Although the pediatric foot and ankle examination is essentially similar to that of the adult patient, there are subtle differences unique to a child's examination. The unique findings are discussed in this article based on the following: weight-bearing examination, gait evaluation, non weight-bearing examination, vascular examination, neurologic examination, and dermato-logic examination for pediatric patients. A comprehensive overview is provided; however, other challenges presented in evaluating children include management of a child's parents and a child's temperament. The setting for an examination and a child's mood must be taken into consideration to ensure a successful outcome.

The Lapidus Bunionectomy Revolution Current Concepts and Considerations

Christy M. King, Francesca M. Castellucci-Garza

Summary: For centuries, the HV deformity has been a problem for the human foot. Despite numerous bunionectomy procedures and fixation options, there is no perfect solution. Surgeons should address each patient's deformity in all three planes and choose a technique they are comfortable with to ensure successful reduction and stabilization with minimal complications. Early weight-bearing protocols are supported in the literature but can be personalized based on surgeon preference and patient characteristics.

Clubfoot Emphasis on the Complex and Atypical Subsets

Mitzi L. Williams, Matthew B. Dobbs

Summary: Clubfoot, a common congenital musculoskeletal condition, is best treated using the Ponseti technique. Accurate and thorough neurological examinations are necessary for identifying atypical cases and the need for specific corrective maneuvers. Long-term follow-up is crucial for minimizing progressive deformities, emphasizing the importance of individualized care.

Considerations in Charcot Reconstruction

Luke J. McCann, Joseph D. Dickinson

Summary: The authors emphasize the importance of aggressive treatment for Charcot foot, which not only helps control deformities but also improves patients' quality and length of life. They suggest that Charcot reconstruction surgery is a logical choice to delay mortality, considering the known risks associated with limb amputation. Charcot foot is a challenging condition that requires the application of basic surgical principles and a team approach for successful management.

Management of High-Risk Ankle Fractures

Craig E. Krcal, David R. Collman

Summary: Deficiencies in bone strength, fracture healing, soft tissue resilience, and host comorbidities present challenges for treating unstable ankle fractures in elderly and diabetic patients. However, satisfactory outcomes can be achieved through rational application of internal fixation principles and early cast or brace protected weight bearing in select patient groups.
