Expert Rev. Respir. Med. 6(1), 91-103 (2012) Expert Rev. Respir. Med. 6(1), 91-103 (2012) Asthma and chronic obstructive pulmonary disease are both common conditions with an increasing prevalence worldwide. Inhaled therapy for these conditions has a number of advantages over systemic therapy, including reduced side effects and quicker onset of action. The effective use of inhaled therapy is critically dependent upon the nature of the drug-delivery system and the ability of the patient to use the system correctly. There are a wide number of inhaler devices on the market, each with positive and negative aspects. A crucial part of patient care is to ensure that the choice of inhaler device for the individual is an effective therapy. There are a number of interventions that can help with the choice of inhaler device and also improve the ability of the patient to use inhaled therapy. Inhaler technique training needs to be a cornerstone of the care of patients with asthma or chronic obstructive pulmonary disease to ensure optimal therapy.

Consistency of recommendations from clinical practice guidelines for the management of critically ill COVID-19 patients

Chia Siang Kow, Toby Capstick, Syed Tabish Razi Zaidi, Syed Shahzad Hasan

Summary: This study examined the consistency of recommendations on the management of critically ill COVID-19 patients between various clinical practice guidelines and those of the WHO. Inconsistencies were observed between some recommendations, highlighting the need for consensus among relevant bodies to address these discrepancies in clinical practice.

Smartphone pedometers in adults with asthma: a practical approach to physical activity assessment? A pilot validation study

Caroline Reilly, Antonis Stavropoulos-Kalinoglou, Ian Clifton, Jim McKenna, Daniel Peckham, Oliver J. Price

Summary: The findings suggest that the built-in iPhone (R) pedometer offers a practical approach for assessing physical activity in adults with and without asthma. During self-paced walking, all devices provided excellent step-count estimates. In free-living conditions, both Yamax Digiwalker (TM) SW800 and iPhone (R) pedometers showed similar accuracy, indicating the potential of smartphone pedometers in monitoring and promoting physical activity.

Why is mock care not a good proxy for predicting hand contamination during patient care?

M. F. King, A. M. Wilson, M. Lopez-Garcia, J. Proctor, D. G. Peckham, I. J. Clifton, S. J. Dancer, C. J. Noakes

Summary: The study observed healthcare workers' behaviors during different types of care procedures, finding significant differences in surface contacts, contact time, and hand hygiene behaviors between actual and mock care settings. The study also highlighted variations in duration and hand hygiene practices among different healthcare professionals. Additionally, the study showed that mock care underestimated hand microbial contamination by approximately 30% compared to actual care.

Effects of patient room layout on viral accruement on healthcare professionals' hands

Amanda M. Wilson, Marco-Felipe King, Martin Lopez-Garcia, Ian J. Clifton, Jessica Proctor, Kelly A. Reynolds, Catherine J. Noakes

Summary: Healthcare professionals are exposed to highly infectious viruses such as norovirus through multiple exposure routes. A study evaluated the influences of hospital patient room layout on differences in predicted hand contamination from deposited norovirus particles. Differences in viral accruement were influenced by bioaerosol deposition patterns and behavior sequences, with doctors' rounds resulting in the greatest predicted viral accruement on hands.

Summary: This study aimed to investigate the nutritional status and intakes in patients with bronchiectasis, finding that most patients did not meet energy requirements but exceeded protein RNI, with inadequate intake of vitamin D. BMI masked functional, body composition, and nutritional deficits in patients, especially in the younger PCD subgroup.
