Infertility is known to decrease quality of life among adults. In some cases, infertility is caused by medical conditions and/or treatments prescribed in childhood, and using methods to protect or preserve fertility may expand future reproductive possibilities. Structured programs to offer counseling about infertility risk and fertility preservation options are essential in the care of pediatric patients facing fertility-threatening conditions or treatments, yet multiple barriers to program development exist. This report was developed from the institutional experiences of members of the Pediatric Initiative Network of the Oncofertility Consortium, with the intent of providing guidance for health care providers aiming to establish programs at institutions lacking pediatric fertility preservation services. The mechanics of building a fertility preservation program are discussed, including essential team members, target populations, fertility preservation options (both established and experimental), survivorship issues, research opportunities, and ethical considerations. Common barriers to program development and utilization, including low referral rates and financial concerns, are also discussed, and recommendations made for overcoming such barriers. Outcomes of Fertility Preservation Consults for Women at Risk for Primary Ovarian Insufficiency Due to History of Cancer Treatment or Mosaic Turner Syndrome

Summary: This study reported consultation and treatment outcomes for 21 cancer survivors and 3 mosaic Turner syndrome patients. It found that ovarian stimulation for mosaic TS AYA and survivors is feasible even with diminished ovarian reserve. Further research is needed to establish guidelines for patient selection, treatment timing, and stimulation protocols.

Summary: Since 2007, the Oncofertility Consortium has held an annual conference to disseminate emerging research findings in fertility preservation and facilitate scientific communication. In 2020, the pandemic forced a shift to a virtual meeting, which attracted over 700 participants and showcased global advances in oncofertility and fertility preservation.

Summary: Purpose: To compare the oncofertility practices for childhood cancer in limited resource settings versus optimum resource settings based on data collected in the Repro-Can-OPEN Study Part I & II. Methods: Surveyed 39 oncofertility centers from different regions. Results: Showed higher oncofertility scores in optimum resource settings, frequent utilization of certain techniques, and rare utilization of advanced options. Conclusions: Presented a best practice model to optimize care for children with cancer in different resource settings.

Summary: After pausing testosterone in a transgender mouse model, estrous cyclicity resumes but with ovarian stromal aberrations and upregulated inflammation. The impact of taking and pausing testosterone on the ovaries is not fully understood. This study utilized a mouse model to characterize the ovarian dynamics following testosterone cessation and found stromal changes, immune pathway upregulation, and the presence of multinucleated macrophages in the ovaries.

Summary: The conflation of sex assigned at birth and gender has made it difficult to identify transgender and nonbinary people in large datasets. This study developed a method using diagnostic and procedural codes to determine the sex assigned at birth of these patients, with the aim of expanding available datasets for exploring sex-specific conditions in this population.
