Longer-term drug holiday, or combination of drug holiday and other prophylaxes, could be explored in future studies for prevention of MRONJ in patients with osteoporosis.

The analysis of 174 cancer patients revealed that treatment duration, chemotherapy, and hypertension were risk factors, while endocrine therapy and obesity were protective factors for MRONJ.

Summary: This study revealed that long-term use of bisphosphonates such as Zoledronic Acid (ZA) may lead to Bisphosphonate-related Osteonecrosis of the Jaw (BRONJ) by inhibiting macrophage efferocytosis and promoting osteocytic apoptosis. Additionally, supplementing with geranylgeraniol (GGOH) can restore Rac1 homeostasis and rescue macrophage efferocytosis, potentially providing a new treatment approach for BRONJ.

Further research is needed to confirm these findings.

Summary: This study investigated the effects of sex and treatment on the establishment of ONJ and bone biomechanical alterations in a ZA-related osteonecrosis mouse model. The results showed that ZA-treated groups had lower bone density and females had lower bone density compared to males. The ZA treatment decreased flexural modulus and yield stress but increased stiffness due to higher bone volume. Additionally, ZA increased crystallinity in bone and dental structure. In conclusion, ZA modifies the mineral matrix in bone and dental structure, improving bone mechanical properties despite the occurrence of osteonecrosis.
