Summary: This study analyzed the indication for and effect of fresh frozen plasma (FFP) transfusion in patients with septic shock. The results showed that early FFP administration did not have significant harm or associated benefit in septic shock, and plasma should only be used in patients with a strong indication according to current recommendations.

Summary: This study investigated the association between transfusion trigger and other transfusion parameters with mortality and adverse outcomes in critically ill sepsis patients. The results suggest that acute physiology and chronic health evaluation, cumulative fluid balance, and admission platelet count are predictors of 28 and 90 day mortality. The study concludes that a transfusion trigger below 7 g/dl does not affect adverse outcomes in sepsis patients.

Summary: This study aimed to investigate whether observational data could replicate the results of randomized controlled trials and be generalized to broader specialties and a lower transfusion threshold. The results of the study showed that transfusion at a stable hemoglobin concentration of 7-10 g/dL did not alter surgical outcomes.

Summary: This study evaluated the association between admission-shock-onset-time and in-hospital mortality among patients with septic shock and determined the optimal cutoff period to categorize early- and late-onset septic shock. The results showed that in-hospital mortality increased as admission-shock-onset-time increased in patients with septic shock. The adjusted odds ratio for in-hospital mortality exceeded 2 between 20.1 and 54.6 hours and surpassed 3 between 54.6 and 148.4 hours.

Summary: Critically ill patients are at risk of gastrointestinal bleeding, and pharmacological stress ulcer prophylaxis (SUP) is commonly used to minimize this risk. The effect of enteral nutrition as SUP on GI bleeding rates is uncertain, with conflicting data and variable practice. This study aims to explore the association between enteral nutrition and GI bleeding rates in ICU patients and evaluate any interactions with pharmacologic SUP. The results may provide important insights for guiding practice and designing future clinical trials on the effect of enteral nutrition on GI bleeding.

Summary: In trauma patients, shock-induced endotheliopathy (SHINE) is associated with poor prognosis. This study identified four metabolic phenotypes in trauma patients and analyzed their intracellular metabolic profiles using a computational network analysis. One of the phenotypes, characterized by an inability to maintain redox balance, was associated with increased mortality. This research highlights the importance of understanding metabolic patterns in trauma patients for prognosis and potential therapeutic interventions.

The results showed that most ICU patients had low serum levels of magnesium, phosphate, or zinc, and some received supplementation. Summary: Severe traumatic injury can lead to direct and indirect organ injury, but there is a lack of tissue-specific biomarkers in clinical practice. This study used proteomic and metabolomic databases to identify organ injury patterns after severe injury in humans. The results showed that nonresolvers had a wide release of tissue-specific damage markers at admission, and at 72 hours, cardiac, liver, neurologic, and pulmonary injury biomarkers remained significantly elevated in nonresolvers compared with resolvers.

Summary: This study reports coagulation profiles stratified by age and finds an association between age and hemostatic capability. Although similar patterns of coagulation abnormalities are observed across age groups following severe injury/shock, thresholds for single clotting factors differ. These age-related differences may need to be considered in clinical treatments (e.g., transfusion therapy).
