Community-based automated external defibrillator only resuscitation for out-of-hospital cardiac arrest patients

Background Speed is the cornerstone of rescue for out-of-hospital cardiac arrest. As a consequence, community participation programs have been initiated to decrease response times. Even in the very best of these programs, however, short-term survival rates hover around 10% and long-term survival rates are half that. In most locales, survival is far worse. In Piacenza, Italy, responders have been trained for more than a decade to use publicly available automated external defibrillators (AEDs) and eschew the performance of cardiopulmonary resuscitation (CPR). It is known locally as Progetto Vita. Methods From 2001 to 2014, we prospectively collected outcome data on all Progetto Vita-treated patients and all 3271 standard emergency medical services (EMS) patients. Progetto Vita rescuers simply accessed a public AED, turned it on, and only followed its instructions. Progetto Vita rescuers did not do CPR of any sort. If EMS arrived prior to initiation or even completion of the Progetto Vita protocol, EMS-supplanted Progetto Vita efforts and patients were not included in the Progetto Vita cohort. Follow-up was collected by each responder's data files, medical record review, and use of the Italian system death index. All cardiac arrest patients' death status was validated in 100% of patients through August 1, 2014. Findings Survival to hospital discharge occurred in 39 (41.4%) of the 95 patients treated by Progetto Vita and in 193 (5.9%) of the 3271 EMS patients. At 13-year follow-up, the Kaplan-Meier estimates of survival were 31.8% when AEDs only were used and 2.4% for standard EMS/CPR response. Estimates of survival are significantly better for Progetto Vita AED-only therapy when survival was stratified by time to respond, gender, location of cardiac arrest, and shockable rhythm. Relative to the 95 EMS patients with the fastest response times, Progetto Vita intervention was associated with a more than 2-fold increased rate of survival. Interpretation This is the first demonstration of excellent long-term survival from out-of-hospital cardiac arrest by promoting speed and ease of lay AED response without CPR.

Decongestive treatment adjustments in heart failure patients remotely monitored with a multiparametric implantable defibrillators algorithm

Summary: The HeartLogic algorithm combines data from multiple implantable defibrillators (ICD)-based sensors to predict impending heart failure (HF) decompensation. This study implemented the algorithm for remote monitoring of HF patients and evaluated the approach to diuretic dosing and timing of intervention in patients with device alerts. The results showed that the use of decongestive treatment adjustments triggered by alerts seemed safe and effective, with early use of decongestive treatment and higher dosages of diuretics associated with more favorable outcomes.

Summary: The clinical use of implantable loop recorder (ILR) in elderly patients with recurrent, traumatic, unexplained syncope is still limited. In this specific patient population, there is a lack of data on the actual risk of traumatic syncope recurrence during ILR monitoring. This prospective registry study aimed to assess the risk of traumatic and non-traumatic syncopal recurrences during ILR observation. The findings demonstrate that ILR monitoring is effective and safe in patients with unexplained, recurrent, traumatic syncope. Remote monitoring significantly reduces the time to diagnosis and significantly reduces the risk of syncope recurrence, including both traumatic and non-traumatic syncope.

Summary: This study compares device-related complications, infections, and inappropriate therapies between drug-induced type 1 Brugada Syndrome patients with transvenous-ICD (TV-ICD) and subcutaneous-ICD (S-ICD). The results show that there were no significant differences between S-ICD and TV-ICD in terms of inappropriate ICD therapies, device-related complications, and infections. However, the S-ICD group had a trend towards reduced lead-related complications.

Summary: The subsequent waves of the COVID-19 pandemic in Italy had a significant impact on cardiac care. During the first wave, there was a reduction in elective pacemaker, defibrillator, and cardiac resynchronization device implantations, as well as ablations. However, there was a gradual return to pre-pandemic volumes in the later waves, with slower recovery for ablations. Emergency procedures showed recovery during the second wave, with some variations. Remote monitoring was widely implemented during the first wave but needs further implementation.

Summary: In recent decades, the treatment of atrial fibrillation has seen significant advancements in risk prevention, anti-coagulation strategies, and improvement of quality of life. However, anti-arrhythmic drugs have been overshadowed by technological advancements, such as devices and procedures, which have become the standard of care for many patients. This paper discusses the current evidence to determine the remaining role of anti-arrhythmic drugs, their proper usage, and the potential for a personalized approach in treatment.

Electrophysiology fellowship experience and requirements: an EHRA survey

Summary: This study surveyed the current educational experience and individual requirements for electrophysiologists in training. The survey revealed that hands-on participation and observation of EP procedures are crucial, and the reputation and volume of the center, as well as the availability of a structured EP fellowship program, are the main motivations in choosing an EP fellowship institution. Most respondents expressed satisfaction with their fellowships, but areas of lesser confidence were identified. Conducting research during the fellowship was common and considered beneficial for early career development, although obstacles were reported.

If successful, this approach could have significant implications on how future clinical practice leverages consumer devices for early diagnosis and disease prevention.
