Attention-deficit hyperactivity disorder and suicidality: The mediating effects of psychiatric comorbidities and family function

Objective: To explore the association between Attention-Deficit Hyperactivity Disorder (ADHD) and suicidality in children. Mediating effects of family function and psychiatric co-morbidities were also explored. Methods: This is a national representative cross-sectional survey of school-based sample of 4739 children aged 7-15. Logistic regression was used to assess the excess risks of suicidality (i.e., suicidal ideation, suicide plan, and suicide attempts) in 412 children with ADHD. Serial multiple mediator models were conducted to assess the mediating effects of family function and psychiatric comorbidities. Results: The prevalence of suicidality in children with ADHD was approximately 20% in Taiwan. After adjusting for potential confounders, the risk of suicidality among children with ADHD was approximately four times higher than among non-ADHD children [(adjusted Odds Ratio [OR]=3.82, 95% Confidence Interval (CI) (2.73, 5.34)] for suicidal ideation, adjusted OR=4.18, 95% CI (2.57, 6.80) for suicide plan and adjusted OR=4.45, 95% CI (1.99, 9.93) for suicide attempts. The mediating effects of anxiety/depression and conduct problems were about 20% and 8%, respectively, across all suicide outcomes. The mediating effects of family function were around 6-7% for suicidal ideation and 16-18% for suicide plan and suicide attempts. The effects of ADHD on suicidality, in general, remained after considering the mediating roles of family function and psychiatric comorbid conditions. Conclusions: Children with ADHD are at a high risk of suicide. Although the family function and psychiatric co-morbidities partially mediate this association, ADHD in itself is a potent suicide risk factor and should be an important target for suicide prevention.

Developmental Changes of Autistic Symptoms, ADHD Symptoms, and Attentional Performance in Children and Adolescents with Autism Spectrum Disorder

Summary: This study followed up on ADHD/autistic symptoms and attentional performance in children/adolescents with ASD and TD over a period of 5-7 years. The results showed that ADHD symptoms, especially hyperactivity, and attentional functions significantly improved during follow-up, with greater improvement observed in children compared to adolescents, in both ASD and TD. More omission errors and perseverations were noted in ASD compared to TD throughout the follow-up. Children with ASD showed more improvement in reaction time, while adolescents with ASD showed less improvement in commission errors and detectability compared to TD. The lack of correlation between attentional functions and ADHD symptoms in ASD suggests different neural mechanisms of ADHD symptoms between ASD and ADHD.

Population-attributable risk of psychiatric disorders for suicide among adolescents and young adults in Taiwan

Atypical development in white matter microstructures in ADHD: A longitudinal diffusion imaging study

Summary: In cross-sectional studies, white matter microstructure alterations are observed in children with ADHD, but not as prominent in adults with ADHD compared to typically-developing controls. Limited longitudinal imaging studies have made the developmental trajectories of white matter microstructures in ADHD unclear. This study used diffusion spectrum imaging to examine the longitudinal changes in white matter tracts in ADHD and found that participants with ADHD showed more rapid development in certain tracts. Furthermore, more rapid development in specific tracts was associated with slower decreases in inattention symptoms and greater improvement in attention performance.

A lack of efficacy of continuous theta burst stimulation over the left dorsolateral prefrontal cortex in autism: A double blind randomized sham-controlled trial

Summary: Although previous open-label trials suggest the potential of inhibitory rTMS over the DLPFC in ASD, our double-blind sham-controlled trial did not find superior efficacy of cTBS over the left DLPFC in children, adolescents, and adults with ASD. Time effects of cTBS on symptoms and executive function were significant in both active and sham groups. These findings suggest the need for more rigorous rTMS/TBS studies in ASD.

Sex differences in medication-naive adults with attention-deficit/ hyperactivity disorder: A counting Stroop functional MRI study

Summary: Emerging evidence suggests deficits in executive functions in the fronto-striato-parietal network in individuals with ADHD. This study aimed to investigate whether these deficits are also present in women with ADHD by using functional magnetic resonance imaging. The results showed that compared to healthy controls, adults with ADHD exhibited reduced activation in the caudate nucleus and inferior frontal gyrus. Furthermore, women with ADHD showed greater difficulties in resolving interference compared to men with ADHD, as indicated by the increased differences in brain activation.

Differential neural processing of value during decision-making in adults with attention-deficit/hyperactivity disorder and healthy controls

Summary: This study examined altered neural processing of stimuli values associated with risk-taking decision behavior in adults with ADHD. The results showed that adults with ADHD had slower response times and tended to accept more stakes with a middle-to-low probability of winning compared to healthy controls. Adults with ADHD also exhibited lower activity in the dorsolateral prefrontal cortex and reduced sensitivity in the ventromedial prefrontal cortex in response to linear changes in probability.

Prescribing patterns for attention deficit hyperactivity disorder among children and adolescents in Taiwan from 2004 to 2017

Summary: This study examined the use of methylphenidate and atomoxetine in patients diagnosed with attention deficit hyperactivity disorder (ADHD) in Taiwan between 2004 and 2017. The study investigated the initial treatment for ADHD, the time between diagnosis and prescription, and medication-switching patterns. The majority of patients (98.2%) received methylphenidate, while atomoxetine use increased over time. The study also found that the prevalence of pharmacotherapy for ADHD was relatively low.

The relationship between psychological pain and suicidality in patients with major depressive disorder: A meta-analysis

Summary: This study systematically reviewed the association between psychological pain and suicidality in patients with major depressive disorder (MDD). The results showed that psychological pain was a risk factor for suicidality in MDD patients, especially for those of advancing age. Reducing psychological pain in MDD patients is important for preventing suicidality.

The role of family accommodation in child posttraumatic stress symptoms and functional impairment in the context of cognitive behavioral therapy

Summary: This study examined the relationship among family accommodation (FA), posttraumatic stress symptoms (PTSS), and functional impairment. The results showed that FA was significantly associated with PTSS and functional impairment. Baseline FA partially mediated the relationship between baseline PTSS and functional impairment. Changes in FA from pre- to post-treatment were associated with relevant outcome variables at post-treatment and 12-month follow-up.
