Vitamin D and body mass index in Egyptian multiple sclerosis patients

Background: Vitamin D deficiency and obesity may be related to the pathogenesis and disease activity of multiple sclerosis (MS). This study aimed to assess the correlation between the serum level of 25(OH) vitamin D, body mass index (BMI) and the Expanded Disability Status Scale (EDSS) in a sample of Egyptian MS patients. Subjects and Methods: This was an observational study that included 130 MS patients who were recruited consecutively among the patients attending the MS unit of Ain Shams University Hospital, Cairo, in the period between December 2017 and March 2018. The serum level of 25(OH) D, BMI and EDSS were recorded. Results: : Females represented 77.7% of the study sample, the mean age was 32.4 +/- 8.2 years. MS types were: RRMS 83.1%, SPMS 14.6% and PPMS in 2.3%. Serum level of 25(OH) vitamin D was deficient (less than 10 ng/ml) in 69.2% and insufficient (10-30 ng/ml) in 19.2% of the study population. The mean BMI was 25.5 +/- 4.7 kg/m(2) (classified as overweight). The mean EDSS was 3.5 +/- 1.9. The relationship between the EDSS score and 25(OH) D level was inversely correlated, all patients with EDSS <= 2 had sufficient levels while all patients with EDSS >= 4.5 had deficient levels. High EDSS scores were statistically correlated (p < 0.001) to high BMI and low Log 25(OH) D levels. An inverse correlation was found between the BMI and log 25(OH) D. Conclusion: Vitamin D deficiency and overweight are predominant among Egyptian MS patients. The EDSS was positively correlated to the BMI and negatively correlated to 25(OH) D. These factors may possibly play a role in the pathogeneses and progression of MS in Egypt.

A pilot study of the ticagrelor role in ischemic stroke secondary prevention

Summary: This study aimed to evaluate the safety and efficacy of ticagrelor in the rapid treatment of first-ever ischemic stroke patients. The results showed that patients who received ticagrelor had better outcomes in terms of NIHSS improvement and mRS score compared to those who received aspirin, leading to a shorter hospital stay when treated within 9 hours of symptom onset.

The rate of stroke within 72 hours post-operation was low, and there were a few cases of TIA and stroke during long-term follow-up.

Vasomotor reactivity in patients with cerebral small vessel disease vs. internal border zone infarction and its correlation with disease outcome

Summary: This study compared the differences in vasomotor reactivity between lacunar infarctions and internal border zone infarctions, and correlated them with MRI perfusion, disease severity, and outcome. The results showed that patients with internal border zone infarctions had more impairment in vasomotor reactivity and MRI perfusion compared to patients with lacunar infarctions, and they also had higher clinical disability and worse prognosis.

Gender disparity versus equality in acute stroke: a Middle Eastern country hospital-based study

Summary: This study observed gender differences in acute ischemic stroke patients in Egypt and found that there is no gender disparity in acute stroke management. However, there are differences in risk factors, onset to door time, and treatment outcomes between males and females, with females having more severe conditions and less favorable outcomes when not receiving rtPA treatment. Therefore, it is important to promote early arrival and raise awareness of risk factors management.

Stroke in women: experience in a developing country

Summary: This study investigates the relationship between stroke and gender in a developing country. The results indicate that men have a higher incidence of stroke compared to women. Women who had a stroke were older and had a higher prevalence of hypertension and atrial fibrillation, as well as more severe strokes and worse outcomes. However, there was no significant difference in the promptness or frequency of revascularization therapy between genders.

A clinical comparative analysis between expanded NIHSS and original NIHSS in posterior circulation ischemic stroke

Summary: This study assesses the clinical value of e-NIHSS in posterior circulation strokes and finds that e-NIHSS has higher scores than NIHSS at baseline and on discharge. It is more sensitive in predicting 90-day functional outcome and shows high sensitivity and specificity. The study concludes that e-NIHSS is a valuable tool for diagnosis and prognosis in posterior circulation strokes.

Impairment of daily occupations in multiple sclerosis: analysis of neuroimaging, general and social cognition, and reserve

Summary: This study found that patients with multiple sclerosis (MS) have more frequent work restrictions and impairment in money management and driving abilities compared to controls. Cognitive function, physical disability, and MS lesion burden are strongly associated with work restrictions, and social cognition can influence financial capacity. Cognitive and brain reserve can help retain some of these daily occupations.

The association between sleep disorders and resilience in persons with multiple sclerosis

Summary: This study examined the differences in resilience levels among individuals with multiple sclerosis (PwMS) who had sleep disorders, sleep problems, and no sleep disorders/problems. The results showed that PwMS with sleep disorders had significantly lower resilience, with the difference being driven by the support from family and friends. These findings highlight the connection between sleep and resilience in PwMS, emphasizing the importance of social support.

Frequency and risk factors of rebound after fingolimod discontinuation - A retrospective study

Summary: This study aimed to investigate the frequency and risk factors of FTY rebound in a hospital district in Southern Finland. The results showed that 10.5% of patients experienced a rebound, and 28% of patients had relapses of any severity after discontinuing FTY. Younger age at diagnosis, longer exposure to FTY, lower lymphocyte count, and higher neutrophil to lymphocyte ratio after discontinuation were identified as risk factors for a rebound.
