C-Reactive Protein: Higher During Acute Psychotic Episodes and Related to Cortical Thickness in Schizophrenia and Healthy Controls

There is increasing evidence for the role of inflammation in schizophrenia, yet the stability of increased peripheral inflammation in acute psychosis and the degree to which peripheral inflammation relates to cortical thickness, a measure of the degree of neuropathology, are unknown. In independent samples, we assessed the peripheral inflammation marker C-reactive protein (CRP) to determine the extent to which: (1) CRP was elevated and stable across admissions for acute psychosis, (2) cognition, daily function and symptom severity are characteristic of chronically ill patients with schizophrenia displaying elevated CRP, and (3) CRP levels predict cortical thickness. Study 1 assessed peripheral CRP (primary outcome) and other blood measures in 174/280 people with acute psychosis while Study 2 assessed peripheral CRP, cognition and cortical thickness (primary outcomes), symptoms, and daily function in 85/97 chronically ill patients with schizophrenia and 71/87 healthy controls. In acute psychosis, CRP and neutrophil-to-lymphocyte ratio were significantly elevated relative to a normal cutoff (with 59.8% of patients having elevated CRP) which remained elevated across admissions. CRP was significantly elevated in 43% of chronically ill patients with schizophrenia compared to 20% in controls. Elevated CRP patients displayed significantly worse working memory and CRP was inversely correlated with cortical thickness in frontal, insula, and temporal brain regions. This work supports the role of inflammation in psychotic illnesses and suggests that use of peripheral markers (e.g., CRP) in conjunction with diagnosis could be used to identify patients with more cortical neuropathology and cognitive deficits.

Summary: A genome-wide association study identified significant genetic loci associated with suicide attempts, particularly one locus on chromosome 7 which is implicated in risk-taking behavior, smoking, and insomnia. The study also revealed strong genetic correlations between suicide attempts and psychiatric disorders, as well as other nonpsychiatric traits like smoking, pain, and lower socioeconomic status. Conditioning on psychiatric disorders reduced the genetic correlations between suicide attempts and psychiatric disorders, indicating a shared biology with nonpsychiatric risk factors.

Peripheral NF-κB dysregulation in people with schizophrenia drives inflammation: putative anti-inflammatory functions of NF-κB kinases

Summary: Elevated levels of pro-inflammatory cytokines and CRP are associated with impairments in cognitive abilities and more severe psychiatric symptoms in schizophrenia. NF-kappa B pathway genes are increased in the brains of schizophrenia patients and are closely related to neuroinflammation. IL-1 beta and IFN-gamma mRNA levels were increased in patients, while IL-6, IL-8, IL-18, and TNF-alpha mRNA levels did not differ. TLR4 and RelB appear to be more related to inflammatory status, while IKK beta and NIK are unique to high inflammation patients.

Summary: The study found that group differences in cortical area for attention-deficit/hyperactivity disorder, schizophrenia, and Child Behavior Checklist were dominant in multimodal association cortices across 11 cortical regions. These differences were associated with interregional profiles of prenatal cell-specific gene expression, implying that prenatal cell-specific processes may lead to deviations from typical brain development and vulnerability to mental illness. Additionally, genes coexpressed with radial glia and endothelial and mural cells were enriched with genes related to known pre/perinatal risk factors for psychosis, supporting a neurodevelopmental model of mental illness vulnerability.

Interventions for perinatal borderline personality disorder and complex trauma: a systematic review

Summary: Perinatal borderline personality disorder (BPD) and complex post-traumatic stress disorder (cPTSD) are associated with impaired interpersonal functioning and risk of intergenerational transmission. However, there is a lack of evaluation for interventions targeting these disorders. This systematic review examines the literature on interventions for perinatal BPD and cPTSD, finding limited evidence but suggesting positive effects of Dialectical Behavioural Therapy and multimodal therapeutic approaches. However, more research is needed to establish robust conclusions and determine the feasibility of implementing intensive interventions.

A randomized controled feasibility trial of metacognitive training with adolescents receiving treatment for anorexia nervosa

Summary: This pilot study aimed to assess the feasibility and initial efficacy of the metacognitive training for eating disorders (MCT-ED) program in adolescents with anorexia nervosa (AN). The results showed that the MCT-ED condition had a low treatment attrition rate and received positive evaluation from participants. Significant differences were observed between the MCT-ED group and the waitlist controls in terms of perfectionism and eating disorder pathology at post-intervention and 3-month follow-up.

Summary: Left-right asymmetry is an important feature of the healthy brain that may be altered in schizophrenia. This study used a large sample size and consistent methods to investigate structural brain asymmetries in schizophrenia, finding significant differences. These differences may reflect changes at the molecular, cytoarchitectonic, or circuit levels that have functional relevance for the disorder. Reduced left middle temporal cortical thickness is consistent with altered left-hemisphere language network organization in schizophrenia.
