Investigation of Novel pmrB and eptA Mutations in Isogenic Acinetobacter baumannii Isolates Associated with Colistin Resistance and Increased Virulence In Vivo

Colistin resistance in Acinetobacter baumannii is of great concern and is a threat to human health. In this study, we investigate the mechanisms of colistin resistance in four isogenic pairs of A. baumannii isolates displaying an increase in colistin MICs. A mutation in pmrB was detected in each colistin-resistant isolate, three of which were novel (A28V, I232T, and Delta L9-G12). Increased expression of pmrC was shown by semi-quantitative reverse transcription-PCR (qRT-PCR) for three colistin-resistant isolates, and the addition of phosphoethanolamine (PEtN) to lipid A by PmrC was revealed by mass spectrometry. Interestingly, PEtN addition was also observed in some colistin-susceptible isolates, indicating that this resistance mechanism might be strain specific and that other factors could contribute to colistin resistance. Furthermore, the introduction of pmrAB carrying the short amino acid deletion Delta L9-G12 into a pmrAB knockout strain resulted in increased pmrC expression and lipid A modification, but colistin MICs remained unchanged, further supporting the strain specificity of this colistin resistance mechanism. Of note, a mutation in the pmrC homologue eptA and a point mutation in ISAba1 upstream of eptA were associated with colistin resistance and increased eptA expression, which is a hitherto undescribed resistance mechanism. Moreover, no cost of fitness was observed for colistin-resistant isolates, while the virulence of these isolates was increased in a Galleria mellonella infection model. Although the mutations in pmrB were associated with colistin resistance, PEtN addition appears not to be the sole factor leading to colistin resistance, indicating that the mechanism of colistin resistance is far more complex than previously suspected and is potentially strain specific.

Summary: This study assessed the prevalence of vancomycin-resistant Enterococcus faecium (VREfm) in German hospitals upon admission and analyzed the risk factors for colonization. The results showed that age, healthcare contacts, antibiotic use, and PPI/antacid consumption increased the individual risk of VREfm colonization. The study also found that the admission prevalence of VREfm increased each year, mainly driven by the emergence of the ST117 strain.

Summary: This study aimed to characterize the genetic environment of metallo-beta-lactamases (MBL) in carbapenem-resistant clinical Acinetobacter pittii isolates. The results showed the existence of a promiscuous plasmid circulating in A. pittii isolates in Germany with high similarity in the plasmid backbone but differences in the MBL-encoding region.

Summary: This study aims to compare the effect of short and long-term treatment in Gram-negative bacteremia on all-cause mortality in pre-specified subgroups. The results show that in patients with stable hemodynamics and normal temperature for 48 hours prior to discontinuation, seven days of antibiotic therapy for enterobacterales bacteremia result in similar outcomes as 14 days.

This study investigated the activity of delafloxacin against a collection of S. aureus isolates and identified key mechanisms for the development of delafloxacin resistance.

Safety of early oral ambulatory treatment of adult patients with bloodstream infections discharged from the emergency department

Summary: This study evaluates the safety of early oral ambulatory treatment for adult patients diagnosed with bacteremia after discharge from the emergency department. A cohort of 206 febrile ambulatory patients was assessed. Bacteremic low-risk patients were recommended oral treatment and were compared with matched febrile non-bacteremic outpatients. Rates of 14-day mortality and unplanned re-consultations were similar and below 5% in both cohorts, highlighting the safety of oral therapy for low-risk bacteremia, even from its onset.

Alteration of the Chicken Upper Respiratory Microbiota, Following H9N2 Avian Influenza Virus Infection

Summary: The diversity of gut and respiratory microbiota in chickens is related to H9N2 avian influenza virus (AIV) infection. In the acute phase of infection, the diversity of respiratory microbiota decreases, and it does not fully recover to normal levels in the recovery phase after infection. Lactobacillus is associated with the microbiota of infected chickens at different stages of infection. Additionally, the respiratory microbiota in chickens undergoes age-related changes during maturation.
