Thiamine (vitamin B-1) serves as an important cofactor in body metabolism and energy production. It is related with the biosynthesis of neurotransmitters and the production of substances used in defense against oxidant stress. Thus, a lack of thiamine affects several organ systems, in particular the cardiovascular and nervous system. The cardiac insufficiency caused by thiamine deficiency is known as cardiac beriberi, with this condition resulting from unbalanced nutrition and chronic excessive alcohol intake. Given that the disease is now very rare in developed nations such as Korea, it is frequently missed by cardiologists, with potentially fatal consequences. Herein, we present a case study in order to draw attention to cardiac beriberi. We believe that this case will be helpful for young cardiologists, reminding them of the importance of this forgotten but memorable disease.

The results showed that lower LV-GLS values were associated with higher rates of SCD events. LV-GLS provided additional information to established risk models and improved their predictive performance for SCD.

Summary: The study aimed to investigate the association between low-normal left ventricular ejection fraction (LVEF) and adverse outcomes in hypertrophic cardiomyopathy (HCM), as well as evaluate the predictive power of LVEF in the conventional HCM sudden cardiac death (SCD)-risk model. The results showed that low-normal LVEF was an independent predictor of heart failure hospitalization and cardiovascular death in patients with HCM.

Prediction of Deterioration of Left Ventricular Function Using 3-Dimensional Speckle-Tracking Echocardiography in Patients With Left Bundle-Branch Block

Summary: This study found that 2D global longitudinal strain (GLS) is associated with cardiovascular outcomes in patients with left bundle-branch block, but the predictive value of 3D speckle-tracking echocardiography has not been investigated in these patients. The study retrospectively analyzed the association between 2D and 3D strains and left ventricular ejection fraction (LVEF) in patients with preserved or reduced LVEF. The results showed that 2D-GLS is predictive of maintaining LVEF in patients with preserved LVEF, while a lower 3D-global circumferential strain is associated with persistently reduced LVEF in patients with reduced LVEF.

Association between renin-angiotensin-aldosterone system blockade and clinical outcomes in patients with hypertension: real-world observation from a nationwide hypertension cohort

Summary: This study investigated the association between the use of ACEi, ARB, or non-RASi and incident cardiovascular events in a nationwide hypertension cohort. The results showed that patients on ACEi and ARB had lower risks of myocardial infarction, ischemic stroke, atrial fibrillation, heart failure, and all-cause deaths compared to those on non-RASi. Sensitivity analysis and propensity score matching results supported these findings.

Summary: The aim of this study was to investigate the prognostic utility of left ventricular global longitudinal strain (LV-GLS) in patients with hypertrophic cardiomyopathy (HCM) and a left ventricular ejection fraction (LVEF) of 50-60%. The study included 349 HCM patients with an LVEF of 50-60%, and the results showed that LV-GLS was an independent predictor of a composite outcome including cardiovascular death in these patients. Therefore, LV-GLS can aid in risk stratification in HCM patients with an LVEF of 50-60%.
