity and specificity in predicting mortality, and its combination with the PESI score helped define high-risk PE patients and predict poor prognosis.

Summary: Transthoracic echocardiography (TTE) is commonly used to evaluate isolated tricuspid regurgitation (TR), but there is limited research comparing its quantification with magnetic resonance imaging (MRI). In this study, a novel TTE algorithm was developed to identify severe TR based on TTE parameters. The algorithm had a higher accuracy than the current guidelines' criteria in detecting severe TR by MRI.

Coronary Angiography and Chronic Total Occlusion Angioplasty in a Patient With Situs Inversus from the Right Radial Approach

Summary: This article presents a case of a patient with known situs inversus who underwent cardiac catheterization and was found to have chronic total occlusion in the right coronary artery. Situs inversus is a rare congenital abnormality characterized by the inverted position of chest and abdominal organs. Cardiac catheterization is uncommon in patients with this particular abnormality, highlighting the importance of customizing techniques to engage coronary arteries and optimize guide support for percutaneous coronary intervention if needed.

Pericardial Adipose Tissue Thrombospondin-1 Associates With Antiangiogenesis in Ischemic Heart Disease

Summary: This study evaluated the angiogenic capacity of adipose tissue in patients undergoing cardiac surgeries and found that patients with coronary artery disease (CAD) had reduced angiogenic capacity in their pericardial adipose tissue. Gene expression analysis revealed upregulated expression of the anti-angiogenic gene TSP-1 in CAD patients, with no significant differences in other angiogenic factors. Additionally, inhibiting TSP-1 expression significantly improved angiogenic deficiency in CAD patients.

Evaluation of MADIT-II Risk Stratification Score Among Nationwide Registry of Heart Failure Patients With Primary Prevention Implantable Cardiac Defibrillators or Resynchronization Therapy Devices

Summary: The current guidelines recommend prophylactic implantable cardioverter-defibrillator (ICD) for patients with symptomatic heart failure (HF) and low left ventricular ejection fraction. However, not all patients will benefit from ICD treatment. This study evaluated the feasibility of using the MADIT-II-based Risk Stratification Score (MRSS) to predict the survival benefit of prophylactic ICDs in patients with HF. The results showed that different risk subgroups had varying levels of ICD survival benefit.
