Summary: This study examined the diagnostic stability of first-episode psychosis (FEP) diagnoses over a long-term follow-up and found that most FEP diagnoses changed over time. Family history, obstetric complications, developmental delay, and other factors were associated with the change to schizophrenia, while these factors had the opposite effect on the change to bipolar disorder. These findings suggest that FEP diagnoses other than schizophrenia or bipolar disorder should be considered provisional.

Summary: By using Connectivity-Based Parcellation (CBP) method, this study examined the differentiation of hippocampal subregions during childhood and adolescence. The results showed that in late childhood, the hippocampus mainly differentiated along the anterior-posterior axis, while in adolescence, a differentiation along the medial-lateral axis was evident. Further analysis revealed that the hippocampal head is related to higher order functions in late childhood, while posterior subicular networks are associated with action-oriented and reward systems in early adolescence.

Summary: The study found that childhood-onset schizophrenia (COS) patients have a longer illness course, more frequently insidious onset, less frequent delusions, more severe negative symptoms, and bizarre behaviors than adolescent-onset schizophrenia (AOS) patients. The rate of improvement in psychotic symptoms after hospitalization for COS and AOS patients were 38.3% and 47.8% respectively, with a statistically significant difference in efficacy between the two groups. The predictors of treatment efficacy in EOS individuals include days of hospitalization, age of onset, presence of flat affect, PANSS total and negative score at admission.

Previous drugs targeting D1R have faced issues like poor bioavailability and insufficient target engagement at tolerable doses, but newer drugs like CVL-562 offer potential solutions if carefully tested.

Summary: Duration of untreated psychosis (DUP) is a critical period for patients, and longer DUP can lead to poorer outcomes. This column discusses a quality improvement intervention aimed at reducing the delay between eligibility confirmation and admission to care within a benchmark period of 7 days. Over 4 years, the median delay significantly decreased and the proportion of admissions meeting the benchmark increased. This intervention provides a sustainable model for reducing wait times at co-ordinated specialty care (CSC) services.

Summary: Structural alterations or quantitative abnormalities of mitochondrial ion channels and exchangers are associated with mental illness. Levels of mitochondrial calcium ion channel proteins in plasma neuron-derived extracellular vesicles (NDEVs) were assessed in patients with first episodes of psychosis (FP). The study found significant differences in the levels of certain proteins, which have been linked to neurodevelopmental disorders, mental retardation, schizophrenia, and major depressive diseases. Understanding the altered calcium homeostasis in schizophrenia could lead to improved diagnosis and treatment.

Summary: This study investigated the neural processing of eye-contact in individuals with autism spectrum disorder (ASD) using eye tracking technology. The results showed that individuals with ASD exhibited differences in neural responses to eye-contact, which was related to sensory and anxiety symptoms. When the eye-contact was predictable, individuals with ASD showed attenuated neural responses. These findings highlight the potential role of anticipation, expectation, and gaze perception in the vulnerabilities of eye-contact during social interactions in ASD.
