Summary: This study investigates the impact of lymph node yield after transthoracic esophagectomy on lymph node metastasis, complications, and survival in patients with esophageal adenocarcinoma. The results demonstrate that a lymph node yield of 31 or higher is associated with an increased number of positive lymph nodes, improved disease-free survival, and overall survival, without increasing morbidity.

Summary: The study showed that for patients with cT2N0M0 adenocarcinoma, neoadjuvant chemoradiotherapy (NT) does not increase technical complexity or have adverse effects on postoperative outcomes after esophagectomy. Furthermore, NT was selectively associated with improved survival in some patients with cT2N0M0 tumors larger than 5 cm in diameter.

Summary: The study shows that lymph node ratio (LNR) is an independent prognostic factor for patients with esophageal squamous cell cancer (ESCC) after minimally invasive esophagectomy (MIE). Particularly for patients with pN1 status, LNR is more useful than the number of metastatic lymph nodes for predicting survival outcomes.

Summary: This study suggests that a higher number of examined lymph nodes (ELNs) in patients with esophageal squamous cell carcinoma (ESCC) who underwent right transthoracic esophagectomy is associated with more accurate nodal staging and better long-term survival. The optimal ELN count threshold was found to be 21.

Summary: This study compared the survival outcomes of neoadjuvant chemoradiotherapy followed by operation, definitive chemoradiotherapy, and esophagectomy alone in patients with locoregional esophageal squamous cell carcinoma. It found that neoadjuvant chemoradiotherapy followed by operation and esophagectomy alone were associated with better overall survival compared to definitive chemoradiotherapy.

Summary: This study aimed to investigate the impact of prolonged time to surgery (TTS) on the outcomes of patients with locally advanced esophageal squamous cell carcinoma (ESCC) undergoing neoadjuvant immunochemotherapy (nICT). The results showed that prolonged TTS did not significantly affect pathological complete response rate, disease-free survival, and short-term postoperative outcomes.

Summary: In cN+ esophageal adenocarcinoma patients treated with nCRT, those with ypN0 and no signs of lymph node response (LNR) have a tripled median overall survival compared to those with LNR, and their survival is comparable to cN+/pN0 upfront esophagectomy patients.

Summary: This study aimed to determine whether adjuvant chemotherapy provides a survival benefit for high-risk patients with T2-4a, pathologically node-negative distal esophageal adenocarcinoma. The results showed that adjuvant chemotherapy, with or without radiation, did not demonstrate a significant survival benefit compared to no adjuvant therapy.

Summary: The lymph node invasion mechanism plays a prognostic role in pancreatic ductal adenocarcinoma (PDAC) after resection, but it is not addressed in the current cancer classification system. This study analyzed the prognostic role of different mechanisms of lymph node invasion on PDAC and suggests a rethinking of the TNM classification based on lymph node metastasis mechanisms in PDAC.

Summary: This study compared minimally invasive and open surgery approaches in treating ESCC patients who underwent neoadjuvant therapy. Results showed that MIE had shorter operative time, less intraoperative bleeding, and lower anastomotic leakage rate compared to open surgery. Overall survival and disease-free survival did not differ between the two approaches for patients receiving neoadjuvant chemotherapy, but for patients receiving neoadjuvant chemoradiotherapy, MIE showed significantly better overall survival rates.

Summary: NACRT followed by esophagectomy with three-field lymph node dissection is feasible and offers the potential for long-term survival of clinical Stage III patients and even clinical Stage IVB patients with supraclavicular LN metastasis as the only distant metastatic factor. At least in patients with upper and middle thoracic ESCC, treating supraclavicular LNs as regional LNs seems to be appropriate.

Summary: This study aimed to investigate the association between elevated body mass index (BMI) and perioperative and oncological outcomes among elderly patients undergoing minimally invasive McKeown esophagectomy (MIE). The study found that elevated BMI was associated with prolonged operation time and increased incidence of intraoperative hypoxemia, but there were no significant differences in other perioperative and oncological outcomes between the normal BMI group and the elevated BMI group. After propensity score matching analysis, there were still no significant differences in perioperative and oncological outcomes between the two groups.

Summary: The study investigated the benefit/risk ratio of surgery in octogenarians with early-stage non-small cell lung cancer. Results showed that the introduction of a dedicated geriatric assessment pathway, the use of minimally invasive surgery, and the performance of systematic lymphadenectomy were associated with improved long-term survival in octogenarian patients. Patients with multiple co-morbid conditions, diabetes mellitus, or low ppoDLCO values may be more appropriate candidates for SBRT.
