Summary: Complete mesocolic excision (CME) has been proven to be beneficial for advanced colon cancer. Laparoscopic (Lap) CME with true central vascular ligation (CVL) for advanced right-sided colon cancer is a viable option in surgical oncology. Our institution has been performing Lap D3/modified CME, which involves dissection of main lymph nodes around the root of the middle colic artery (#223LNs) while preserving the left branch of the artery. The median operating time for Lap D3/modified CME was 289 min, with a median blood loss of 57 ml. No metastasis was found in the dissected lymph nodes, indicating the safety and efficacy of this procedure when performed by a skilled surgical team.
