Summary: The use of biomarkers in diagnosing myocardial infarction is well-established, but their value in identifying postoperative myocardial infarction after coronary artery bypass grafting is less understood. The release mechanisms of cardiac biomarkers, such as cTn, after CABG are not fully understood, making interpretation of postoperative increases in cTn concentrations complicated. Future studies are needed to determine the prognostic influence of biomarker release in general and postoperative myocardial infarction in particular.

Summary: This study conducted a two-stage meta-analysis using reconstructed individual patient data to compare long-term mortality between coronary artery bypass grafting (CABG) and percutaneous coronary intervention (PCI) for patients with multivessel disease (MVD) without left main (LM) involvement. The results showed that CABG had a lower cumulative all-cause mortality compared to PCI for MVD patients without LM involvement. However, in patients with moderate to high SYNTAX score, PCI had a significantly higher 10-year all-cause mortality compared to CABG.

Summary: This study aimed to analyze the in-hospital outcomes after coronary artery bypass grafting (CABG) in different age groups (<= 65 years and > 65 years) and found that age significantly influenced adverse events after CABG. In the on-pump CABG population, older patients had higher postoperative in-hospital mortality, incidence of postoperative symptomatic cerebral infarction (POSCI), and postoperative atrial fibrillation (POAF) compared to younger patients. In the off-pump CABG population, older patients had a higher incidence of POAF.

Summary: This study found that socioeconomic determinants of health influenced the choice between percutaneous coronary intervention (PCI) and coronary artery bypass grafting (CABG) for patients with coronary artery disease. Patients from areas with higher socioeconomic distress were more likely to receive PCI and had higher postprocedural mortality. Targeted resource allocation in these areas may help eliminate barriers to CABG.

Summary: Coronary artery revascularisation can be done through surgery or percutaneous intervention. Surgery has higher risks and longer recovery time, but offers long-term reduction of recurrent cardiac events. The choice between surgical or percutaneous intervention depends on clinical and anatomical reasons or personal preferences. Medical therapy is crucial in combination with coronary revascularisation, and some patients may achieve similar results with medical therapy alone. The existing revascularisation data mainly focus on white, non-elderly, male populations in high-income countries, so there is an urgent need for robust data in other populations.

Summary: This study investigated the association between proprotein-convertase subtilisin/kexin type 9 inhibitors (PCSK9i) and hyperglycaemia using the FDA adverse event reporting system (FAERS). The results showed that PCSK9i treatment was associated with increased reporting of mild hyperglycaemia, but not diabetes, in a real-world setting. PCSK9i had a more favorable glycaemic safety profile compared to statins.

Summary: Through pharmacovigilance study of the FDA's global database, it was found that SGLT2 inhibitors were significantly associated with adverse events such as amputations, Fournier gangrene, diabetic ketoacidosis, genitourinary infections, nocturia, dehydration, and fractures in both adults and older adults. However, these safety signals did not show a significant increase in older adults, thus indicating the safety of SGLT2 inhibitors for older adults.
