Epidemiology of intrahepatic, perihilar, and distal cholangiocarcinoma in the French population

Objectives The objective of this study was to investigate the differences between cholangiocarcinoma (CC) subtypes in terms of incidence rate, clinical presentation, management and survival by applying a stable anatomical classification in a population-based study. Patients and methods Cancer data for the period 2000-2012 were obtained from a specialized digestive cancer registry in the Department of Calvados, France. Patients' files were checked, and the diagnosis was verified by two hepatobiliary surgeons. This approach prevents classifying perihilar cholangiocarcinoma (PHCC) as intrahepatic cholangiocarcinoma (ICC), thereby allowing an accurate estimation of their respective epidemiological characteristics. Results A total of 320 patients with CC were included. ICC represented 41% (130), whereas PHCC and distal cholangiocarcinoma represented 36 (116) and 23% (74), respectively. The mean age at the time of diagnosis differed significantly between the three subtypes (P<0.05). ICC was discovered accidently more frequently than PHCC, which was associated significantly with clinical symptoms. No change in the incidence or survival rates of CC subtypes were noticed, except for PHCC, in which female individuals had a significantly shorter median and 5-year survival rate of 0% (P<0.05). Conclusion The frequency of PHCC is overestimated in the literature. The anatomical reclassification of CC subtypes shows the stability of their incidence and survival rates. Considering ICC and PHCC as two different entities implies the need to assign a specific topographic code for PHCC.

Summary: This study aimed to evaluate the association between the radiological score and the failure of conservative management in adhesive small bowel obstructions (aSBO). The results showed that a history of previous surgery was not a significant risk factor for conservative treatment failure, while a previous episode of aSBO was protective against treatment failure. Additionally, an Angers CT score of >= 5 was identified as an individual risk factor.

Permanent stoma after sphincter preservation for rectal cancer. A situation that occurs more often than you might think

Summary: This study aimed to evaluate the incidence and risk factors for permanent stomas after sphincter-preserving surgery for rectal cancer, as well as compare long-term survival outcomes. Out of 257 patients, 16.7% developed permanent stomas after a median follow-up of 4.8 years, with severe post-operative complications and age being significant risk factors. Patients with permanent stomas had significantly lower overall survival and disease-free survival compared to those without stomas.

Summary: This study aimed to report the incidence of iatrogenic ureteral injury (IUI) during colorectal surgery and analyze the long-term consequences of urological late complications and their impact on oncological outcomes. The results showed that IUI has few consequences if recognized early, but long-term urological sequelae can occur in a third of patients and may affect the timing of adjuvant chemotherapy in colorectal surgery.

New Histoprognostic Factors to Consider for the Staging of Colon Cancers: Tumor Deposits, Invasive Tumor Infiltration and High-Grade Budding

Summary: The study aimed to analyze the impact of new histoprognostic factors on the survival of colon cancer patients and optimize their therapeutic management. The presence of isolated tumor deposits and an infiltrative type of tumor invasion significantly worsened overall survival and recurrence-free survival. High-grade budding was associated with a poor prognosis, but with no significant difference. In conclusion, these histoprognostic factors should be integrated into pathological reports of colon cancers to guide more aggressive treatments for patients.

Mid-term Results of an ERAS Program of Bariatric Surgery in a Tertiary Referral Center

Summary: In a tertiary referral center, a study was conducted to identify preoperative risk factors for failure to discharge after bariatric surgery on or before postoperative day two. The study found that a history of psychiatric disorder, insulin-dependent diabetes, use of anticoagulant medication, distance to the referral center greater than 100 km, gallbladder lithiasis, and planned additional procedures were independent risk factors for delayed discharge.

Gastrointestinal bleeding of undetermined origin: What diagnostic strategy to propose?

Summary: Gastrointestinal bleeding of undetermined origin (GBUO) refers to gastrointestinal bleeding without an identified cause or location despite endoscopic assessment. The majority of cases of GBUO occur in the small intestine due to vascular, inflammatory, or tumoral causes. Video-capsule endoscopy (VCE) is recommended as the first-intention study in Europe, with CT-enteroscopy performed if intestinal obstruction is suspected. Enteroscopy is used for therapeutic or diagnostic purposes depending on the results of VCE or CT-enteroclysis, and intraoperative enteroscopy (IOE) is reserved for cases where preoperative enteroscopy fails or for recurrent GBUO after other studies and explorations.

Influence of Hemorrhagic Complications of Pancreatoduodenectomy in Patients with Cancer on Short- and Long-Term Mortality

Revisional surgery after restrictive surgery: midterm outcomes of a tertiary center

Summary: Revisional bariatric surgery (RBS) is a challenging procedure and this study analyzed data from 252 patients who underwent RBS after laparoscopic adjustable gastric banding (LAGB) or laparoscopic sleeve gastrectomy (LSG). The study found that RBS after LSG had a higher overall morbidity rate, while RBS after LAGB had more perioperative issues.

Quality of life after sigmoid diverticulitis: A review

Summary: This study reports on the quality of life of patients with sigmoid diverticulitis. The results show that long-term quality of life does not differ significantly between patients who received antibiotic therapy and those who received symptomatic treatment alone. However, for patients who experienced recurrent events, elective surgery appears to improve their quality of life. In conclusion, assessment of quality of life is important in diverticular disease and should guide operative indications.

Conventional Transarterial Chemo embolization Using Streptozocin in Patients with Unresectable Neuroendocrine Liver Metastases

Summary: This study evaluated the tolerability and clinical, biological and radiological tumor response and survival rates in patients with unresectable neuroendocrine liver metastases treated by trans-catheter arterial embolization with conventional chemoembolization, using streptozocin, Lipiodol, and embolization microspheres. The results showed that this procedure using streptozocin is an effective and well-tolerated palliative option for patients with unresectable neuroendocrine liver metastases, with a high objective response rate and non-progressive disease rate. The overall survival rate at different time points was also encouraging.

Effect of the inclusion of biological silage offish residues in the digestibility of diets for sheeps

Summary: The objective of this research was to evaluate the effect of the inclusion of biological silage of fish residues in the digestibility of various nutrients in sheep. The results showed that the inclusion of different levels of biological silage of fish residues did not significantly affect the digestibility of dry matter, organic matter, neutral detergent fiber, and energy, but it did have a significant effect on the digestibility of crude protein.
