Summary: Although fertility preservation is not fully integrated into comprehensive cancer care for pediatric and adolescent patients, recent advances and trends have been summarized in this review to guide healthcare professionals. Standardized infertility risk assessment guidelines provide consistency in clinical care and serve as a baseline for future research. Dissemination of information is critical for fertility preservation to become an integral part of comprehensive cancer care.

Summary: This study demonstrates that oocyte cryopreservation is a feasible and efficient option for fertility preservation in cancer patients, based on the comparison with nononcological patients undergoing in vitro fertilization treatments. The results show that cancer patients who undergo oocyte cryopreservation have a high pregnancy rate and birth rate, indicating the effectiveness of this method in preserving fertility.

Summary: Patients with childhood, adolescent, and young adult cancer treated with gonadotoxic therapies are at higher risk for infertility and may desire fertility preservation. A clinical practice guideline offers recommendations for ongoing communication to facilitate effective dialogue between patients and their families.

Summary: The study found that among pediatric and adolescent patients at an academic center undergoing fertility preservation consultation, there were no socioeconomic or clinical barriers to FPT use in those who had not yet undergone gonadotoxic therapy. The only factor negatively associated with odds of pursuing FPT was prior chemotherapy exposure.

Summary: The feasibility of freezing and thawing ovarian tissue is well-documented, but ovarian tissue transplantation is progressing at a slower pace with limited clinical experience. Results from five European centers show a low risk of relapse with ovarian tissue reimplantation and successful recovery of endocrine function in most patients, although in vitro fertilization efficacy needs improvement. Chemotherapy before ovarian tissue cryopreservation does not significantly affect success rates. Further research is needed to optimize the approach for ovarian tissue transplantation.

Summary: Testicular tissue cryopreservation is a feasible and safe option for fertility preservation in prepubertal and adolescent boys, with a high acceptance rate among families and patients. Although still considered experimental, this procedure has shown promising results and offers hope for future reproductive options.

Summary: This study aims to develop clinical practice guidelines for fertility preservation in female patients diagnosed with childhood, adolescent, and young adult cancer at age 25 years or younger. By utilizing the Grading of Recommendations Assessment, Development and Evaluation methodology, transparent recommendations were formed to facilitate the care of high-risk female patients. Collaboration led to a comprehensive review of existing evidence, transparent reporting, and achievement of global interdisciplinary consensus.

Summary: Melatonin pretreatment improves the quality of rat ovarian grafts by promoting the restart of the estrous cycle, increasing the expression of various key proteins in the ovaries, and reducing apoptosis levels. Further studies on reproductive function enhancement are warranted based on these findings.

Summary: Gonadal tissue cryopreservation (GTC) is offered at around 50% of the institutions surveyed in the US for patients with differences of sex development (DSD), but only a minority are preserving tissue for research purposes. GTC is offered for various DSD conditions. Future research will focus on examining the presence and quality of germ cells to support clinical decision making related to fertility preservation for patients with DSD.

Summary: The gender diverse and transgender community is increasingly encountered in clinical settings due to improved awareness and access to medical facilities. Gender-affirming therapy can have an impact on fertility potential, and transgender patients should be counseled on fertility preservation options prior to transitioning. However, there is limited data and clinical information on fertility preservation for transgender individuals. Understanding this clinical agenda and the processes involved is important for providing comprehensive and specific care for transgender individuals who need fertility counseling or treatment options.

Summary: After pausing testosterone in a transgender mouse model, estrous cyclicity resumes but with ovarian stromal aberrations and upregulated inflammation. The impact of taking and pausing testosterone on the ovaries is not fully understood.
