Purpose of ReviewCardiac sarcoidosis (CS) is associated with significant morbidity and mortality. The diagnosis of CS is challenging and typically one that is only entertained after many other conditions have been ruled out. A high index of suspicion is necessary in order to correctly determine appropriate testing for the disease. Transthoracic echocardiography is the most readily available imaging modality available to help establish a diagnosis in a potential patient. However, no one echocardiographic feature is pathognomonic.Recent FindingsOn echocardiography, unusual wall motion abnormalities, which do not fit a classic coronary distribution, along with diastolic dysfunction may alert one to the presence of cardiac sarcoid, particularly in the right clinical context. Myocardial strain imaging on echocardiography may increase the sensitivity of identifying cardiac sarcoidosis. Alternative imaging with cardiac magnetic resonance imaging or positron emission tomography have become more frequently utilized to establish a diagnosis of CS.SummaryCardiac sarcoidosis remains a difficult condition to diagnose. However early diagnosis is critical to decrease the associated high mortality. Endomyocardial biopsy is highly specific but lacks sensitivity due to the patchy nature of the granulomatous deposition. Thus, imaging plays a role in diagnosis as well as for follow-up. Echocardiography remains an hallmark during the workup for CS. Decreased sensitivity of echocardiography has facilitated the use of other techniques to establish the presence of CS.

Summary: This study compared clinical characteristics and in-hospital outcomes between Japanese and European takotsubo syndrome (TTS) patients, revealing differences in triggers and outcomes between the two ethnic groups. Despite worse in-hospital outcomes for Japanese patients, ethnicity itself does not appear to impact TTS patient outcomes. Machine learning analysis identified physical stressors as the most important prognostic factor for both Japanese and European TTS patients.

Summary: The time between symptoms onset and primary percutaneous coronary intervention (PCI), known as symptoms onset to needle time (SNT), is associated with procedural results and the occurrence of ischemic endpoints in primary angioplasty patients. Longer SNTs are related to a higher incidence of ischemic events, especially in high-risk patients.

Clinical and radiological factors predicting stroke outcome after successful mechanical intervention in anterior circulation

Summary: This study analyzed the factors influencing clinical outcomes after mechanical intervention in patients with large-vessel occlusion (LVO) in the anterior circulation. Age, leptomeningeal collaterals, onset to intervention time, ASPECTS, initial NIHSS, and leukoaraiosis were identified as independent determinants of clinical outcomes. The presence of leptomeningeal collaterals and absence of leukoaraiosis predicted a good outcome.

Patient characteristics, treatment strategy, outcomes, and hospital costs of acute coronary syndrome: 3 years of data from a large high-volume centre in Central Europe

Summary: Managing patients with acute coronary syndrome (ACS) in an ageing population with comorbidities is challenging. This study aimed to create a detailed registry of unselected ACS patients in a high-volume center in Central Europe. The data of 1804 patients were analyzed, and the results showed that the in-hospital mortality of unselected NSTE-MI and STEMI patients is not low despite the implementation of guideline-recommended therapy. NSTE-MI patients had the longest total hospital stay and highest financial costs. Age, pre-hospital cardiac arrest and/or mechanical ventilation, subacute STEMI, and ejection fraction below 40% were identified as the most powerful predictors of in-hospital mortality.

Summary: This study describes the outcomes of patients with chronic coronary artery disease and/or peripheral artery disease who were treated with the combination of rivaroxaban and aspirin during a long-term open-label extension. The results showed that the efficacy and bleeding rates during the extended combination treatment were similar to or lower than those during the randomized treatment phase, without any new safety signals.

Is it time to replace acetylsalicylic acid monotherapy in secondary prevention with a more effective treatment?

Summary: Pharmacological treatments have been shown to be more effective than monotherapy with ASA in preventing complications of atherosclerosis. Combination therapy with ASA and anticoagulants had increased bleeding risk, while clopidogrel monotherapy showed slight improvements in efficacy but was not widely adopted due to cost. The combination of ASA with low-dose rivaroxaban has been proven to be the most effective treatment.

Comparison of Investigator-Reported vs Centrally Adjudicated Major Adverse Cardiac Events A Secondary Analysis of the COMPASS Trial

Summary: This secondary analysis of the COMPASS trial found that dual pathway inhibition significantly reduced cardiovascular events among patients with stable atherosclerotic disease compared with aspirin plus placebo, whether assessed by blinded site investigators or adjudicators. These findings suggest that using investigator-reported events in blinded clinical trials may be a more efficient alternative to adjudication.

Summary: This study aimed to investigate whether sex differences play a role in the treatment outcomes of ST-elevation myocardial infarction (STEMI). The analysis of three randomized trials showed that over time, the prognosis for women with myocardial infarction treated with primary PCI has significantly improved. However, timely diagnosis remains a major obstacle in achieving optimal outcomes for women.
