Marginalization: Conceptualizing patient vulnerabilities in the framework of social determinants of health-An integrative review

Scientific advances in health care have been disproportionately distributed across social strata. Disease burden is also disproportionately distributed, with marginalized groups having the highest risk of poor health outcomes. Social determinants are thought to influence health care delivery and the management of chronic diseases among marginalized groups, but the current conceptualization of social determinants lacks a critical focus on the experiences of people within their environment. The purpose of this article was to integrate the literature on marginalization and situate the concept in the framework of social determinants of health. We demonstrate that social position links marginalization and social determinants of health. This perspective provides a critical lens to assess the societal power dynamics that influence the construction of the socio-environmental factors affecting health. Linking marginalization with social determinants of health can improve our understanding of the inequities in health care delivery and the disparities in chronic disease burden among vulnerable groups.

Summary: The study examined the impact of social determinants of health on self-care maintenance in HF patients, finding that marital status and race were significant determinants, with Whites having the highest education level. However, these factors only explained a small portion of the variance in self-care maintenance.

Summary: This study aims to investigate the relationship between neighborhood social conditions, stress-related neural activity, vascular function, and immune system activation through objective measurements of physical activity, sedentary behavior, and neighborhood stress among White and African American women in Washington, DC. The research will recruit 60 healthy women from high and low socioeconomic status neighborhoods in the DC area, exploring the impact of adverse neighborhood environments on biological and psychological health outcomes.

Summary: The study suggests that the effectiveness of smartphone apps in helping people locate physical activity venues depends on the level of user engagement. Only when more than 75% of women are aware of and engage with the app can significant improvements in physical activity levels and reductions in obesity prevalence be seen over a 5-year period.

A Mixed-Methods Study of Social Determinants and Self-care in Adults With Heart Failure

Summary: This study explored the relationship between social determinants of health (SDoH) and self-care in patients with heart failure (HF). The study found that money and resources were important factors influencing self-care maintenance and symptom perception. One-on-one interviews revealed that social connectedness, health insurance coverage, individual upbringing, and personal experiences facilitated self-care behavior.
