The increased expression of the inducible Hsp70 (HSP70A1A) in serum of patients with heart failure and its protective effect against the cardiotoxic agent doxorubicin

The aim of this study was to examine the potential association between the expression of Hsp70 protein and heart failure and to investigate the possible protective effect of Hsp70 against the doxorubicin-induced toxicity. Initially, at clinical level, the expression levels of the inducible Hsp70 were quantified in serum from patients with heart failure. Our results showed that in heart failure, Hsp70 concentration appeared to be increased in blood sera of patients compared to that of healthy individuals. The enhanced expression of Hsp70 in serum of patients with heart failure seemed to be associated with various features, such as gender, age and the type of heart failure, but not with its etiology. Next, in our study at cellular level, we used primary cell cultures isolated from embryos of Hsp70-transgenic mice (Tg/Tg) overexpressing human HSP70 and wild-type mice (F1/F1). After exposure to a wide range of doxorubicin concentrations and incubation times, the dose- and time-dependent toxicity of the drug, which appeared to be reduced in Tg/Tg cells, was demonstrated. In addition, doxorubicin administration appeared to result in a dose- and time-dependent decrease in the activity of two of the major endogenous antioxidant enzymes (SOD and GPx). The increased activity of these enzymes in Tg/Tg cells compared to the control F1/F1 cells was obvious, suggesting that the presence of Hsp70 confers enhanced tolerance against DOX-induced oxidative stress. Overall, it has been indicated that Hsp70 protein exerts a very important protective action and renders cells more resistant to the harmful effects of doxorubicin.

Summary: Hypertrophic cardiomyopathy (HCM) is a genetically inherited cardiomyopathy with common mutations in the genes MYBPC3 and MYH7. Diagnosis requires ruling out secondary causes of left ventricular hypertrophy. HCM is characterized by various clinical manifestations, with diastolic dysfunction and increased ventricular arrhythmogenesis being commonly observed. Current therapies focus on symptom management and prevention of sudden cardiac death, but there is a need for improved treatments targeting heart failure and atrial fibrillation. With advancements in understanding the genetic and molecular basis of HCM, novel treatments are expected in the near future.

A small circulating miRNAs signature predicts mortality and adverse cardiovascular outcomes in chronic hemodialysis patients

Summary: Chronic HD patients with severe cardiac alterations have a higher risk of death and adverse CV outcomes. Imbalanced expression of certain miRNAs is related to pathological cardiac remodeling and worse CV outcomes. In this study, circulating levels of miRNA 30a-5p, 23a-3p, and 451a were found to be lower in HD patients and correlated with the severity of cardiac dysfunction. These miRNAs may serve as predictors of worse clinical outcomes.

Coronary microcirculation and left ventricular diastolic function but not myocardial deformation indices are impaired early in patients with chronic kidney disease

Summary: This study aimed to compare myocardial strain, echocardiographic indices, and coronary flow reserve (CFR) in younger and older CKD patients. The findings showed that younger CKD patients had impaired coronary microcirculation and left ventricular diastolic function, while myocardial strain abnormalities were not observed. These issues worsened with aging in CKD patients. The study suggests that assessing coronary microcirculation and ventricular function may be important in younger CKD patients.

Differences in the Profile of Circulating Immune Cell Subsets in Males with Type 2 Cardiorenal Syndrome versus CKD Patients without Established Cardiovascular Disease

Summary: This study investigated differences in circulating immune cells between type 2 cardiorenal syndrome (CRS-2) patients and CKD patients without cardiovascular disease (CVD). The results showed that CRS-2 patients had higher levels of proinflammatory cells and T regulatory cells, as well as lower levels of lymphocytes and natural killer cells compared to CKD patients. Decreased lymphocytes, T-lymphocytes, and increased proinflammatory cells were associated with mortality. CD4+ T-lymphocytes independently predicted fatal cardiovascular events in the CRS-2 cohort.

Summary: Myocardial perfusion imaging via single-photon emission computed tomography (SPECT MPI) is a well-established method for diagnosing coronary artery disease (CAD). However, the role of SPECT MPI in predicting major cardiovascular events remains uncertain. In this study, the presence of extensive perfusion defects in SPECT MPI was found to be independently associated with mortality at 1 year in high-risk patients with suspected stable CAD.

An All-in-One Tool for 2D Atherosclerotic Disease Assessment and 3D Coronary Artery Reconstruction

Summary: Diagnosis of coronary artery disease relies mainly on invasive imaging modalities such as X-ray angiography, intravascular ultrasound (IVUS), and optical coherence tomography (OCT), with computed tomography coronary angiography (CTCA) as a non-invasive alternative. This study presents a novel tool for reconstructing 3D coronary arteries and characterizing plaques using these imaging modalities. Image processing and deep learning algorithms were used to determine lumen and adventitia borders, plaque properties, and strut detection. The tool combines X-ray angiography with IVUS/OCT analysis for hybrid 3D reconstruction, and uses a 3D level set approach for CTCA image processing. The tool was evaluated for efficiency and usability, achieving high agreement with manual annotations and a high System Usability Scale (SUS) score of 0.89, indicating excellent usability.

Circulating miRNA 122-5p Expression Predicts Mortality and Cardiovascular Events in Chronic Hemodialysis Patients: A Multicentric, Pilot, Prospective Study

Summary: Patients undergoing chronic hemodialysis (HD) are at high risk of cardiovascular (CV) events, but risk prediction in this population is challenging. Recent research has shown that circulating miRNA 122-5p is related to the onset and progression of CV disease. This study found that HD patients had lower levels of circulating miRNA 122-5p compared to healthy controls. ALT, E/e', and CRP were identified as independent predictors of miRNA 122-5p levels. Additionally, higher baseline levels of circulating miRNA 122-5p predicted a significantly increased risk of composite endpoint occurrence in HD patients.

Coronary computed tomography angiography-based endothelial wall shear stress in normal coronary arteries

Summary: This study evaluated CCTA-based ESS values in coronary arteries without atherosclerosis and identified factors affecting ESS values. The findings showed that ESS was highest in the left anterior descending artery and lower in men compared to women, even after adjusting for lumen diameter. ESS values were also highest in small segments and had a correlation with the distance from the ostium.
