Summary: This study conducted a systematic review and meta-analysis of relevant randomized controlled trials to investigate the effects of coronary revascularization on patients with heart failure receiving guideline-recommended pharmacological therapy. The results showed that coronary revascularization was associated with a lower risk of all-cause mortality and cardiovascular mortality compared to medical therapy alone, but had no significant effect on the composite endpoint of hospitalization for heart failure or all-cause mortality.

Summary: This study aimed to investigate the relationship between diabetes mellitus (DM) status and the risk of heart failure (HF) occurrence in patients with coronary artery disease (CAD). The results showed that patients with DM had a significantly higher risk of HF, whether in the acute myocardial infarction (MI) phase or the chronic stable phase. Therefore, it is crucial to prevent HF occurrence in CAD patients with DM.

Summary: This study analyzed the incidence and prognosis of perioperative myocardial infarction (pMI) after coronary artery bypass graft surgery using different definitions. The study found significant differences in the occurrence of pMI depending on the definition used, and the 4th Universal Definition of Myocardial Infarction and Academic Research Consortium criteria were strong independent predictors of all-cause mortality. In addition, new electrocardiographic abnormalities, regional wall motion abnormalities, or coronary angiography detected signs of ischemia were also prognostic factors for primary and secondary endpoints.

Summary: This study used a nationwide database to analyze the trends and outcomes of isolated coronary artery bypass grafting after ST-elevation myocardial infarction. The results showed a decline in the use of this procedure, but no change in in-hospital mortality. A recent cohort study found that performing the grafting on day 2 had similar in-hospital mortality compared with day 3 or more, but lower rates of complications and shorter hospital stays.

Summary: Patients with left main coronary artery disease presenting with ACS have higher rates of early death, but the long-term all-cause mortality rates were similar between PCI and CABG in this high-risk subgroup. The advantages and disadvantages of PCI and CABG in terms of early stroke and long-term spontaneous MI and repeat revascularization were consistent regardless of ACS status.

Summary: This nationwide study in Sweden aimed to describe the incidence, management, and outcome of spontaneous coronary artery dissection (SCAD) patients. The study found a low incidence of SCAD, with a higher proportion of male patients compared to previous studies. SCAD patients were younger with lower cardiovascular risk burden, but still had higher mortality and morbidity rates and more frequent acute coronary reangiography.

Summary: CMD is a challenging condition to manage due to its heterogenous pathophysiology, presentation, and response to therapy. Although awareness of CMD is improving, there is a lack of randomized trials for therapy. Invasive assessment of the coronary microcirculation can improve patient-centered outcomes. Beta-blockers are the cornerstone of therapy for CMD angina, and non-pharmacological interventions play a central role in management. Further research is needed to assess the impact of traditional and novel pharmacological therapies on symptoms and clinical events in different CMD endotypes.
