Summary: Tracheal stenosis is a rare condition where the trachea becomes narrow. A thorough preoperative evaluation is essential to determine the appropriate surgical procedure. Tracheal resection and anastomosis is a viable option, especially for extensive and high-grade lesions or failed endoscopic procedures. The patient in this case had undergone balloon dilatation and tracheal resection before, and was referred to our clinic with residual tracheal stenosis. We illustrate the surgical procedure and highlight a unique way to perform the anastomosis, particularly in a revision surgery.

Summary: Tracheal transplantation is an ideal option for long-segment circumferential tracheal defects. Our group successfully performed the first vascularized single-staged tracheal transplantation in January 2021. Histological changes showed loss of ciliated epithelium and seromucinous cells one week after transplantation, but recovery of normal ciliated epithelium and mucous cells over time.

Summary: Tracheal replacement efforts have been largely unsuccessful due to difficulties in re-vascularising and replicating the mechanical properties of native tissue. This study aimed to improve a collagen-hyaluronic acid scaffold and use 3D-printed polymer designs to optimize the tracheal replacement device. The results showed that the composite scaffolds exhibited angiogenic growth and the presence of blood vessels in vitro and ex vivo.

Summary: The formation of a functional and perfusable vascular network is crucial for the long-term survival and functionality of tissue-engineered tracheal grafts after transplantation. Traditional prevascularization methods for tracheal grafts have limitations, and a one-step vascularization remains challenging. This study proposes a method to construct a tissue-engineered tracheal graft that forms a microvascular network after orthotopic transplantation in vivo. The hybrid tracheal graft prepared in this study met the requirements of being non-immunogenic, biomechanically strong, supportive of cell proliferation, and quickly vascularized. The use of endothelial progenitor cells (EPCs) as vascularized seed cells in this study resulted in the successful formation of microvascularized tracheal grafts, leading to a satisfactory therapeutic outcome. This study represents a promising approach for microvascular tracheal reconstruction.

Summary: In this study, a cell-free tissue-engineered tracheal substitute was developed using a 3D-printed polycaprolactone scaffold coated with a gelatin-methacryloyl hydrogel. Transforming growth factor-β1 (TGF-β) and stromal cell-derived factor-1α (SDF-1) were embedded in the scaffold to facilitate cell recruitment and differentiation. The tracheal substitute showed positive results in promoting the recruitment of cells and enhancing differentiation towards a chondrocyte phenotype.
