Summary: This study aims to systematically evaluate the association between artificial light at night (ALAN) exposure and asthma and allergy, mental health, and cancer outcomes. By conducting comprehensive literature searches, screenings, and data analyses, we hope to understand how ALAN exposure may affect these health outcomes.

Mini-review: Hypertussivity and allotussivity in chronic cough endotypes

Jaclyn A. Smith, Imran Satia, Huda Badri, Paul Marsden

Summary: In recent years, our understanding of the neurophysiological basis of cough has increased, as well as the concepts surrounding chronic coughing. Dysregulation of neuronal pathways mediating cough has been recognized to play a crucial role in certain phenotypes of chronic cough. This review aims to summarize current clinical evidence supporting the hyperexcitability of neuronal pathways contributing to chronic coughing and suggest their alignment with clinical features observed in patients.

Consequences of collagen induced inflammatory arthritis on circadian regulation of the gut microbiome

Devin Amanda Simpkins, Polly Downton, Kathryn J. Gray, Suzanna H. Dickson, Robert J. Maidstone, Joanne E. Konkel, Matthew R. Hepworth, David W. Ray, David A. Bechtold, Julie Elizabeth Gibbs

Summary: The gut microbiota plays an important role in host health and immune system function. Rheumatoid arthritis, an autoimmune disease, is associated with imbalances in gut microbiota. The composition and metabolism of the microbiome exhibit daily oscillations, influenced by food intake timing and circadian clock function in the gut. Inflammatory arthritis leads to changes in circadian rhythmicity in the gut and microbiome, affecting gene expression, barrier integrity, and the composition of the microbiota. These findings highlight the impact of altered circadian rhythmicity on gut integrity and microbiome function during inflammatory diseases.

A Randomized, Double-Blind, Placebo-Controlled, Parallel-Group Phase 2b Trial of P2X3 Receptor Antagonist Sivopixant for Refractory or Unexplained Chronic Cough

Lorcan McGarvey, Jaclyn A. Smith, Alyn Morice, Surinder S. Birring, Kian Fan Chung, Peter Dicpinigaitis, Akio Niimi, Michael S. Benninger, Mandel Sher, Yuko Matsunaga, Sayaka Miyazaki, Mitsuaki Machida, Hiroyuki Ishihara, Adnan Mahmood, Juan-Carlos Gomez

Summary: The goal of this study was to determine the optimal dose of sivopixant for refractory or unexplained chronic cough. The results showed that the 300 mg dose had the greatest improvements in cough frequency and patient-reported outcomes, with mild to moderate reversible taste disturbance.

Novel capsaicin cough endpoints effectively discriminate between healthy controls and patients with refractory chronic cough

Kimberley J. Holt, John Belcher, Jaclyn A. Smith

Summary: Through capsaicin cough challenges, the novel endpoints Emax and ED50 were found to effectively discriminate between healthy controls and refractory chronic cough patients, showing potential as a tool to identify patients with cough hyper-excitability.

Circadian regulation of pulmonary disease: the importance of timing

Peter S. Cunningham, Callum Jackson, Amlan Chakraborty, Jafar Cain, Hannah J. Durrington, John F. Blaikley

Summary: Circadian regulation causes biological processes to vary over a 24-h cycle. The pathological effects of this variation have been studied through pre-clinical models and observational clinical studies, providing insights into circadian mechanisms and their regulation by the molecular oscillator. This review compares and contrasts findings from these approaches in relation to four respiratory diseases, and discusses potential methods to measure circadian oscillations, which can be used as outcome measures in future human trials targeting circadian mechanisms.

Bronchoscopy for refractory/unexplained cough with mucus

James Wingfield Digby, Jenny King, Bashar Al-Sheklly, Paul Marsden, Steve Fowler, Jaclyn Smith

Summary: This study investigated the bronchoscopy, bronchoalveolar lavage (BAL) cell differential, and microbiology findings in patients with refractory/unexplained cough (RUCC) who had mucus production. The results showed that most patients had BAL neutrophilia and excessive dynamic airway collapse (EDAC), which were specific treatable traits for RUCC with mucus production.

Summary: This study aims to assess the burden of cough and its impact on quality of life in newly diagnosed IPF patients. The results indicate that cough burden in IPF is high, but cough-specific quality of life does not have prognostic value and is not associated with MUC5B promoter polymorphism.

Effect of traffic-related air pollution on cough in adults with polymorphisms in several cough-related genes

Michael Yoon, Min Hyung Ryu, Ryan D. Huff, Maria G. Belvisi, Jaclyn Smith, Chris Carlsten

Summary: In a controlled human exposure study, a genetic risk score (GRS) based on cough-related single-nucleotide polymorphisms (SNPs) was found to be correlated with cough count. This suggests that individuals with a high GRS are more susceptible to developing cough, regardless of their exposure. However, further confirmation is needed to determine if this susceptibility is more prominent in the context of traffic-related air pollution.
