The prognosis of invasive fungal infections (IFI) depends on the speed of diagnosis and treatment. Conventional diagnostic methods are of low sensitivity, laborious and too slow, leading to the need for new, faster, and more efficient diagnostic strategies. There are several techniques for diagnosing a candidemia that are faster than the conventional blood culture (BC). Once yeast growth in BC is detected, species identification can be speeded up by mass spectrometry (30 minutes), commercialised molecular techniques (60-80 minutes) or fluorescent in situ hybridization (90 minutes). The combined detection of biomarkers (antimicellium, mannan and antimannan or beta-glucan) has shown to be of greater use than their individual use. Commercialised nucleic acid amplification techniques (Septifast (R), T2Candida (R)) are very reliable alternatives to BC. The detection of the capsular antigen of Cryptococcus, by means of latex agglutination or immuno-chromatography, is a valuable technique for cryptococcosis diagnosis. Direct microscopic examination and culture of representative specimens is used for the conventional diagnosis of IFI by filamentous fungi. Detection of galactomannan and beta-glucan are considered diagnostic criteria for probable invasive aspergillosis and probable IFI, respectively, despite the lack of specificity of the latter. The detection of fungal volatile organic compounds in breath is an interesting diagnostic strategy in pulmonary infections. Although widely used, nucleic acid detection techniques are not considered diagnostic criteria for IFIs caused by moulds in consensus documents, due to their lack of standardisation. However, they are the only alternative to culture methods in invasive infections by Scedosporium/Lomentospora, Fusarium, zygomycetes, or dematiaceous fungi. (C) 2018 Asociacion Espahola de Micologia. Detection of azole resistance in Aspergillus fumigatus complex isolates MALDI-TOF mass

Summary: The study demonstrates the use of matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) to accurately detect azole resistance in species of the Aspergillus fumigatus complex. The results show that MALDI-TOF MS combined with peak analysis is a novel tool that allows the differentiation of A. fumigatus from other species within the complex and the detection of azole-resistant A. fumigatus isolates.

Characterization of the Differential Pathogenicity of Candida auris in a Galleria mellonella Infection Model

Summary: This study used an in vivo insect model to evaluate the virulence of Candida auris, an emergent multidrug-resistant fungal pathogen. It was found to have an intermediate degree of virulence compared to other known Candida species, with significant variability between strains. The ability of C. auris to filament in vivo and its early dissemination through the host, with a remarkable respiratory tropism, were also documented.

Summary: This study assessed the in vitro activity of Ibrexafungerp and other drugs against 434 blood isolates of Candida in Europe. The results showed that Ibrexafungerp exhibited potent activity against these isolates and also showed activity against fluconazole- and echinocandin-resistant isolates.

Novel Chromogenic Medium CHROMagarTM Candida Plus for Detection of Candida auris and Other Candida Species from Surveillance and Environmental Samples: A Multicenter Study

Summary: The CHROMagar(TM) Candida Plus (CC-Plus) medium is able to rapidly identify most clinical Candida species and differentiate C. auris. A multicentric study demonstrated that CC-Plus performed well in detecting C. auris and other clinically relevant Candida species in surveillance and environmental cultures.

Gradient diffusion strips for detecting azole resistance in Aspergillus fumigatus sensu lato

Summary: This study evaluated the performance of gradient diffusion strips (GDSs) in detecting azole resistance in Aspergillus fumigatus. The results showed that GDSs can effectively identify azole-resistant isolates of A. fumigatus sensu stricto, but have poor performance against cryptic species.

Usefulness of Chromogenic Media with Fluconazole Supplementation for Presumptive Identification of Candida auris

Summary: This study describes the morphological characteristics of C. auris colonies on three commercial chromogenic media. CHROMagar (TM) Candida Plus and HiCrome (TM) Candida can provide presumptive identification of C. auris, but identification may be difficult in the presence of mixed cultures. The use of CHROMagar (TM) Candida medium with 32 mg/L fluconazole offers better performance for the identification of C. auris by inhibiting fluconazole-susceptible strains and selecting rare or high fluconazole MIC (>32 mg/L) isolates.

Summary: Fluorogenic probes based on oligonucleotide capped nanoporous anodic alumina films were developed for specific and sensitive detection of human papilloma virus (HPV) DNA. The probes showed high sensitivity and selectivity in detecting 14 different high-risk (hr) HPV types in clinical samples. The optimized synthesis protocol allowed for the production of sensors with high reproducibility and reliable fluorescence signal reading.

Aptamer-Capped Nanoporous Anodic Alumina for SARS-CoV-2 Spike Protein Detection

Summary: The COVID-19 pandemic has emphasized the importance of testing and tracking infected individuals to mitigate the virus spread. This study introduces a biosensor that uses oligonucleotide-gated nanomaterials for specific detection of the SARS-CoV-2 spike protein. The biosensor showed high sensitivity and successfully detected low concentrations of the pseudotype virus. It was also tested on nasopharyngeal samples from suspected COVID-19 individuals.

IFISTRATEGY: Spanish National Survey of Invasive Fungal Infection in Hemato-Oncologic Patients

Summary: Recent years have seen an increase in invasive fungal infections (IFIs), particularly caused by non-Candida albicans species, non-Aspergillus molds, and azole-resistant Aspergillus fumigatus. To determine the most common management strategies for IFIs, a cross-sectional multicenter survey involving 55 hematologists and infectious disease specialists in 31 Spanish hospitals was conducted. Key strategies identified include early treatment for persistent febrile neutropenia, switching to another broad-spectrum antifungal if azole-resistant Aspergillus is suspected, prophylactic use of broad-spectrum azoles and echinocandins in patients receiving midostaurin or venetoclax, and the use of liposomal amphotericin B for breakthrough IFIs after prophylaxis with echinocandins in patients receiving new targeted therapies. When antifungals fail to reach adequate levels and invasive aspergillosis is suspected, combining antifungals from different families is deemed appropriate.

Climate change, animals, and Candida auris: insights into the ecological niche of a new species from a One Health approach
