Summary: The study found that advanced age was associated with a lower likelihood of completing life-sustaining treatment decisions in heart failure patients at 3 months, while higher education and more functional impairment were associated with a higher likelihood. Early AD discussions may be feasible in patients with mild symptomatic heart failure who have poor knowledge about ADs, taking into account both non-modifiable and modifiable factors.

Summary: An advance care planning intervention was conducted on heart failure patients, showing that most participants had positive attitudes towards advance directives but completion rates were low. The discussion of goals between patients and significant others is an important factor in end-of-life planning. Further research is needed to improve strategies for provider discussions and advance directive completion.

Summary: This study aimed to develop a pediatric Advance Care Planning (pedACP) program that meets the specific needs of children, parents, and professionals. Through collaborative allocation of content, participants, tools, and warning notes, the researchers analyzed and translated the results into a pedACP program draft. The participants focused on the children's quality of life and individualized interdisciplinary communication. The program was designed with modules for trust-building, summarizing results, and addressing general themes. The participants appreciated the program's comprehensiveness and flexibility.

Summary: Communication about palliative care principles and advance care planning is limited in pediatric oncology and often occurs later in the illness trajectory. Development of educational and clinical interventions is needed to encourage earlier dialogue on these topics.

Summary: The study developed a care pathway for advance care planning for cancer patients in hospital through a modified Delphi study, resulting in a final version with 148 elements covering 10 domains. This will help standardize the process and ensure cancer patients' care aligns with their preferences and values.

Summary: Adolescents and their caregivers agree that ACP should occur early in the disease course, but there are discrepancies regarding communication of prognosis and perceived adolescent stress related to ACP discussions.

Summary: This study aims to identify the potential benefits and missed opportunities of ACP and provide an evidence-informed, clinically relevant path for ACP in serious illness. Early ACP practices focused on document completion, potentially missing holistic preferences. ACP models that emphasize communication show promising results for patients and clinicians.

Summary: Early palliative care improves patient quality of life and reduces aggressive care at the end of life. Completed advance directives and palliative care consultation initiated at least 90 days prior to death are associated with reduced aggressive care indicators.

Summary: The study found that palliative care is more effective for patients with heart failure compared to other noncancer illnesses, reducing potentially unwanted hospital admissions and promoting advance care planning. Additionally, there was an improvement in symptom burden after palliative care intervention.

Summary: Heart failure and chronic obstructive pulmonary disease are major burdens worldwide, often coexisting and leading to worsened quality of life and mortality. Palliative care plays a significant role in improving the quality of life for people with these conditions, and early integration is emphasized in current guidelines.

Summary: This study aims to pilot an educational intervention to improve knowledge of conservative kidney management and advance care planning among older and/or functionally impaired patients with advanced chronic kidney disease. The research will assess the feasibility and effectiveness of the intervention through a randomized controlled trial, and compare the intervention effects on knowledge and preferences communication among different racial groups.

Summary: Left ventricular (LV) pacing in transcatheter aortic valve implantation (TAVI) shows similar safety and efficacy to right ventricular (RV) pacing. LV pacing is associated with a lower risk of tamponade due to RV perforation and shorter procedure duration. The crossover rate to RV pacing is low.

Summary: This article provides an overview of the epidemiology of acute heart failure (AHF) and discusses patient characteristics, classifications, and management options, including novel therapies. There is significant heterogeneity among AHF patients, and a new classification scheme has been proposed to better stratify patients and guide treatment. Intravenous diuretics, vasodilators, and inotropes are still the mainstay of acute phase management, and the use of mechanical circulatory support is expanding. Promising results have been seen with treatments that have shown efficacy in chronic heart failure.

Summary: This cross-sectional study aimed to estimate the seroprevalence rate of Chagas disease among people of Latin American descent in Suffolk County, Long Island, New York. Screening was done using the Chagas Detect Plus Rapid Test, followed by confirmatory tests and a questionnaire to identify risk factors. The study found a seroprevalence rate of 10.74% and identified residence in palm leaf houses, houses with triatomines, and a history of triatomine bites as significant risk factors. The findings highlight the importance of early screening for this underdiagnosed disease in high-risk populations.

Summary: The study found that the risk of concussions is higher on artificial turf compared to natural grass, and this risk is exacerbated in cold weather and later in the season. The complex interplay among playing surface, temperature, and week of the season highlights the importance of accounting for multiple factors and their interactions in investigating and mitigating sports injury risk factors.

Summary: This study compared the outcomes of transcatheter mitral valve repair (TMVr) and surgical mitral valve repair (SMVr) in patients with secondary mitral regurgitation (MR) using a global federated network database. The results showed that SMVr had significantly better survival rates and lower reintervention rates at 1- and 3-years compared to TMVr.
