Plasma rich in growth factors (PRGF) in intraoral bone grafting procedures: A systematic review

Purpose: This systematic review aimed at assessing the effects of PRGF in new bone formation, soft tissue healing and post-operative pain and swelling in sites that underwent ridge preservation, ridge augmentation and maxillary sinus augmentation procedures. Materials and methods: A comprehensive literature search employing seven databases was conducted by two independent reviewers. Only randomized and non-randomized controlled clinical trials using PRGF alone or in combination with bone grafting materials were selected. Results: Overall, 919 studies were identified, of which a total of 8 articles were included in the qualitative analysis. Two of the selected studies reported on ridge preservation, one on ridge augmentation and five on maxillary sinus augmentation. Positive results were recorded for soft tissue healing and post-operative pain and swelling following these procedures. However, outcomes of PRGF on new bone formation post extraction and on maxillary sinus augmentation when combined with other biomaterials were conflicting. Meta-analysis could not be conducted for any variables due to the heterogeneity of selected studies. Conclusion: Limited evidence exists on the effects of PRGF in different intraoral bone grafting procedures, with some benefit reported on soft tissue healing and post-operative symptomatology. As this platelet concentrate is commonly used in clinical practice, further research is needed to fully assess its clinical indications and effectiveness. Effect of periodontal phenotype characteristics on post-extraction dimensional changes of the alveolar ridge: A prospective case series

Summary: The aim of this study was to assess the impact of specific periodontal phenotypical characteristics on alveolar ridge remodelling after tooth extraction. The results showed a marked dimensional reduction of the alveolar ridge over time, with greater bone loss and volume reduction in sites exhibiting a thin facial bone phenotype. Overall, alveolar ridge remodelling is a physiological phenomenon that occurs after tooth extraction.

Influence of healing time on the outcomes of alveolar ridge preservation using a collagenated bovine bone xenograft: A randomized clinical trial

Summary: This study evaluated the healing outcomes in non-molar post-extraction sockets filled with deproteinized bovine bone mineral with collagen (DBBM-C) as a function of time. The results showed that the proportion of tissue compartments in the sockets was influenced by the healing time, and the alveolar ridge experienced dimensional reduction over time, especially in the facial aspect. The baseline buccal bone thickness was found to be a strong predictor of bone and soft tissue modeling. The study also revealed a variable degree of alveolar ridge atrophy, which was more pronounced in sites with thin facial bone.

Impact of peri-implant soft tissue characteristics on health and esthetics

Summary: The objective of this study was to review the impact of key peri-implant soft tissue characteristics on health and esthetics. The width of keratinized mucosa, the thickness of the mucosa, and the height of supracrestal tissue are crucial for the peri-implant soft tissue phenotype. Inadequate width of keratinized mucosa has been associated with discomfort during oral hygiene and increased risk of peri-implant diseases. Sufficient thickness of the mucosa is required to prevent esthetic issues related to prosthetic elements and to maintain long-term mucosal stability. Supracrestal tissue height is directly related to marginal bone remodeling and insufficient height may lead to unfavorable prosthetic contours and biofilm accumulation. Peri-implant soft tissue dehiscences are associated with esthetic issues and are often seen in sites with deficiencies in keratinized mucosa width, mucosal thickness, and/or supracrestal tissue height.

The results showed that the 2018 Classification of Gingival Recession Defects and Gingival Phenotype had highest intra- and inter-examiner agreement for keratinized tissue width (KTW) and lowest agreement for gingival thickness (GT).

Comparative histological evaluation of intra- and extraorally de-epithelialized connective tissue graft samples harvested from the posterior palate region

Summary: This study evaluated the effectiveness of two different methods for de-epithelialization of autologous connective tissue graft samples. The results showed that both methods were effective in removing most of the oral epithelium, but none of them could completely predictably remove all oral epithelium.

Errors and complications in clinical periodontal practice due to methodologic bias and bad interpretation of the evidence

Summary: Different types of errors and complications can occur during and after periodontal or implant-related procedures. Methodological biases and misinterpretation of evidence are often overlooked causes of these issues. Proper evaluation of the literature requires solid clinical knowledge and a systematic approach to recognize and avoid common biases and interpretive errors for the effective application of available information. This review focuses on methodological biases, interpretive errors, and the choice of relevant endpoints to accurately assess the real impact and potential consequences of therapeutic approaches.

Summary: To improve evidence-based implant dentistry and quality of care, this study aimed to develop a core outcome set and measurements for implant dentistry clinical trials. Through systematic reviews, patient focus groups, and expert discussions, 13 mandatory outcome domains were identified, including pathophysiology, implant/prosthesis lifespan, life impact, and access to care. These domains captured both the benefits and harms of therapy and will contribute to improving evidence-informed implant dentistry and quality of care.

The results identified 22 core outcomes in four domains, including pathophysiology, implant/prosthesis lifespan, life impact, and access to care. This study is of significant importance in improving implant dentistry and quality of care.

Non-invasive assessment of peri-implant mucosal thickness: A cross-sectional study

Summary: This study aimed to evaluate the reliability and reproducibility of different non-invasive methods for the assessment of peri-implant mucosal thickness. The results showed that the assessment methods using digital file superimposition, DICOM files alone, and non-ionizing ultrasound were all comparably reliable for quantifying peri-implant mucosal thickness.

Relationship between supracrestal soft tissue dimensions and other periodontal phenotypic features: A cross-sectional study

Summary: This study examined the association between periodontal supracrestal soft tissue dimensions and other phenotypic features in non-molar maxillary teeth. The findings showed that males had thicker gingival tissue and alveolar bone, as well as taller periodontal supracrestal tissue and wider keratinized tissue. Tooth type, gingival architecture, and interproximal papilla height were also associated with periodontal phenotype. These phenotypic features varied across subjects, different apico-coronal levels, and as a function of gender and tooth type.

Treatment alternatives for the rehabilitation of the posterior edentulous maxilla

Long-term stability of gingival margin and periodontal soft-tissue phenotype achieved after mucogingival therapy: A systematic review

Summary: This systematic review aimed to evaluate the long-term stability of gingival margin position, KTW, and GT in sites that underwent RC or GA, and found different treatment approaches had varying effects on these parameters. The study highlighted that greater short-term KTW was associated with more stable gingival margin, the use of autogenous soft-tissue grafts was linked to lower RD increase over time, and treatment approaches enhancing the three-dimensional gingival phenotype were associated with greater gingival margin stability.
