Validation of the albumin-indocyanine green evaluation model in patients with resected hepatocellular carcinoma and comparison with the albumin-bilirubin score

Background The albumin-indocyanine green evaluation (ALICE) model based on serum albumin and indocyanine retention rate has been shown to be an effective method for predicting postoperative outcomes in hepatocellular carcinoma patients. Aim of the study was to validate the ALICE model in a large Western cohort of patients by comparing the albumin-bilirubin (ALBI) score and Child-Turcotte-Pugh (CTP) score. Methods A total of 400 patients who underwent hepatic resection from January 2005 to June 2016 at three centers were enrolled. The ALICE, ALBI, and CTP scores were computed for all patients. Results The ALICE score correlated better with ALBI (r = 0.428) than with CTP score (r = 0.302). Both the ALICE (grade 1: 49%; grade 2: 51%) and the ALBI (grade 1: 52.5%; grade 2: 47.5%) scores stratified the CTP class A patients into two distinct classes. The incidence of ascites (grades 1-3: ALICE 11%, 20%, 58%; ALBI 11%, 23%, 50%) and severe liver failure (ALICE 8.7%, 10.5%, 41.7%; ALBI 8.6%, 12%, 50%) increased with increasing ALBI and ALICE grade and were similar for the same grade. Conclusions The ALICE model can assess hepatic functional reserve and predict postoperative outcomes with efficacy comparable with the ALBI grade and better than the CTP score.

Summary: In this study, the perioperative and long-term oncological outcomes of salvage hepatectomy for recurrent hepatocellular carcinoma after primary hepatectomy or locoregional treatments were evaluated in a large Western cohort. The results showed that salvage hepatectomy is as safe, feasible, and effective as primary hepatectomy in terms of long-term outcomes.

Survival benefit of adequate lymphadenectomy in patients undergoing liver resection for clinically node-negative intrahepatic cholangiocarcinoma

Summary: This study aimed to determine whether lymphadenectomy improves long-term outcomes in patients with clinically node-negative intrahepatic cholangiocarcinoma (iCCA). The results showed that adequate lymphadenectomy significantly improved survival outcomes for patients with clinically node-negative iCCA who were found to be node-positive at pathology. Therefore, routine use of adequate lymphadenectomy is recommended for patients with clinically node-negative iCCA.

Summary: This study aimed to investigate the relationship between tumor size and the difficulty of laparoscopic left lateral sectionectomy (L-LLS). Three optimal tumor size cutoffs (40-, 70-, and 100-mm) were identified. As tumor size increased, there was a stepwise increase in the rates of open conversion, operative time, blood loss, intraoperative blood transfusion, Pringle maneuver use, major morbidity, and 30-day readmission.

Bile duct injury following cholecystectomy: delayed referral to a tertiary care center is strongly associated with malpractice litigation

Summary: The aim of this study was to evaluate the risk factors for malpractice litigation in patients with bile duct injury (BDI) referred to a tertiary care center. The results showed that patients in the late-referral group were more likely to have undergone surgical and endoscopic repair before referral. Prompt referral to experienced tertiary care centers without any attempt at repair may reduce the risk of litigation.

Summary: This study aimed to create a machine learning predictive model to allocate patients with recurrent hepatocellular carcinoma (HCC) to their best potential treatment based on their specific characteristics. An individualized algorithm for treatment allocation was developed using factors such as age, cirrhosis, number and size of recurrent nodules, extrahepatic spread, and time to recurrence. The algorithm provides recommendations for the best potential treatment according to patients' profiles.

Plasma Bile Acid Profiling and Modulation of Secreted Mucin 5AC in Cholangiocarcinoma

Summary: Through a case-control study, it was found that the concentration of circulating bile acids (BAs) is significantly higher in patients with cholangiocarcinoma compared to those with benign biliary disease. The study also suggests that BAs may play a role in modulating the expression of the oncogenic mucin 5AC (MUC5AC) gene. In vitro experiments showed that primary conjugated BAs or TCDCA alone can result in the expression of a novel 230 kDa isoform of MUC5AC.

Summary: The impact of timing of repair on outcomes of patients with bile duct injury repaired with Hepp-Couinaud hepatico-jejunostomy (HC-HJ) remains controversial. This observational study showed that HC-HJ can be successfully performed regardless of the timing of repair, as long as the patients are stable and have dilated bile ducts without bile leak.
