Summary: Periprocedural myocardial injury and myocardial infarction are common complications of percutaneous coronary intervention, associated with increased short- and long-term mortality. Inflammation levels before, during, and after the procedure are linked to higher rates of myonecrosis and future cardiovascular events. In addition to inflammation from underlying coronary disease, PCI itself triggers an acute inflammatory response.

Summary: This study evaluated the role of ST-segment resolution (STR) and Thrombolysis in Myocardial Infarction (TIMI) flow in reperfusion evaluation after primary percutaneous coronary intervention (PPCI) for ST-segment-elevation myocardial infarction. The results showed that STR =50% and complete STR were significantly associated with lower 2-year mortality compared to STR <50%. Additionally, the combination of STR and TIMI flow provided incremental prognostic information beyond either measure alone.

Summary: This article reports a case series of seven patients with cardio-cerebral infarction who underwent primary percutaneous coronary intervention. All patients presented with stroke-related symptoms and thrombus aspiration was the main reperfusion strategy. However, only two patients were diagnosed with coronary embolism, and the rest required stenting for severe atherosclerotic lesions. Overall, cardio-cerebral infarction is a rare but fatal condition that requires further investigation for optimal treatment strategies.

Summary: In Western Denmark, the implementation of primary percutaneous coronary intervention (pPCI) for ST-segment elevation myocardial infarction (STEMI) has led to a significant reduction in 1-year mortality, with the greatest decline occurring within the first 30 days after pPCI. This highlights the importance of optimizing early management for improving overall survival in STEMI patients.

Summary: We compared the outcomes of NSTEMI patients who underwent successful stent implantation based on their pre-PCI TIMI grade and SBT time. We found that shortening the SBT time may benefit the survival of NSTEMI patients. The outcomes differed between the pre-PCI TIMI 0/1 group and the TIMI 2/3 group.

Summary: This study aimed to assess the incidence and risk factors of major adverse cardiovascular events (MACEs) in patients with systemic sclerosis (SSc). A nationwide cohort study was conducted in Taiwan, including 1379 SSc patients and 2758 non-SSc individuals. The study found that SSc was associated with increased risk of myocardial infarction and peripheral arterial occlusion disease, but not of ischaemic stroke. Age, gender, residential area, valvular heart disease, rheumatoid arthritis, and the use of clopidogrel and aspirin were identified as independent risk factors for MACEs in SSc patients. The findings of this study are clinically significant in understanding the risk factors for MACEs in SSc patients.

Summary: The prognostic importance of cardiac procedural myocardial injury and myocardial infarction (MI) in chronic coronary syndrome (CCS) patients undergoing elective percutaneous coronary intervention (PCI) is still debated. Post-PCI cTn elevation of >= 5 x 99th percentile URL used to define Type 4a MI is associated with increased 1-year mortality in CCS patients with normal baseline cTn levels.

Summary: This study found that inflammatory biomarkers, including neutrophil to lymphocyte ratio, high-sensitive CRP, IL-6, TNF-a, and RDW, were elevated in patients with coronary artery ectasia compared to healthy controls. The findings suggest that inflammatory dysregulation is involved in the pathogenesis of coronary artery ectasia and should be further investigated.

Summary: Heart failure with reduced ejection fraction (HFrEF) has a negative impact on prognosis, quality of life, and healthcare expenditure. Despite advances in management, survival and hospitalization rates are still concerning. Recently, sodium-glucose co-transporter 2 inhibitors, vericiguat, omecamtiv mecarbil, and sacubitril/valsartan have shown promising results in reducing cardiovascular mortality, heart failure hospitalizations, and improving outcomes in HFrEF patients. Anti-inflammatory agents have also been studied, but current therapies are ineffective. This mini-review summarizes the recent studies in HFrEF therapeutics, covering the latest advancements.

Summary: Pulmonary vein isolation (PVI) is essential for atrial fibrillation (AF) ablation, but the role of the superior vena cava (SVC) in AF is increasingly recognized. Studies have explored the efficacy and safety of SVC isolation (SVCI) in AF patients, but results are inconclusive. Empiric and as-needed SVCI have been compared, but no studies have compared different ablation energy sources or randomized SVCI on top of PVI. More data are needed regarding cryoablation and SVCI in patients with cardiac devices. The main question remains which clinical phenotype of AF patients might benefit from SVCI.

Summary: The P wave contains information about atrial electrical activity, but technical and biological reasons make it challenging to gather and utilize this information. However, simple ECG descriptors like P-wave dispersion have predictive value for assessing the risk of atrial fibrillation. More complex analysis methods and machine learning techniques may improve the accuracy and reliability of obtaining clinically useful information from the P wave.
