The Role of Vitamin D in Fertility and during Pregnancy and Lactation: A Review of Clinical Data

Vitamin D deficiency is common and there exists a huge gap between recommended dietary vitamin D intakes and the poor vitamin D supply in the general population. While vitamin D is important for musculoskeletal health, there are accumulating data suggesting that vitamin D may also be important for fertility, pregnancy outcomes and lactation. Significant changes in vitamin D metabolism during pregnancy such as increased production of the active vitamin D hormone calcitriol support the important role of vitamin D in this setting. Observational studies show that vitamin D deficiency is a risk marker for reduced fertility and various adverse pregnancy outcomes and is associated with a low vitamin D content of breast milk. Meta-analyses of randomized controlled trials (RCTs) document that physiological vitamin D supplementation during pregnancy is safe and improves vitamin D and calcium status, thereby protecting skeletal health. Although certain RCTs and/or meta-analyses reported some other beneficial effects, it is still not clear whether vitamin D supplementation improves fertility or decreases the risk of adverse pregnancy outcomes such as low birth weight, pre-eclampsia and neonatal mortality, or reduces wheeze/asthma in the infants. Nevertheless, vitamin D supplementation in pregnant women is frequently required to achieve a sufficient vitamin D status as recommended by nutritional vitamin D guidelines. In this review, we provide an overview of systematic reviews, meta-analyses and large trials reporting clinical data on the role of vitamin D for fertility, pregnancy and lactation.

Different patterns of changes in free 25-hydroxyvitamin D concentrations during intermittent fasting among meat eaters and non-meat eaters and correlations with amino acid intake

Summary: This study prospectively assessed changes in free 25-hydroxyvitamin D [25(OH)D] and vitamin D binding protein (VDBP) among overweight adults who followed a pescatarian Orthodox intermittent fasting regimen and controls who followed a low-fat 12:12 diet. The findings suggest that diet can affect free 25(OH)D concentrations, independently of exposure to sunlight, through variations in amino acid intake, providing novel mechanistic insights into the future planning of vitamin D supplementation strategies. However, larger studies are needed to test this hypothesis.

Approach to the Patient: Management of Parathyroid Diseases Across Pregnancy

Summary: Taking care of pregnant patients with parathyroid disorders requires consideration of changes in bone and mineral metabolism. The diagnostic and therapeutic approaches differ from the nonpregnant population. Parathyroidectomy is recommended for primary hyperparathyroidism before pregnancy to reduce complications. Conservative treatment and active surveillance are recommended for hypoparathyroidism. Postpartum monitoring is necessary, as there is an increased risk of hypercalcemia.

The Efficacy and Safety of a Low-Dose Tranexamic Acid Bolus-Only Protocol Compared with the Moderate-Dose Protocol in Valvular Heart Surgery

Summary: We compared a moderate dose of tranexamic acid (TXA) protocol with a 1.5-g bolus-only protocol in 873 propensity score-matched pairs of valvular heart surgery patients. The number of transfused patients and transfused red blood cell concentrates was higher in the bolus-only protocol group. The risks of re-exploration and convulsive seizures were similar between the two groups.

Intermittent Fasting in Breast Cancer: A Systematic Review and Critical Update of Available Studies

Summary: This systematic review examined the impact of intermittent fasting (IF) on breast cancer patients. It found that IF may have positive effects on quality of life, radiological response, and cancer recurrence during chemotherapy, but the data quality was low and further research is needed for validation.

Summary: Accumulating evidence suggests an association of the TRP-KYN pathway with atherosclerosis and cardiovascular risk factors. Linear regression analyses showed significant associations between TRP, KYN, QUIN, and HDL, as well as smoking status and TRP/KYN pathway parameters. Further studies are needed to explore the diagnostic and therapeutic potential of the TRP-KYN pathway for cardiovascular diseases.

Summary: It is found that vitamin D deficiency is prevalent among the Polish population based on epidemiological studies. To address this concern, a Polish multidisciplinary group reviewed the evidence and formulated recommendations for prevention and treatment of vitamin D deficiency. The final Polish recommendations included a range of serum vitamin D levels indicating deficiency, guidelines for prophylaxis and treatment, and the importance of improving the overall population's vitamin D status and treating at-risk patient groups.

Long-term supplementation with 3200 to 4000 IU of vitamin D daily and adverse events: a systematic review and meta-analysis of randomized controlled trials

Summary: This study summarized the harmful effects of a daily vitamin D supplement of 3200-4000 IU, showing that this dosage increases the risk of hypercalcemia and other adverse events in some individuals.

Vitamin D deficiency and driveline infection in patients with a left ventricular assist device implant

Summary: This study investigated the association between vitamin D deficiency and driveline infection in patients with continuous flow LVAD implants. The results showed that patients with circulating 25-hydroxyvitamin D concentrations < 25 nmol/L had a higher risk of driveline infection. Although endocrine regulators of calcium and phosphorus metabolism were not significantly associated with the risk of infection, these data suggest that deficient vitamin D status in LVAD recipients may predict the occurrence of driveline infection.

Protein intake and bone health: an umbrella review of systematic reviews for the evidence-based guideline of the German Nutrition Society

Summary: This umbrella review aimed to assess the impact of protein intake exceeding the current recommendations for different age groups on bone health. The review analyzed systematic reviews of studies published between 2008 and 2021, evaluating the effect of both animal and plant protein. The overall certainty of evidence was found to be insufficient, except for a possible link between high protein intake and reduced hip fracture risk.

High Efficacy and Low Early Mortality in High-Volume Center Tricuspid Valve Surgery

Summary: By analyzing the data from 369 patients undergoing isolated tricuspid valve surgery, the study found that the surgical approaches and pre- and postoperative valve function were associated with the expected mortality rates. The majority of patients showed improved tricuspid valve function postoperatively. Randomized controlled trials are needed to compare the long-term outcomes of surgical and interventional procedures.

Summary: This study aimed to evaluate the effect of prophylactic left ventricular (LV) unloading with Impella pump during VA-ECMO therapy for refractory cardiogenic shock. The results showed that compared with the bail-out unloading strategy, prophylactic unloading was associated with lower 30-day mortality risk and higher probability of myocardial recovery.

25(OH)D Concentration in Neonates, Infants, Toddlers, Older Children and Teenagers from Poland-Evaluation of Trends during Years 2014-2019

Summary: This study aimed to assess the concentration of 25(OH)D and its potential changes during the growth and maturation period in children. The study found that 18.9% of the subjects had vitamin D deficiency from 2014 to 2019. Effective prevention of vitamin D deficiency was observed in children aged 3 years and younger. There was no relationship between the concentrations of 25(OH)D and calcium.
