Once unimaginable, fertility management is now a nationally established part of cancer care in institutions, from academic centers to community hospitals to private practices. Over the last two decades, advances in medicine and reproductive science have made it possible for men, women and children to be connected with an oncofertility specialist or offered fertility preservation soon after a cancer diagnosis. The Oncofertility Consortium's National Physicians Cooperative is a large-scale effort to engage physicians across disciplines - oncology, urology, obstetrics and gynecology, reproductive endocrinology, and behavioral health - in clinical and research activities to enable significant progress in providing fertility preservation options to children and adults. Here, we review the structure and function of the National Physicians Cooperative and identify next steps.

Impact of a single-session psychosocial counseling intervention for women with vulvodynia

Summary: This study evaluated the impact of a single session of psychosocial counseling on patients with vulvodynia. Results showed that patients who received the counseling intervention reported improved understanding and lessened emotional impact of their symptoms, compared to the control group.

Summary: Since 2007, the Oncofertility Consortium has held an annual conference to disseminate emerging research findings in fertility preservation and facilitate scientific communication. In 2020, the pandemic forced a shift to a virtual meeting, which attracted over 700 participants and showcased global advances in oncofertility and fertility preservation.

Pharmacokinetic comparison of three delivery systems for subcutaneous testosterone administration in female mice

Summary: In this study, three different modes of testosterone (T) administration (subcutaneous injections, commercially available pellets, and silastic implants) were compared in terms of their release dynamics and pharmacokinetic profiles in mice. The results showed that all three methods were able to increase T levels in plasma, but had different recovery speeds after cessation of treatment. Silastic implants provided sustained elevation of T levels for at least 21 weeks.

Initiating gender-affirming hormones for transgender and non-binary people: A qualitative study of providers' perspectives on requiring mental health evaluations

Summary: This qualitative study explored the range of attitudes and approaches to mental health evaluation among providers of gender-affirming hormones (GAH) in the United States. The findings revealed variations in providers' perspectives, with some advocating for universal mental health evaluation and others seeing it as a barrier to care. Providers' views were primarily influenced by their understanding of transgender identity and transition, as well as their interpretation of the risk of significant mental illness associated with being transgender.

Summary: This study successfully developed a mouse model to investigate the reproductive consequences of testosterone therapy in transgender men. The model provides a tool for researchers to study the effects of gender-affirming hormone therapy in peripubertal transmasculine youth.

Management of testosterone around ovarian stimulation in transmasculine patients: challenging common practices to meet patient needs-2 case reports

Summary: Approximately 50% of transmasculine people use testosterone for gender affirmation, but little is known about its effects on reproductive capacity. The current practice of stopping testosterone prior to ovarian stimulation may have negative effects on mental health. Two case studies are presented to support the continuation of testosterone during stimulation.

Summary: Purpose: To compare the oncofertility practices for childhood cancer in limited resource settings versus optimum resource settings based on data collected in the Repro-Can-OPEN Study Part I & II. Methods: Surveyed 39 oncofertility centers from different regions. Results: Showed higher oncofertility scores in optimum resource settings, frequent utilization of certain techniques, and rare utilization of advanced options. Conclusions: Presented a best practice model to optimize care for children with cancer in different resource settings.

Summary: After pausing testosterone in a transgender mouse model, estrous cyclicity resumes but with ovarian stromal aberrations and upregulated inflammation. The impact of taking and pausing testosterone on the ovaries is not fully understood. This study utilized a mouse model to characterize the ovarian dynamics following testosterone cessation and found stromal changes, immune pathway upregulation, and the presence of multinucleated macrophages in the ovaries.

Summary: The conflation of sex assigned at birth and gender has made it difficult to identify transgender and nonbinary people in large datasets. This study developed a method using diagnostic and procedural codes to determine the sex assigned at birth of these patients, with the aim of expanding available datasets for exploring sex-specific conditions in this population.
