Higher vs. lower haemoglobin threshold for transfusion in septic shock: subgroup analyses of the TRISS trial

Background: Using a restrictive transfusion strategy appears to be safe in sepsis, but there may be subgroups of patients who benefit from transfusion at a higher haemoglobin level. We explored if subgroups of patients with septic shock and anaemia had better outcome when transfused at a higher vs. a lower haemoglobin threshold. Methods: In post-hoc analyses of the full trial population of 998 patients from the Transfusion Requirements in Septic Shock (TRISS) trial, we investigated the intervention effect on 90-day mortality in patients with severe comorbidity (chronic lung disease, haematological malignancy or metastatic cancer), in patients who had undergone surgery (elective or acute) and in patients with septic shock as defined by the new consensus definition: lactate above 2 mmol/l and the need for vasopressors to maintain a mean arterial pressure above 65 mmHg. Results: The baseline characteristics were mostly similar between the two intervention groups in the different subgroups. There were no differences in the intervention effect on 90-day mortality in patients with chronic lung disease (test of interaction P = 0.31), haematological malignancy (P = 0.47), metastatic cancer (P = 0.51), in those who had undergone surgery (P = 0.99) or in patients with septic shock by the new definition (P = 0.20). Conclusion: In exploratory analyses of a randomized trial in patients with septic shock and anaemia, we observed no survival benefit in any subgroups of transfusion at a haemoglobin threshold of 90 g/l vs. 70 g/l.

Associations between enteral nutrition and outcomes in the SUP-ICU trial: Protocol for exploratory post hoc analyses

Summary: Critically ill patients are at risk of gastrointestinal bleeding, and pharmacological stress ulcer prophylaxis (SUP) is commonly used to minimize this risk. The effect of enteral nutrition as SUP on GI bleeding rates is uncertain, with conflicting data and variable practice. This study aims to explore the association between enteral nutrition and GI bleeding rates in ICU patients and evaluate any interactions with pharmacologic SUP. The results may provide important insights for guiding practice and designing future clinical trials on the effect of enteral nutrition on GI bleeding.

Summary: This systematic review of randomized clinical trials found that dexmedetomidine may reduce the occurrence of serious adverse events and delirium in intensive care or post-operative care units. However, there was no conclusive evidence for its effects on all-cause mortality. The certainty of evidence ranged from very low for delirium to low for the remaining outcomes.

Endothelial Cell Phenotypes Demonstrate Different Metabolic Patterns and Predict Mortality in Trauma Patients

Summary: In trauma patients, shock-induced endotheliopathy (SHINE) is associated with poor prognosis. This study identified four metabolic phenotypes in trauma patients and analyzed their intracellular metabolic profiles using a computational network analysis. One of the phenotypes, characterized by an inability to maintain redox balance, was associated with increased mortality. This research highlights the importance of understanding metabolic patterns in trauma patients for prognosis and potential therapeutic interventions.

Abnormal serum levels of magnesium, phosphate, and zinc in ICU patients-Characteristics, management, and outcomes: The WhyTrace cohort study

However, the associations between serum levels and clinical outcomes were inconclusive due to insufficient data.

Summary: This study compared the long-term outcomes of restrictive versus standard IV fluid therapy in adult ICU patients with septic shock. High-dimensional proteomics identifies organ injury patterns associated with outcomes in human trauma

Summary: Severe traumatic injury can lead to direct and indirect organ injury, but there is a lack of tissue-specific biomarkers in clinical practice. This study used proteomic and metabolomic databases to identify organ injury patterns after severe injury in humans. The results showed that nonresolvers had a wide release of tissue-specific damage markers at admission, and at 72 hours, cardiac, liver, neurologic, and pulmonary injury biomarkers remained significantly elevated in nonresolvers compared with resolvers.

Summary: This study found associations between the ABO and RhD blood group systems and susceptibility to various diseases, including tongue cancer, monocytic leukemia, cervical cancer, osteoarthrosis, asthma, as well as HIV and hepatitis B infection. Additionally, marginal evidence of associations between blood groups and age at first diagnosis was found.

The Effect of Targeted Temperature Management on the Metabolome Following Out-of-Hospital Cardiac Arrest

Summary: Targeted temperature management (TTM) may reduce injury from out-of-hospital cardiac arrest by slowing metabolism. However, studies have found higher lactate levels in patients cooled to 33 degrees C compared with 36 degrees C even days after TTM cessation. Larger studies on the effect of TTM on the metabolome have not been conducted.

Summary: The study aimed to examine changes in biomarkers of endothelial glycocalyx shedding, endothelial damage, and surgical stress following major open abdominal surgery and their correlation to postoperative morbidity. The findings revealed that major abdominal surgery significantly increased levels of biomarkers associated with endothelial glycocalyx shedding, endothelial damage, and surgical stress, with higher levels observed in patients developing high postoperative morbidity.
