Consensus Report: Definition and Interpretation of Remission in Type 2 Diabetes

Matthew C. Riddle, William T. Cefalu, Philip H. Evans, Hertzel C. Gerstein, Michael A. Nauck, William K. Oh, Amy E. Rothberg, Carel W. le Roux, Francesco Rubino, Philip Schauer, Roy Taylor, Douglas Twenefour

Summary: Improvement of glucose levels in people with diabetes can occur spontaneously or after medical interventions. Newer forms of treatment may lead to more sustained improvement. However, the terminology and objective measures for defining this process are not established, and the long-term risks and benefits are not well understood. An international expert group proposed the term "remission" and HbA1c < 6.5% as the usual diagnostic criterion. They also suggested active observation and further research on predictors and outcomes of remission.

Effects of Vitamin D Supplementation on Insulin Sensitivity and Secretion in Prediabetes

Neda Rasouli, Irwin G. Brodsky, Ranee Chatterjee, Sun H. Kim, Richard E. Pratley, Myrlene A. Staten, Anastassios G. Pittas

Summary: Supplementation with vitamin D-3 for 24 months did not improve beta-cell function in people with prediabetes, but showed benefits in those with very low baseline vitamin D levels.

Prevalence and Clinical Determinants of Obesity in Adults With Type 1 Diabetes Mellitus: A Single-Center Retrospective Observational Study

Yumiko Tsushima, Keren Zhou, James F. Bena, Sangeeta R. Kashyap

Summary: The objective of this study was to assess the prevalence of obesity and related treatments in the adult population with type 1 diabetes mellitus. The study found that more than half of the patients were overweight or obese, and obesity was associated with a higher risk of cardiovascular disease. Additionally, the obesity group had higher levels of triglycerides and lower levels of high-density lipoprotein compared to the normal weight group. The use of weight loss medications was limited.

Early Post-Renal Transplant Hyperglycemia

Anira Iqbal, Keren Zhou, Sangeeta R. Kashyap, M. Cecilia Lansang

Summary: Early post-renal transplant hyperglycemia (EPTH) is associated with significant complications and needs to be recognized and managed proactively. It increases the risk of acute graft failure, rehospitalizations, cardiovascular events, post-renal transplant diabetes, and infections.

The effect of vitamin D supplementation on cardiovascular risk in patients with prediabetes: A secondary analysis of the D2d study

Cyrus Desouza, Ranee Chatterjee, Ellen M. Vickery, Jason Nelson, Karen C. Johnson, Sangeeta R. Kashyap, Michael R. Lewis, Karen Margolis, Richard Pratley, Neda Rasouli, Patricia R. Sheehan, Anastassios G. Pittas

Summary: The study investigated the effects of vitamin D supplementation on cardiovascular outcomes in adults with prediabetes, finding that it did not reduce Major Adverse Cardiovascular Events (MACE) but did have a small favorable effect on ASCVD risk score.

Effects of Metabolic Medicine and Metabolic Surgery on Patient-Reported Outcomes Among Patients with Type 2 Diabetes

Karim Kheniser, Ali Aminian, Sangeeta R. Kashyap

Summary: The assessment and management of patient-reported outcomes (PROs) is considered secondary to that of cardiometabolic outcomes. Health-related quality of life (HRQOL), a PRO, can provide information that can't be obtained from cardiometabolic assessments, such as quantifying and treating physical and mental distress, as well as evaluating treatment convenience and satisfaction.

Metabolic labeling unveils alterations in the turnover of HDL-associated proteins during diabetes progression in mice

Prabodh Sadana, Melissa Edler, Mirjavid Aghayev, Andrea Arias-Alvarado, Emilie Cohn, Serguei Ilchenko, Helen Piontkivska, Jagan A. Pillai, Sangeeta Kashyap, Takhar Kasumov

Summary: This study investigates the impact of gradual hyperglycemia during the progression of diabetes on HDL-associated protein dynamics in prediabetic and diabetic mice. The findings show that the fractional synthesis rate of HDL-associated proteins increases in prediabetic mice and decreases in diabetic mice compared to control mice. These kinetic changes provide insights into the mechanism of altered protein levels and HDL dysfunction during diabetes progression.

Association between first-line monotherapy with metformin and the risk of atrial fibrillation (AMRAF) in patients with type 2 diabetes

Anira Iqbal, Zehra Tekin, Michael W. Kattan, Xinge Ji, Alex Milinovich, Kevin M. Pantalone, Robert S. Zimmerman, Mina K. Chung, Sangeeta Kashyap

Summary: Initiation of metformin as a first line monotherapy for T2D is not associated with a decreased risk of developing AF.

Prevalence and clinical determinants of non-alcoholic fatty liver disease by liver scores in adults with type 1 diabetes

Michelle D. Lundholm, James Bena, Keren Zhou, Yumiko Tsushima, Sangeeta R. Kashyap

Summary: This study aimed to investigate the prevalence and clinical risk factors for non-alcoholic fatty liver disease (NAFLD) in type 1 diabetes (T1DM) using liver scores. The results showed that liver steatosis was common in adults with T1DM with obesity and/or metabolic syndrome, and was associated with older age, hypertension, and dyslipidemia. However, the diagnosis and treatment of NAFLD were still inadequate.

Indices of hepatic steatosis and fibrosis in prediabetes and association with diabetes development in the vitamin D and type 2 diabetes study*

Summary: NAFLD is common in individuals at high risk for developing T2D and is associated with worse glycemic control. Routine screening for NAFLD using non-invasive scores may help reduce the burden of T2D and identify individuals who require further follow up to assess liver health.

Metabolic syndrome biomarkers relate to rate of cognitive decline in MCI and dementia stages of Alzheimer's disease

Jagan A. Pillai, James Bena, Lynn Bekris, Nandan Kodur, Takhar B. Kasumov, James B. R. Leverenz, Sangeeta R. Kashyap

Summary: The relationship between biomarkers of metabolic syndrome and cognitive decline in individuals with mild cognitive impairment (MCI) and dementia stages of Alzheimer's disease (AD) was investigated. Elevated plasma triglyceride/HDL cholesterol (TG/HDL-C) ratio and plasma Apolipoprotein A1 (ApoA1) levels were associated with worse cognitive outcomes in MCI and dementia participants. However, CSF ApoA1 levels were found to relate to slower cognitive decline in MCI.

Glycemic and metabolic sub-classification of prediabetes and risk factors for cardiovascular disease in the D2d cohort

Sangeeta R. Kashyap, Cyrus Desouza, Vanita R. Aroda, Sun H. Kim, Lisa M. Neff, Shannon S. Wu, Philip Raskin, Richard Pratley

Summary: This study analyzed the association between glycemic and metabolic sub-classifications of prediabetes and cardiovascular disease (CVD) risk, finding that multiple glycemic and metabolic criteria are needed to fully recognize the elevated CVD risk profile in prediabetes.
