Platelet-rich plasma for foot and ankle pathologies: A systematic review

Background: The aim of this article is to review systematically all the literature available on the clinical application of PRP for the treatment of foot and ankle pathologies, to understand its potential and best indications for clinical use. Methods: A systematic search of the PubMed database was performed. Research criteria were the following: (1) papers in the English language, (2) dealing with the clinical application of PRP for the treatment of orthopedic-related conditions affecting the foot and ankle district, (3) with I to IV level of evidence, and (4) reporting clinical results. Results: A total of 17 studies fulfilled the inclusion criteria. Nine papers dealt with Achilles tendon management, 2 articles with plantar fasciitis, 3 papers with talar osteochondral lesions, 2 with PRP application in total ankle replacement, and 1 article with PRP in foot and ankle fusions. The overall evaluation of the results reported does not clearly demonstrate the potential of PRP treatment in any of the specific fields of application. Conclusions: Considering the literature currently available, no clear indications for using PRP in the foot and ankle district emerged. Radiofrequency ablation is as safe and effective as surgical excision for spinal osteoid osteoma: a systematic review and meta-analysis

Summary: This systematic review and meta-analysis compared the safety and efficacy of radiofrequency ablation (RFA) and surgical excision for the treatment of spinal osteoid osteoma (OO). The results showed that both RFA and surgical excision were effective in pain relief with low recurrence and complication rates. RFA, being a less invasive procedure, proved to be a safe and effective alternative to surgical excision.

Summary: The combination of subchondral and intra-articular BMAC injections showed positive clinical and imaging outcomes for the treatment of knee osteoarthritis. The treatment resulted in stable subjective scores and improvements in KOOS subscales, while VAS pain worsened over time. The MRI findings showed a reduction in bone marrow edema.

Intra-Articular Injection of Botulinum Toxin for the Treatment of Knee Osteoarthritis: A Systematic Review of Randomized Controlled Trials

Summary: The purpose of this study was to review the evidence on intra-articular botulinum toxin (BTX) injection for knee osteoarthritis (OA) and compare it with other treatment options. A systematic review was conducted on randomized controlled trials (RCTs) published in the past 20 years. Nine studies involving 811 patients were included. The quality of the included studies varied, but overall, BTX injection showed positive effects on pain relief and improved function and quality of life. However, compared to other established treatments for knee OA, no clear indication emerged. The methodological quality of the available RCTs on BTX intra-articular injection for knee OA was modest.

Summary: This study describes and assesses the integration of artificial intelligence (AI) technology in preventive orthopaedics of the knee and hip joints. The study found that there are insufficient, varied, and low-quality data for training and validating AI models, as well as a lack of rational evaluation of the models. However, the study suggests that optimizing and confirming the models' quality on different datasets are the final technical barriers for creating usable software and integrating them into the routine of an orthopaedic physician.

Summary: This study aimed to evaluate the clinical and functional outcomes of a salvage surgical approach combining meniscal allograft transplantation (MAT), anterior cruciate ligament reconstruction (ACLR), and high tibial osteotomy (HTO) in patients with complex knee lesions. The patients were evaluated using various scores and tests at short and mid-term follow-ups. The combined procedure showed good and stable results, reducing pain, improving knee function, and slowing down the progression of osteoarthritis, making it a valid treatment option for complex patients.
