Summary: Nutritional therapy for critically ill patients should be tailored to individual patients, focusing on the phase of critical illness, metabolic tolerance, leading symptoms, and comorbidities. Personalized and disease-specific nutritional therapy in the ICU requires an interdisciplinary approach and frequent reevaluation for optimal clinical outcomes.

Summary: Following the new ESPEN Standard Operating Procedures, the previous 2019 guideline on medical nutrition therapy for critically ill patients has been shortened and partially revised. This publication proposes a practical guideline based on the published scientific guideline, but with shortened and illustrated flow charts. The main goal of this practical guideline is to increase understanding and facilitate implementation of Nutrition in the ICU guidelines, including all items discussed in the previous guidelines and special conditions.

Summary: In pediatric intensive care units, malnutrition screening is crucial for critically ill children. The Children's Wisconsin Nutrition Screening Tool (CWNST) is a more sensitive tool compared to the Pediatric Nutrition Screening Tool (PNST) and can predict nutrition risk in PICU patients through electronic medical records.

Summary: Higher nutritional risk at ICU admission is associated with disability in survivors of critical illness, but the long-term follow-up did not show significant correlation. The mNUTRIC scores are related to discharge destination and disability in basic ADLs but not consistently associated with cognition and health-related quality of life.

Summary: The study found that delayed enteral nutrition (EN) ordering in ICU patients was associated with factors such as the use of vasopressors and orders placed by physicians. Delayed EN ordering was linked to shorter ICU-free days, longer ICU admissions, and longer hospitalizations.

Summary: This study compared the applicability of French (2014) and American (2017) pediatric guidelines in a PICU setting. It found that hemodynamic failure was a risk factor for non-application of both guidelines. Additionally, the study suggested that the American 2017 guidelines, which utilize predictive Schofield equations, may be more useful and applicable than the higher recommendations of the 2014 guidelines. Further multicenter studies to validate pediatric guidelines are deemed necessary.

Summary: This study aimed to describe the nutrition processes of care from ICU admission to discharge from inpatient rehabilitation. The findings showed that the length of stay in rehabilitation was longer than in the ICU or acute wards, but patients in rehabilitation were assessed the least by a dietitian and the time to assessment was the longest. Nutrition impacting symptoms were common during the rehabilitation period, indicating a need for further investigation of nutrition interventions in critical illness beyond acute care settings.

Summary: This study aimed to investigate the causes, frequency, and periods of enteral nutrition interruption (ENI) in the ICU. The study found that unplanned ENIs were most common in the first 3 days of ICU admission, with diagnostic reasons being the main cause. Approximately 25% of patients failed to meet their calculated caloric and protein requirements during the first 4 days of admission.

Summary: Personalization of ICU nutrition is important for the future of critical care. Guidelines recommend low-dose enteral or parenteral nutrition within 48 hours of admission, with consideration for alternative routes when necessary. Indirect calorimetry should be used to measure energy expenditure, and protein delivery should be adjusted based on patient stability. Micronutrient evaluation and muscle monitoring are also important considerations in post-ICU nutrition. Further research is needed on specialized anabolic nutrients and rehabilitation interventions for post-ICU recovery.

Summary: This study found that the use of immunonutrition formulas during ICU stay may be associated with improved outcomes. Patients who received immunonutrition formulas had higher caloric and protein intake, as well as better survival rates. Furthermore, the use of immunonutrition was also related to lower need for vasopressor support and continuous renal replacement therapies.

Summary: Mitochondria play a crucial role in providing energy and participating in metabolism and signaling processes. However, mitochondrial dysfunction in critical illness can lead to organ failure and muscle wasting. This review aims to discuss the molecular mechanisms underlying mitochondrial dysfunction in intensive care unit-acquired weakness and critical illness myopathy, and its implications for muscle function and potential therapeutic approaches.

Summary: Nutrition support is a crucial therapy that aims to meet the nutrition requirements of individuals who cannot meet their needs through oral intake alone. Registered dietitian nutritionists play an integral role in providing information and setting professional standards to ensure competent practice in nutrition support. The Standards of Practice and Standards of Professional Performance help RDNs assess their practice and identify areas for improvement in various settings.

Summary: This study investigated the sleep patterns of patients receiving home parenteral nutrition (HPN) treatment. The results showed that most patients experienced disrupted sleep, primarily due to difficulty maintaining sleep. Although the extent to which HPN contributed to poor sleep was unclear, addressing factors that contribute to sleep disturbances and considering sleep interventions may improve the quality of life for HPN-dependent patients.

Summary: This survey evaluated the application of nutrition assessment in patients with acute or chronic illnesses, and identified the lack of standardization in current practices. Parameters such as nutrient intake and weight were widely used in the assessment of both adult and pediatric patients. The survey found that malnutrition rates were 32%-40% in adults and 4%-30% in pediatric patients. Most patients received nutrition interventions, but 30% did not receive appropriate interventions.

Summary: Serum albumin and prealbumin are not reliable markers for nutrition assessment as they mainly reflect inflammation in the body instead of nutritional status or protein-energy malnutrition. Inflammation in critical or chronic illnesses leads to a redistribution of serum proteins and lower concentrations of albumin and prealbumin, making them better indicators of patient risk for adverse outcomes.
