A qualitative study of quality of life domains and subdomains relevant to patients with spondyloarthritis

Aims To identify quality of life (QoL) domains and subdomains relevant to patients with different spondyloarthritis (SpA) subtypes in Singapore, and to assess how identified QoL domains and subdomains map onto currently used patient-reported outcome measures (PROMs). Methods Nine focus group discussions (FGDs), including two previously conducted FGDs from the Updating the Psoriatic Arthritis Core Domain Set Study, were conducted with patients with SpA in Singapore. The FGDs were organized by SpA subtype and language spoken. All FGDs were audio-taped, transcribed verbatim. After thematic analysis through open and axial coding, the domains were organized using the World Health Organization Quality of Life (WHOQOL) framework. Identified QoL domains and subdomains were mapped to currently used PROMs. Results The nine FGDs included 51 patients. In total, 27 domains and 92 subdomains were identified and then organized within the 7 broad categories of the WHOQOL framework. Patients in Singapore were more concerned about financial resources, work satisfaction and positive feelings while less concerned about freedom, physical safety and security than patients in Western countries. Home environment and work satisfaction emerged as unique QoL domains relevant to patients with axial SpA (axSpA). PROMs for psoriatic arthritis (PsA) can capture both identified domains of PsA and axSpA. Conclusions A wide range of QoL domains and subdomains are relevant to SpA patients in Singapore, and there appears to be minimal differences in their relative importance between SpA subtypes. This study supports the development and validation of common QoL-specific PROMs for usage in SpA.

Prevalence and correlates of use of digital technology for managing hypertension among older adults

Summary: This study assessed the prevalence and correlates of using digital technology for managing hypertension among older adults in Singapore. The study found that only 7.1% of older adults with hypertension used digital technology for managing hypertension, and there was no association between the use of digital technology and hypertension control. Although users of digital technology had slightly lower blood pressure values, the differences were not statistically significant.

Evaluation of Machine Learning Methods Developed for Prediction of Diabetes Complications: A Systematic Review

Summary: This study evaluated the quality and performance of machine learning models in predicting microvascular and macrovascular diabetes complications in a Type 2 diabetes population. The results showed that neural networks were the most frequently used models, and age, duration of diabetes, and body mass index were common predictors. Most models demonstrated good discrimination performance, with random forest showing the best overall performance. However, further validation studies are needed before the clinical implementation of these models.

The study selected core outcome instruments through a systematic literature review and assessment of the instruments' psychometric properties. Additionally, the study endorsed additional instruments to enhance the evaluation of axSpA trials.

Validity and Reliability of the European Foot and Ankle Society (EFAS) Score in Patients With Hallux Valgus in Singapore

Efficacy and safety of colchicine for the treatment of osteoarthritis: a systematic review and meta-analysis of intervention trials

Summary: This systematic review and meta-analysis evaluated the efficacy and safety of colchicine for the treatment of osteoarthritis (OA). The study found that colchicine did not show significant benefits in reducing pain and improving physical function compared to placebo or other active comparators. The results suggest that future trials should focus on subgroups of OA patients with inflammation and mineralization who might benefit from colchicine.

Summary: The objective of this study was to provide evidence for the efficacy and safety of therapeutic options for the peripheral arthritis domain of PsA for the revised 2021 GRAPPA treatment recommendations. Evidence was derived from new RCTs and recommendations were made for different patient groups based on their treatment history and response. Different classes of therapeutic options were strongly recommended for different patient groups.

Summary: A national retrospective cohort study found a moderately high rate of flares of autoimmune inflammatory rheumatic diseases (AIIRD) after COVID-19 mRNA vaccination, but also improvement in several patients. Severe flares and hospitalization were rare. Thus, vaccination remains safe and highly recommended.

Summary: This study aimed to explore patient-defined flares in psoriatic arthritis (PsA) and compare them to an increase in disease activity in psoriatic arthritis (DAPSA). The validity of a patient-reported flare question was also analyzed. The results showed that patient-reported flares were associated with disease activity and symptoms, and there was moderate agreement between patient and physician definitions of worsening.

Caregivers' Absenteeism and Its Association With Health Shocks and Functional Impairment Among Persons With Severe Dementia

Summary: The study found that adult child caregivers of persons with severe dementia often experience absenteeism due to caregiving responsibilities. The caregivers' absenteeism was quantified and found to be associated with the functional impairment and health shocks of the patients. It was also observed that certain characteristics of caregivers influenced their likelihood of experiencing absenteeism.

Social cognitive theory to improve symptom appraisal and help-seeking among patients with autoimmune rheumatic diseases: A qualitative study

Summary: This study explores the applicability of social cognitive theory (SCT) in improving symptom appraisal and help-seeking among patients with autoimmune rheumatic diseases (ARDs). Semi-structured interviews were conducted with 33 ARD patients and the transcripts were coded using SCT as the framework. The study found that behavioral capacity and self-efficacy were lacking in symptom interpretation, leading to delayed help-seeking. Possible approaches to address this include education to improve behavioral capacity and self-efficacy and enhance expectations, observational learning, reinforcements, and reciprocal determinism.

Community Efficacy for Non-Communicable Disease Management and Medication Adherence: The Sequential Mediating Role of Self-Efficacy and Depressive Symptoms

Summary: This study examined the sequential mediating effects of self-efficacy and depressive symptoms on the relationship between community efficacy and medication adherence. The results showed that enhancing individual self-efficacy may decrease depressive symptoms and improve medication adherence. Furthermore, the effects varied by sex, with self-efficacy being more significant for females and depressive symptoms being more significant for males.

Effect of intralymphatic allergen-specific immunotherapy on house dust mite in a murine model of allergic rhinitis

Summary: This study aimed to determine the efficacy of intralymphatic immunotherapy (ILIT) in a house dust mite (HDM) mouse model of allergic rhinitis. The results showed that ILIT decreased serum IgE level and eosinophil infiltration, and regulated the expression of various cytokines and T reg cells, indicating its potential as a treatment for allergic rhinitis.
