Pathophysiology of Critical Illness and Role of Nutrition

Critical illness is a hypercatabolic state. It has been hypothesized that timely and adequate nutrition support may optimize the host response and thereby minimize nutritionally related complications while improving overall outcome. Any illness in due course can lead to a malnourished statecritical illness can worsen this state as patients may become immunocompromised and unable to mount an adequate inflammatory response and therefore susceptible to poor outcomes. Data indicate that prevalence of malnutrition in the ICU ranges from 38% to 78% and is independently associated with poor outcomes. Hence, exploring the role of nutrition as a way to mitigate critical illness is important. In this review, the basic pathophysiology of critical illness and how it alters carbohydrate, protein, and fat metabolism are discussed. This is followed by a discussion of malnutrition and how it affects patient and hospital outcomes. Finally, a summary of the available evidence regarding nutrition support and its impact on outcomes are provided. This review is not intended to provide practice-based guidelines; instead, it intends to highlight available data on the role of nutrition support in critically ill patients.

Sleep patterns of patients receiving home parenteral nutrition: A home-based observational study

Summary: This study investigated the sleep patterns of patients receiving home parenteral nutrition (HPN) treatment. The results showed that most patients experienced disrupted sleep, primarily due to difficulty maintaining sleep. Although the extent to which HPN contributed to poor sleep was unclear, addressing factors that contribute to sleep disturbances and considering sleep interventions may improve the quality of life for HPN-dependent patients.

Key approaches to diagnosing malnutrition in adults

Current nutrition assessment practice: A 2022 survey

Summary: This survey evaluated the application of nutrition assessment in patients with acute or chronic illnesses, and identified the lack of standardization in current practices. Parameters such as nutrient intake and weight were widely used in the assessment of both adult and pediatric patients. The survey found that malnutrition rates were 32%-40% in adults and 4%-30% in pediatric patients. Most patients received nutrition interventions, but 30% did not receive appropriate interventions.

Concomitant use of monoamine oxidase inhibitor and tyrosine in parenteral nutrition

The Use of Visceral Proteins as Nutrition Markers: An ASPEN Position Paper

Summary: Serum albumin and prealbumin are not reliable markers for nutrition assessment as they mainly reflect inflammation in the body instead of nutritional status or protein-energy malnutrition. Inflammation in critical or chronic illnesses leads to a redistribution of serum proteins and lower concentrations of albumin and prealbumin, making them better indicators of patient risk for adverse outcomes.
