Summary: Screening for early-stage transthyretin cardiac amyloidosis in patients with prior surgery for bilateral carpal tunnel syndrome showed a clinical yield of approximately 5%. The yield was higher (>1 in 5) when focusing on nonobese men aged 70 years or older, suggesting the potential for systematic screening.

Summary: Carpal tunnel release is a commonly performed procedure, with most patients experiencing improvement in their symptoms. However, there is a small subset of patients who may experience failure of the initial surgery. The management of these patients involves thorough clinical examination, electrodiagnostic studies, and step-wise surgical approaches for revision surgery.

Summary: This study aimed to evaluate the correlation between carpal tunnel pressure (CTP) and the clinical presentations in patients with carpal tunnel syndrome (CTS), as well as explore the possible predictors for the postoperative recovery pattern. The results showed that preoperative CTP was well correlated with the clinical presentations and could be a useful predictor for the postoperative clinical recovery pattern.

Summary: Bilateral carpal tunnel syndrome patients, particularly those with left ventricular hypertrophy, may have a higher risk of transthyretin cardiac amyloidosis. Screening with electrocardiography and echocardiography appeared feasible and effective, especially in patients with LVH. Early detection of the disease may be achieved through this method.

Summary: This study compared the efficacy of local steroid injections (LSIs) and carpal tunnel release (CTR) for the treatment of carpal tunnel syndrome (CTS). The results showed that both methods were effective and feasible, with LSIs providing better pain relief. However, this superiority was not permanent, so LSIs may be considered in patients with CTS, but CTR should be recommended.

Summary: This study compared the effects of wrist immobilization and immediate wrist mobilization on the outcomes of endoscopic carpal tunnel release surgery. The results showed that wrist immobilization can alleviate pain and improve grip and pinch strength, but no significant difference was observed at the final follow-up.

Summary: This study investigated changes in the median nerve, retinaculum, and carpal tunnel on ultrasound after successful endoscopic carpal tunnel release. Despite clinical improvement after ECTR, nearly all ultrasound parameters remained abnormal at one year post-ECTR, indicating they should not be relied upon to diagnose persistent CTS.

Summary: Research has shown that performing carpal tunnel release (CTR) using the wide-awake, local-anesthesia, no-tourniquet (WALANT) technique is a cost-effective, safe, and straightforward option. It offers cost savings and workflow efficiencies, can be performed in an outpatient clinic with minimal resource use and waste production, and consistently provides standard or better post-operative pain control and patient satisfaction.

Summary: For patients with primary surgery failure for carpal tunnel syndrome (CTS), hydrodissection with 5% dextrose may be an effective treatment method for persistent or recurrent CTS after surgery. The treatment has shown positive long-term effectiveness, with a significant improvement in symptoms reported by the majority of patients.

Summary: Nearly 40,000 carpal tunnel releases (CTRs) are performed annually in Japan, with open CTR being performed almost 4 times more often than endoscopic CTR. The incidence of CTR peaks in the 80-84 age range, and there is a mild correlation between the number of endoscopic CTRs and the number of hand surgery specialists.

Summary: Carpal tunnel syndrome (CTS) is a common compressive neuropathy worldwide. This study conducted a systematic review to analyze the clinical and surgical outcomes of bilateral CTS treatment. It found that both clinical and surgical treatments can improve symptom severity, functional status, and pain intensity in patients with bilateral CTS during one-and three-month follow-up periods.

Summary: The purpose of this case-series study was to assess the outcomes of surgery for intramuscular haemangiomas in the extremities. The results showed that operative treatment was effective in reducing pain and improving grip strength and hand function. The study concluded that surgical treatment of these rare tumors is safe and effective, although it may be challenging in some cases.

Summary: This article presents the results of using the kite flap for the reconstruction of soft tissue defects around the thumb. The study showed that all 14 patients who underwent the kite flap procedure had successful healing of the flaps. After a mean follow-up of 3 years, all patients expressed satisfaction with the surgical outcomes. Although some patients experienced slightly limited thumb mobility, it did not significantly affect hand function.

Summary: Benign tumours of the upper extremity, such as giant-cell tumours of the tendon sheath and lipomas, are common in hand surgeons' practice. This study investigated the distribution of these tumours, their symptoms, and surgical outcomes, particularly recurrence rates. A total of 346 patients who underwent surgery for upper extremity tumours were enrolled, with the most common tumours being giant cell tumour of the tendon sheath and lipoma. Recurrence was noted in 23% of cases, with the highest rates seen in surgeries for rheumatoid nodules and giant-cell tumours of the tendon sheath. Factors associated with increased recurrence risk included histological type of the lesion and incomplete or non-en bloc resection.

Summary: This study aimed to assess the concentration of vitamin D and its metabolites in patients before thyroidectomy. The results showed that patients undergoing thyroidectomy have a marked vitamin D deficiency, which may affect their subsequent convalescence and prognosis. Therefore, determining vitamin D levels prior to thyroidectomy may be useful for considering supplementation in the clinical management of these patients.

Summary: Schwannoma is a nerve tumor originating from Schwann-cells of the nerve sheath. This study aimed to analyze the outcomes of surgery for upper limb Schwannomas in the authors' institution. The results showed generally good outcomes for the rare tumors, but functional deficits should be considered in cases involving motoric nerves.
