Summary: A study in Pakistan on hypertensive hemodialysis patients revealed that most patients had normal body mass index and reduced salt intake, with pain/discomfort being the most problematic dimension. Significant improvements were observed in self-care and usual activities during follow-up visits.

Summary: The study shows that for patients over 67 years old, peritoneal dialysis (PD) has a significant advantage over in-center hemodialysis in terms of Medicare expenditures, although there is no significant difference in the dialysis modality. Over time, the gap in dialysis expenditures gradually narrowed, while the gap in nondialysis expenditures widened.

Summary: Peritoneal dialysis allows for home-based therapy and greater autonomy, leading to improved satisfaction compared to in-center care. However, the treatment burden remains significant, highlighting the importance of person-centered care. Shared decision making is central to prescribing PD and balancing the benefits and burdens of treatment.

Summary: This study aims to assess the quality of life (QoL) and the associated predictors in ESKD Saudi Arabian patients receiving either hemodialysis (HD) or peritoneal dialysis (PD). The results showed significant differences in the KDQOL-SF-36 physical composite score between the HD and PD groups, with the PD group having a higher mean. However, more PD patients reported feeling depressed compared to HD patients.

Summary: The study reviewed the effectiveness of group session interventions in improving the well-being of family caregivers of patients undergoing hemodialysis (HD) and found significant improvements. Insufficient evidence exists to make recommendations for caregivers of patients undergoing peritoneal dialysis (PD).

Summary: This study found that the combination therapy of PD and once-weekly hemodialysis is associated with lower infection-related mortality but not with a lower incidence of EPS in the short term. Patients on combination therapy had a decreasing D/P Cr ratio over time, while those on PD alone had an increasing ratio.

Summary: Peritoneal dialysis is an important treatment option for patients with kidney failure, providing various benefits such as enhanced participation in daily activities, flexibility in schedules, cost savings, and improved quality of life. However, the utilization of peritoneal dialysis has remained static or declining in certain regions due to concerns about PD-related infections. This review aims to provide an overview of the definition, epidemiology, risk factors, prevention, and treatment of PD-related infections based on current evidence.

Summary: Identifying performance-based tests that are meaningful for patients can help in implementing rehabilitation programs. This study found associations between different performance-based tests and health-related quality of life among elderly peritoneal dialysis patients, with the timed up and go (TUG) test being particularly valuable for assessing physical health. Depressive symptoms and nutrition were also identified as important areas for optimizing the overall health-related quality of life in this population.

Summary: Incremental peritoneal dialysis (PD) refers to lower than standard dose PD prescription, which has potential benefits like better preservation of residual kidney function and reduced risk of peritonitis. However, it may also lead to reduced solute clearance and fluid overload.

Summary: This study compared the mental health of in-center hemodialysis and peritoneal dialysis patients during the COVID-19 pandemic and found that in-center hemodialysis patients were more prone to mental symptoms. However, there was no difference in mental health-related quality of life between the two dialysis modalities.

Summary: The study evaluated the significance of MMP-3, CXCL-13, and C5a in different stages of AAV. The three markers showed potential in distinguishing active vasculitis from other pathologies, with CXCL-13 being a potential marker in distinguishing active vasculitis from long term remission. MMP-3 levels were related to kidney function expressed by eGFR and caution should be taken in interpreting its elevation in patients with kidney failure.

Summary: The study evaluated the efficacy and safety of Rituximab induction therapy in patients with small vessel vasculitis. The results showed a high remission rate, with 85% achieving complete remission and 10% achieving partial remission within 6 months. However, adverse events such as infections were noted, with advanced kidney failure being one of the risk factors for these side effects.

Summary: In diabetic patients receiving dialysis, those who died had higher levels of HbA1c and fructosamine, as well as higher comorbidity. History of stroke or transient ischemic attack was an independent risk factor of all-cause death, while regular physical activity significantly reduced the risk of all-cause death.

Summary: Extracorporeal membrane oxygenation (ECMO) is a treatment modality that requires frequent use of antibiotics. However, there are several variables that can affect the success of antibiotic therapy during ECMO implementation, including changes in drug distribution, clearance, adsorption into circuit components, and complex interactions with critical care illnesses. Additionally, ECMO complicates the assessment of antibiotic effectiveness due to difficulties in detecting fever or other signs, and the immunogenicity of the circuit can affect biomarkers and disrupt the immune system.

Summary: This study investigated the changes in complement in the blood during SARS-CoV-2 infection and its relationship with viral burden, tissue necrosis, and immune response. The results showed that C5a levels remained elevated, peaking at 5-7 days. Different components of complement formed three clusters in COVID-19 patients. Decreased levels of FH and ApoE were predictive of death, while TCC and C5a were correlated with patient length of stay, APACHE, and CRP.

Summary: Cardiac surgery triggers acute changes in serum leptin and fibroblast growth factor 21 (FGF-21). The long-term dynamics after cardiac surgery are unknown. This study found that leptin and FGF-21 evolve independently from the inflammatory response and correlate with cardiac remodeling and neurodegeneration markers.

Summary: Poor outcomes in Subarachnoid Hemorrhage (SAH) can be attributed to Delayed Cerebral Ischemia (DCI), a unique form of secondary neurological injury. DCI is characterized by new neurological insults that occur beyond 72 hours after the hemorrhage, even without radiographic evidence of vasospasm. Recent evidence suggests that Cortical Spreading Depolarizations (CSDs) may be responsible for DCI, triggering neuroinflammation, microthrombi formation, and vasoconstriction. Ketamine and Nimodipine show promise in treating and preventing CSDs, but further research is needed to determine their therapeutic potential.

Summary: The variances in operating room cancellation rates between different service lines and operators within these service lines were assessed by reviewing the electronic medical record covering 34,561 cases performed by 199 OR operators in 2018. The study found that the cancellation rates differed between service lines but were minimal between operators within the same service line. The main factors contributing to cancellation variability were patient-specific factors, seasonal factors, and administrative issues.

Summary: This study aimed to assess facilitators and barriers of Tele-Critical Care (TCC) perceived by SCCM members, finding that consumers preferred algorithmic coverage and scheduled rounds, with reactive and on-demand models being less utilized. Those who terminated TCC services were mainly due to factors related to costs, perceived lack of need for services, and workflow challenges.
